Oral Rehabilitation of a Patient With Ectodermal Dysplasia Treated With Fresh-Frozen Bone Allografts and Computer-Guided Implant Placement: A Clinical Case Report.

Ectodermal dysplasia (ED) is an inherited disorder characterized by abnormality of ectodermally derived structures. A recurrent oral finding is oligodontia, which in turn leads to a severely hypotrophic alveolar process with typical knife-edge morphology and adverse ridge contours. This unfavorable anatomy can seriously hamper proper implant placement. Fresh-frozen bone (FFB) allografts recently have been proposed to augment the residual bone volume for implant placement purposes; however, scientific evidence concerning the use of FFB to treat ED patients is absent. Similarly, data reporting c

A retrospective analysis of early and immediately loaded osseotite implants in cross-arch rehabilitations in edentulous maxillas and mandibles up to 7 years.

BACKGROUND: Immediate loading of full-arch restorations yields good results in selected cases, but long-term follow-up and the outcome in compromised bone are scarcely evaluated. PURPOSE: To evaluate immediately loaded Osseotite implants (Biomet 3i, Palm Beach, FL, USA) installed in healed or grafted bone, with regard to implant survival and peri-implant bone loss up to 7 years in function. MATERIALS AND METHODS: Information was retrospectively retrieved from 83 patients' records with 749 Osseotite implants supporting immediately loaded semipermanent full-arch acrylic restorations. A new method to eliminate the risk of maxillary sinusitis with zygomatic implants.

PURPOSE: A new approach for zygomatic implant placement was proposed to eliminate the risk of maxillary sinusitis related to the procedure. MATERIALS AND METHODS: A prospective study of this new approach was conducted, and consecutive patients treated between June 2007 and December 2008 were included. An extended sinus lift with retained bone window was performed, such that zygomatic implants were placed completely outside the displaced maxillary sinuses. All patients were followed up radiologically at regular intervals using cone beam computed tomography to evaluate the status of the zygomat

Augmentation of the atrophic edentulous mandible by a bilateral two-step osteotomy with autogenous bone graft to place osseointegrated dental implants.

Extensive resorption of the mandible increases the interarch space and rehabilitation with traditional dentures is often unsatisfactory due to the superficialization of intraoral muscles. A study of 19 patients who underwent augmentation of an atrophic mandible using a bilateral two-step osteotomy and interpositional bone graft technique is presented. Three horizontal bone cuts (one in the intraforamina and two in the molar region) were made and jointed together by two short vertical bone cuts mesialy to the mental nerve. The cranial fragment was lifted and the iliac bone graft was interposed

One-step oral rehabilitation by means of implants' insertion, Le Fort I, grafts, and immediate loading.

PURPOSE: In the last decade, several investigators have reported that autologous and homologous fresh frozen bones are effective materials to restore alveolar ridges before insertion of dental implants. Recently, we have used femur homograft derived from living donors. Here, we reported a 1-step oral rehabilitation of the severely resorbed maxilla by means of implants' insertion, Le Fort I osteotomy for maxillary advancement, grafts, and immediate loading. METHODS: Patients were treated with 1-step oral rehabilitation. Age, sex, implant length and diameter, tooth site, loading, and grafts wer

The influence of the bucco-palatal distance on sinus augmentation outcomes.

BACKGROUND: Maxillary sinus augmentation is one of the most reliable implant site development options to increase vertical bone height. However, graft consolidation requires adequate angiogenesis and migration of cells involved in osteogenesis and bone remodeling. It is speculated that these biologic events are greatly determined by the dimensions of the maxillary sinus cavity. Hence, the purpose of this study is to assess the influence of the distance from the lateral to the medial wall of the maxillary sinus on the outcomes of sinus augmentation procedures. METHODS: A total of 25 patients i

Endoscopic maxillary sinus lift without vestibular mucosal incision or bone graft.

AIM: To augment the thickness of the maxilla in cases with marked absorption of the alveolar bone of the upper jaw due to severe periodontal disease by performing a minimally invasive sinus lift procedure without an iliac bone graft. METHODS: An endoscope is inserted into the nasal cavity, and a large drainage opening is created in the inferior meatus extending from the maxillary sinus to the nasal cavity. The endoscope and forceps for mucosal removal are then inserted into the maxillary sinus via the drainage opening, and approximately, the lower half of the maxillary sinus mucosa is excised

Early implant placement with simultaneous guided bone regeneration following single-tooth extraction in the esthetic zone: 12-month results of a prospective study with 20 consecutive patients.

BACKGROUND: Early implant placement is one of the treatment options in postextraction sites in the anterior maxilla. Implant placement is performed after a soft tissue healing period of 4 to 8 weeks. Implant placement is combined with a simultaneous guided bone regeneration (GBR) procedure to rebuild esthetic facial hard and soft tissue contours. METHODS: In this prospective case-series study, 20 consecutive patients treated with an implant-borne single crown were prospectively followed for 12 months. Clinical, radiologic, and esthetic parameters were recorded to assess treatment outcomes. R

A randomized, controlled clinical trial to evaluate a new membrane for guided bone regeneration around dental implants.

OBJECTIVES: The use of barrier membranes in guided bone regeneration (GBR) procedures for the treatment of alveolar bone defects is common practice. The objective of this study was to test whether a synthetic bioresorbable polyethylene glycol (PEG) hydrogel membrane could result in a similar amount of vertical bone fill as a standard collagen membrane, both combined with a membrane supporting material. MATERIAL AND METHODS: The study enrolled 37 patients requiring implant treatment with an expected osseous defect in the posterior maxilla or mandible. After raising a mucoperiosteal flap, the i

Osteotome sinus floor elevation and simultaneous placement of implants--a 1-year retrospective study with Astra Tech implants.

