A 36-year-old male patient, who was admitted with atypical chest pain and had normal physical examination and laboratory findings, including on an echocardiogram and a maximal exercise treadmill/thallium test, is described. His electrocardiograms (ECGs) revealed ST-segment elevation in leads V(1) through V(1), suggestive of early repolarization variant (ERPV); in addition, there was evidence of accelerated atrioventricular conduction (short PR interval) and incomplete right bundle branch block (IRBBB). It is postulated that these 2 features represent ECG "correlates" of ERPV, which is characte

Radiofrequency catheter ablation (RFCA) has recently become a management option for pediatric tachycardia. We reviewed the records of a total of 100 patients (aged 10 months to 19 yr) who had undergone RFCA, from March 2000 to June 2004. Types of arrhythmia (age, acute success rate) were as follows: atrioventricular reentrant tachycardia (AVRT, 9.0+/-3.7 yr, 66/67), atrioventricular nodal reentrant tachycardia (AVNRT, 13+/-2.5 yr, 16/16), ectopic atrial tachycardia (6.4+/-3.3 yr, 5/5), junctional ectopic tachycardia (10 month, 1/1), ventricular tachycardia (12+/-4.9 yr, 6/6), postsurgical intr

OBJECTIVES: We sought to characterize a method of attenuating interatrial conduction using radiofrequency ablated lesions applied to the rig … arked conduction slowing or block during sinus rhythm and pacing. Ablation did not adversely affect atrioventricular node conduction, nor did it facilitate sustenance of an atrial arrhythmia. Pathologic analysis revealed that complete interatrial conduction block was associated with confluent ablation of both targeted zones.
CONCLUSIONS: Catheter ablation of the right …

Atrial tracking pacemakers may improve exercise capacity and symptoms because they maintain atrioventricular synchrony and preserve the physiological response of heart rate to exercise. A rate responsive pacemaker which reacts to physical activity may be effective in patients with sinus node disease who are unsuitable for VDD pacing. At least three months after implant a double blind randomised short and long term crossover study was performed in ten patients with complete heart block: block was present at rest and during exercise on a modified Bruce protocol. Symptoms were assessed on a visua

Baroreflex sensitivity was measured by the heart rate response to the transient rise in blood pressure induced by phenylephrine, in 11 patients with carotid sinus syndrome, 6 patients with sick sinus syndrome and nine age and blood pressure matched controls. Patients with carotid sinus syndrome were divided into those with a hypersensitive carotid sinus reflex manifest as sinus arrest (n = 9) and those with a reflex manifest as atrioventricular block (n = 2). The mean gain of the baroreflex sensitivity was significantly increased (p less than 0.001) in patients with carotid syndrome manifest a

POPDC2 encodes for the Popeye domain-containing protein 2 which has an important role in cardiac pacemaking and conduction, due in part to its cAMP-dependent binding and regulation of TREK-1 potassium channels. Loss of Popdc2 in mice results in sinus pauses and bradycardia and morpholino knockdown of popdc2 in zebrafish results in atrioventricular (AV) block. We identified bi-allelic variants in POPDC2 in 4 families that presented with a phenotypic spectrum consisting of sinus node dysfunction, AV conduction defects and hypertrophic cardiomyopathy. Using homology modelling we show that the ide

Atrioventricular nodal reentrant tachycardia (AVNRT) is one of the most common causes of paroxysmal supraventricular tachycardia. For many years, the pharmacological approach was the only therapeutic modality available for managing this arrhythmia. More recently transcatheter radiofrequency (RF) ablation has become a safe and effective alternative to medical therapy. During the last 2 years, 20 patients with AVNRT were evaluated in our department. The diagnosis was made using the classical electrophysiological protocols with three multipolar catheters placed in AV node-His region, coronary sin

Primary cardiac lymphoma (PCL) only rarely occurs and it is defined as a lymphoma in which the bulk of the tumor is located within the heart and pericardium. A 53-year-old woman was referred due to dyspnea, and an electrocardiogram exhibited atrial fibrillation (AF). Echocardiography revealed no abnormal findings. Scintigraphy and a lymph node biopsy led to a diagnosis of PCL. After the start of chemotherapy, AF was converted to atrial tachycardia prior to sinus rhythm with a first-degree atrioventricular block, which was finally restored to a normal sinus rhythm. PCL is only rarely encountere

