Drug-Induced Anaphylaxis in Children: A Comparative Analysis of Inpatient versus Outpatient Settings.

INTRODUCTION: Studies specifically focusing on drug-induced anaphylaxis (DIA) in children are few. There is no study comparing inpatient (IP) and outpatient (OP) settings in DIA, either in children or in adults. We aimed to compare the characteristics of pediatric DIAs observed in IP and OP settings. METHODS: Patients aged ≤18 years who were diagnosed with DIA at our institution over the past 15 years were included in the study. The diagnosis of DIA was made clinically, and diagnostic testing was performed in eligible patients. Patients were categorized as IP and OP groups based on the settin…

Severe recurrent anaphylaxis: investigating underlying mechanisms beyond IgE-mediated allergy - a case report.

Anaphylaxis is a sudden, life-threatening, and generalized allergic reaction that develops within minutes to hours following exposure to an allergen. Cutaneous, respiratory, and cardiovascular symptoms may occur in various combinations, and the severity of the reaction can vary, potentially leading to anaphylactic reactions, which, if untreated with epinephrine, may be fatal. Although anaphylaxis is most commonly caused by allergic mechanisms, cofactors and comorbidities can contribute to increased severity. We present the case of a 34-year-old man who repeatedly experienced severe anaphylacti…

Anaphylaxis during hemodialysis associated with a polysulfone membrane: evidence from basophil activation testing.

Dialyzer-associated anaphylaxis is an uncommon but serious complication of hemodialysis. Although severe anaphylaxis is rare, milder hypersensitivity reactions are likely underrecognized and frequently misattributed to cardiopulmonary or volume-related causes. In clinical practice, the diagnosis is often inferred indirectly from symptom resolution after dialyzer substitution rather than confirmed by immunologic testing. Serum tryptase is widely used as a marker of mast cell activation in anaphylaxis; however, its diagnostic utility is limited in patients with chronic kidney disease (CKD), in w…

Palonosetron-induced anaphylaxis during general anesthesia in a patient with multiple drug hypersensitivity.

BACKGROUND: Palonosetron, a second-generation 5-HT3 receptor antagonist, is widely used for postoperative nausea and vomiting (PONV) prophylaxis. Anaphylaxis induced by palonosetron during general anesthesia is extremely rare, and the risk of severe anaphylaxis upon re-exposure in polysensitized atopic patients is inadequately recognized. CASE REPORT: A 50-year-old female underwent an uneventful hepatectomy, receiving palonosetron without adverse reaction. Ten weeks later, during pre-induction for laparoscopic rectal cancer surgery, she received oxycodone, palonosetron, and penehyclidine hydr…

Zingerone Targets LKB1/AMPK to Block FcεRI-Dependent Mast Cell Degranulation and Anaphylaxis.

AMP-activated protein kinase (AMPK) acts as a cellular energy sensor and a central regulator of metabolism. Recent studies indicate that pha … x through LKB1/AMPK activation. In vivo, Zingerone (25-50 mg/kg, oral) attenuated passive cutaneous anaphylaxis (reduced Evans blue extravasation) and systemic anaphylaxis (inhibited histamine/LTC4/PGD2 release). These findings demonstrate that Zingerone inhibits FcεRI-dependent mast cell activation and anaphylaxis via the LKB1/AMPK pathway, highlighting its …

Molecular Diagnosis to IgE-mediated Wheat Allergy and Wheat-Dependent Exercise-Induced Anaphylaxis.

IgE-mediated wheat allergy is an emerging problem worldwide, particularly prevalent in Northern Europe and parts of Asia. Another unique manifestation of wheat allergy, wheat-dependent exercise-induced anaphylaxis (WDEIA), and hydrolyzed wheat protein-induced urticaria/anaphylaxis/WDEIA, has increasingly been reported in recent decades. Major wheat protein allergens are classified into two main categories: water/salt-soluble proteins (e.g., alpha-amylase inhibitors, lipid transfer proteins (LTP), and avenin-like proteins) and alcohol/diluted acid-soluble proteins (e.g., gliadins and glutenins)…

Clinical characteristics and management of pediatric egg-induced anaphylaxis: A cross-sectional study.