BACKGROUND: The bone support for implants in the posterior part of the maxilla is often poor. This condition may be treated with augmentation of the maxillary sinus floor. The most common technique used is to elevate the sinus floor by inserting a bone graft through a window opened in the lateral antral wall, although less invasive techniques with osteotomes have been used since 1994. PURPOSE: The aim of this study was to evaluate the clinical and radiographic outcome of implants placed in the posterior maxilla with the osteotome sinus floor elevation (OSFE) technique without grafting. Clinical considerations for enhancing the success of implant-supported restorations in the aesthetic zone with delayed implant placement.

Several factors (e.g., root configuration, periapical pathoses, periodontal disease, severe bone loss, patient accessibility) determine the timing of the implant placement. When delayed implant placement is the only feasible option for implant restoration, achieving a functional and aesthetic restoration is more complex and challenging. This article will discuss the surgical and prosthetic methods for enhancing the success of an implant-supported crown in the aesthetic zone in cases where delayed implant placement is the only available treatment option for patients missing single teeth.

Demineralized Dentin Matrix for Dental and Alveolar Bone Tissues Regeneration: An Innovative Scope Review.

BACKGROUND: Dentin is a permeable tubular composite and complex structure, and in weight, it is composed of 20% organic matrix, 10% water, and 70% hydroxyapatite crystalline matrix. Demineralization of dentin with gradient concentrations of ethylene diamine tetraacetic acid, 0.6 N hydrochloric acid, or 2% nitric acid removes a major part of the crystalline apatite and maintains a majority of collagen type I and non-collagenous proteins, which creates an osteoinductive scaffold containing numerous matrix elements and growth factors. Therefore, demineralized dentin should be considered as an exc

Efficacy of Bioresorbable Mesh Sheets Made of 85:15 Poly(lactic-co-glycolic acid) in the Anterior Maxilla for Alveolar Bone Augmentation.

This study evaluated the efficacy of poly(lactic-co-glycolic acid) (PLGA) mesh sheets for alveolar bone reconstruction in the anterior maxilla with significant bone defects. This prospective case series included 4 male patients (mean age, 34.5 years; range, 20-49 years) with anterior maxillary bone defects. Bone augmentation was performed using a staged guided bone regeneration technique with PLGA mesh sheets as barrier membranes and a 1:1 mixture of autogenous and xenogeneic bone as grafting material. The effectiveness of bone augmentation was evaluated using preoperative and postoperative co

Sinus floor elevation using osteotomes or piezoelectric surgery.

The aim of this paper is to describe a technique for sinus floor augmentation with a 1-step crestal approach where the residual bone is ≤ 7.5mm. 36 implants were installed in 25 patients in the atrophic posterior maxilla immediately after sinus floor elevation. Sinus floor elevation was performed with a crestal approach using either osteotomes and burs or piezosurgery. Standardized intraoral radiographs were taken prior to surgery and 1 year after surgery. The mean residual bone height was 5.61 mm (range 3-7.5mm). The mean gain of sinus elevation was 6.78 mm (range 3.5-10mm) at 1 year after su

Transcrestal sinus floor elevation with a minimally invasive technique.

BACKGROUND: Recently, we proposed a minimally invasive technique to limit the postoperative morbidity of transcrestal sinus floor elevation procedures. The technique is based on the use of specially designed drills and osteotomes. The purpose of the present study is to present data on the clinical outcomes and postoperative morbidity of sinus floor elevation procedures performed using the proposed technique. METHODS: Fourteen implants were placed in the posterior portions of the maxilla areas of 11 patients using the proposed technique. Postoperative pain and discomfort were assessed using a

Immediate loading of dental implant after sinus floor elevation with osteotome technique: a clinical report and preliminary radiographic results.

Edentulous ridges in the posterior maxilla are often compromised by reduced bone volume. This anatomic condition often limits dental implant placement of 10 mm in length without prior or simultaneous sinus augmentation. The osteotome technique is an alternative and conservative technique for sinus floor augmentation and immediate implant placement in the posterior region of the maxillary jaw. According to the relevant literature, the osteotome technique appears to be a predictable and safe method for augmenting bone at the sinus floor and to improve bone density and quality of the implant site

Maintenance of class III trifurcated molars versus implant placement in regenerated extraction sockets: long-term results of 2 cases.

Studies to date have reached differing conclusions regarding the long-term prognosis of teeth with class III furcation involvement. Replacement of such teeth with implants could be an alternative. This report compares the treatment outcomes of 2 cases with similar disease progression: 1 treated by implant therapy and 1 maintained with nonsurgical periodontal treatment. Two patients with advanced chronic periodontitis and class III furcation involvement of all molars were treated. Case 1 received a conservative periodontal and antibiotic treatment, followed by 15 years of maintenance. In case 2

Simplified osteotome sinus augmentation technique with simultaneous implant placement: a clinical study.

The aim of this study was to describe and evaluate a modified maxillary sinus floor augmentation technique: the simplified osteotome technique. The study consisted of 26 patients treated with 39 Brånemark implants (Nobel Biocare) placed using the simplified osteotome technique between September 1997 and November 2004 (87 months). Implant length ranged from 10 to 15 mm, while the loading time ranged from 5 to 74 months (mean: 35.2 months). The success rate was 97.4% according to Albrektsson's criteria. These preliminary data indicate that the simplified osteotome technique is effective and safe
