Outcome of hypospadias reoperation based on preoperative antimicrobial prophylaxis.

From August 1981 to June 2004, we retrospectively reviewed 66 hypospadias, treated with 123 operations, to analyze the outcome of reoperatio … la in following reoperations (66.7%, p = 0.043). Of the 123 repairs, only 46 (37%) had preoperative antibiotic prophylaxis, and these had lower early complication rates, both in primary and reoperation groups (42.3% vs 52.5% and 40.0% vs 54.1%, respectively). The differences, however, were not statistically significant (p = 0.231 and p = 0.289, respectively). In terms o …

The patient with a heart murmur: evaluation, assessment and dental considerations.

BACKGROUND: Heart murmurs, a common finding in dental patients, are of major concern to dental professionals because certain dental procedur … sionals and results in improved patient care. CLINICAL IMPLICATIONS: Beyond the need to administer antibiotic prophylaxis, the dentist also needs to address the underlying causes of a patient's heart murmur. By providing dental care to such patients, oral health care providers become part of the patient's overall health care team.

Postoperative endophthalmitis.

Postoperative endophthalmitis remains a serious clinical problem in ophthalmology, with an incidence of approximately 0.5%. Prognosis is largely determined by the virulence of the offending organism. The Endophthalmitis Vitrectomy Study (EVS) was a prospective, randomized trial comparing various diagnostic and treatment modalities in cases of endophthalmitis that followed cataract surgery. The EVS found that vitrectomy was only beneficial for patients presenting with very poor visual acuity and that intravenous antibiotic treatments had no additional benefit, compared with intravitreal antibio

Effectiveness of antibiotics in preventing meningococcal disease after a case: systematic review.

OBJECTIVE: To summarise the evidence for the role of antibiotics in preventing further cases of meningococcal disease through chemoprophylaxis given to the index patient, household contacts, and children in day care settings after a single case. DESIGN: Systematic review. METHODS: Studies were identified by searching Embase (1983-2003), Medline (1965-2003), and CAB Health (1973-2003) and by contacting the World Health Organization and the European meningococcal disease surveillance network and examining references of identified papers. The review included all studies with at least 10 cases i

[Borrelia in ticks biting humans and prevention of Lyme borreliosis].

We examined by dark field microscopy 195 of 209 Ixodes ricinus ticks that were removed from humans in the Czech Republic (mainly in southern Moravia) during 1997-2001. The majority of the ticks were nymphs (62%), 31% were females and 7% larvae; 10.7% of the nymphs, 20.3% of the females but no larvae were found to be infected with B. burgdorferi s.l., the agent of Lyme borreliosis (LB). This observation supports for Central Europe the conclusions drawn from North America about nymphal ixodid ticks as the most important vector stage in transmission of LB. In the course of the year, we found the

[Infectious complications of lymphedema].

Erysipelas and lymphangitis are frequent complications of lymphedemas (20 to 30%). The most important risk factor for erysipelas is lymphedema since this is a protein rich edema that contributes to the risk of infection. In case of lymphedema the treatment is the usual consensus treatment for erysipelas. A prophylactic treatment with penicillin is requested as soon as the first recurrence. This prophylactic treatment includes skin care, particularly treatment of injuries and intertrigos. Hyperplastic skin leads to maceration and then mycoses. Physiotherapy does not increase the risk for infect

Update of guidelines for the prevention and treatment of infection in patients with an absent or dysfunctional spleen.

Guidelines for the prevention and treatment of infection in patients with an absent or dysfunctional spleen were first published by the British Committee for Standards in Haematology in 1996. Key aspects of these guidelines related to anti-infective prophylaxis, immunisation schedules and treatment of proven or suspected infection. A recent review of the guidelines was undertaken, with a view to updating the recommendations where necessary The guideline review process did not reveal any major change in patient groups considered at risk. Occupational exposure to certain pathogens may, however,

Prophylactic antibiotics in plastic and reconstructive surgery.

There is no consensus in the literature on the use of prophylactic antibiotics to prevent postoperative infection. This study was performed … ecimens were obtained from patients who had wound infections. According to our clinical experience, antibiotic prophylaxis is not necessary in plastic surgery. At the end of our 6-year study, a significant difference could not be found between the antibiotic prophylaxis and placebo groups.

Orthopaedic implant infections: current management strategies.

Orthopaedic joint replacement and spinal stabilisation surgery has provided pain relief and functional improvement for millions. Postoperative infections involving the implanted hardware can have devastating functional, emotional and financial consequences. Implementation of relevant preoperative patient preparation, utilisation of appropriate prophylactic antimicrobials and intraoperative infection control measures have combined to diminish orthopaedic implant infection frequency significantly. Our adeptness at establishing the presence of an implant-related infection with certainty is not as

Duration of antibiotic treatment in surgical infections of the abdomen. Undesired effects of antibiotics and future studies.

Effects of antibiotic administration other than bacterial killing are common and include drug interactions, toxic, and allergic reactions. Effective antibiotics do release endotoxin from enteric bacteria, but the clinical significance of this effect is uncertain, and present knowledge does not support alteration of clinical practice based on this information. The duration of antibiotic administration that is necessary for prophylactic purposes is short, probably 12 hours or less in all cases. The duration of administration that is necessary for treatment of established infections is not known