BACKGROUND: Egg is the third most common food allergy in children; however, data on pediatric egg-induced anaphylaxis are sparse. OBJECTIVE: To describe the clinical characteristics, management, and outcomes of pediatric egg-induced anaphylaxis. METHODS: Children presenting with anaphylaxis were recruited from 13 emergency departments as part of the Cross-Canada Anaphylaxis Registry, from which data on anaphylaxis triggered by egg were extracted. Multivariate logistic regression was used to determine factors associated with prehospital epinephrine autoinjector (EAI) use and to compare anaphy…

Anaphylaxis mortality in the perioperative setting: Epidemiology, elicitors, risk factors and knowledge gaps.

Perioperative anaphylaxis (PA) is a severe condition that can be fatal, but data on PA mortality are scarce. The aim of this article is to review the epidemiology, elicitors and risk factors for PA mortality and identify knowledge gaps and areas for improvement regarding the management of severe PA. PA affects about 100 cases per million procedures. Mortality is rare, estimated at 3 to 5 cases per million procedures, but the PA mortality rate is higher than for other anaphylaxis aetiologies, at 1.4% to 4.8%. However, the data are incomplete. Published data mention neuromuscular blocking agents…

A clinical case of anaphylaxis after eating oatmeal contaminated with booklice (Liposcelis bostrychophila).

A 52-year-old man presented to our department with generalized erythema, watery stools, and vomiting 30 min after ingesting coconut, oatmeal, and vegetable juice. On arrival, his blood pressure was 120/79 mm Hg, heart rate was 126 beats per min, blood oxygen saturation was 96%, and lip cyanosis was observed. The patient experienced diffuse redness throughout the body and was diagnosed as having severe anaphylaxis. A skin prick test for the leftover coconut, oatmeal, and vegetable juice, which the patient had consumed immediately before the onset of symptoms, showed a positive reaction only to …

Successful Management and Birth After Perimortem Cesarean Delivery and Stillbirth Due to Anaphylaxis.

Perimortem cesarean delivery is an effective procedure for cardiopulmonary resuscitation during pregnancy. However, there are no reports documenting long-term outcomes in perimortem cesarean delivery survivors. This may be the first report of a successful live birth, occurring two years after perimortem cesarean delivery. A 29-year-old primipara was transferred to the emergency center on account of cardiopulmonary arrest, at 33 weeks of gestation. She was resuscitated 47 min after cardiopulmonary arrest by perimortem cesarean delivery amongst other treatment modalities, although the fetus died

A rare case of echo enhancing agent anaphylaxis complicated by acute coronary syndrome.

Enhancing agents in echocardiography are used frequently to improve endocardial border visualization and assessment of structural heart disease. We present a unique case of anaphylactic shock with acute coronary syndrome following administration of sulfur hexafluoride echo enhancing agent. This case emphasizes the importance of recognizing anaphylaxis to enhancing agents, as well as recognizing the potential relationship between anaphylaxis and acute coronary syndrome with in-stent thrombosis.

Clonal mast cell disorders and hereditary α-tryptasemia as risk factors for anaphylaxis.

The association between Hymenoptera venom-triggered anaphylaxis (HVA) and clonal mast cell-related disorders (cMCD) has been known for decades. However, recent breakthroughs in peripheral blood screening for KIT p.D816V missense variant have revealed the true extent of this clinical association whilst adding to our understanding of the underlying aetiology. Thus, recent large studies highlighted the presence of KIT p.D816V among 18.2% and 23% of patients with severe Hymenoptera venom-triggered anaphylaxis. A significant proportion of those patients have normal serum basal tryptase (BST) levels…

Anaphylaxis After Consumption of Guar Gum-Containing Food: A Report of Two Cases.

Guar gum is a food additive that acts as a thickening agent. Although the relationship between guar gum and occupational rhinitis/asthma has already been established, only very few cases of anaphylaxis were associated with guar gum ingestion. We present two examples of anaphylaxis induced by guar gum. Both cases were successfully treated with adrenaline. Moreover, serum-specific immunoglobulin E (IgE) with the culprit agent was detected in blood samples. To the best of our knowledge, here we present the first case of class 6 guar gum-specific IgE-positive anaphylaxis. This report aims to raise…