BACKGROUND: Very slow atrioventricular nodal reentrant tachycardia (AVNRT) can mimic nonreentrant accelerated junctional rhythm (JR) or junctional tachycardia (JT).
OBJECTIVE: This study aimed to describe the clinical features and mechanism of slow AVNRT masquerading as accelerated JR or JT (collectively called JRs).
METHODS: We reviewed all AVNRT and JT ablations performed at Ascension St. Vincent Hospital, Indianapolis, from July 2016 to June 2024. Patients with a tachycardia cycle length (TCL) of ≥500 ms were analyzed, focusing on clinical characteristics and electrophysiological findings

Lacosamide, one of the last antiepileptic drugs marketed, can cause extension of PR interval. Precautions are recommended when used in elderly and with other drugs extending PR interval. Cases of severe third-degree atrioventricular block have been reported only in post-marketing case reports when used at high-doses and remain rare. We report the case of an 88-year-old woman treated with bisoprolol, who experienced a complete atrioventricular block after initiation of lacosamide for epilepsy associated with neurodegenerative disease. This dramatic event required a pacemaker implementation. Not

Dual atrioventricular nodal (DAVN) physiology has been reported in up to 63% of pediatric patients with anatomically normal hearts, yet atrioventricular nodal reentrant tachycardia (AVNRT) accounts for only 13%-16% of supraventicular tachycardia (SVT) in childhood. The incidence of AVNRT increases with age and becomes the most common form of SVT by adolescence. We investigated the age related electrophysiological responses to programmed atrial and ventricular stimulation in 14 pediatric patients who underwent intracardiac electrophysiological study prior to radiofrequency catheter ablation for

INTRODUCTION: Ozanimod is a sphingosine-1-phosphate (S1P) modulator that inhibits lymphocyte trafficking from lymph nodes to the circulation. It is approved by the US Food and Drug Administration (FDA) for the treatment of relapsing multiple sclerosis and most recently for the management of moderate-severe ulcerative colitis (UC).
AREAS COVERED: Here we review the status of drugs approved for moderate-severe UC, the unmet needs in the management of UC, proposed mechanisms of action of S1P modulators, clinical data regarding ozanimod in UC, and emerging S1P modulators being evaluated in inflam

In 1899, Karel F. Wenckebach unraveled the complicated arrhythmia that bears his name--one of the most famous eponyms in medicine. He reported his findings before the benefit of clinical electrocardiography or the discovery of the sinoatrial and atrioventricular nodes. Born and educated in The Netherlands, Wenckebach first worked in Utrecht in the physiology laboratory of T.W. Engelmann, his respected mentor, where he became familiar with kymographic recordings and rhythm disturbances in frog experiments. He then entered country practice in 1891, where he gained great respect for practicing ph…

BACKGROUND: Noninvasive visualization and characterization of the atrioventricular conduction axis (AVCA) can help avoid iatrogenic injury and improve the accuracy of heart rhythm therapy guidance.
OBJECTIVE: This study aimed to identify the AVCA structure using noninvasive rotating-frame relaxation-time mappings (relaxation along a fictitious field in the second rotating-frame relaxation time [TRAFF2] and longitudinal rotating-frame relaxation time [T1ρ]) in ex vivo swine hearts.
METHODS: Ex vivo swine heart tissue blocks were scanned on a 3T clinical magnetic resonance imaging system. The

Radiofrequency ablation of the cavotricuspid isthmus is the first-line treatment for typical atrial flutter. Despite the close proximity of the right coronary artery (RCA) to the cavotricuspid isthmus, only four cases of arterial injury have been reported during radiofrequency ablation, all detected postablation by inferior ST elevation. Here, we report atrioventricular (AV) conduction delay during coronary sinus pacing as a possible early sign of RCA involvement and review the previous literature on RCA damage and variations of AV nodal circulation.

Sonic hedgehog (Shh) is a secreted morphogen necessary for the production of sidedness in the developing embryo. In this study, we describe the morphology of the atrial chambers and atrioventricular junctions of the Shh null mouse heart. We demonstrate that the essential phenotypic feature is isomerism of the left atrial appendages, in combination with an atrioventricular septal defect and a common atrioventricular junction. These malformations are known to be frequent in humans with left isomerism. To confirm the presence of left isomerism, we show that Pitx2c, a recognized determinant of mor

Patients with advanced heart failure continue to experience high morbidity and mortality despite recent progress made with the use of such drugs as beta-blockers and angiotensin-aldosterone inhibitors. Cardiac transplantation has severe limitations due to the short supply of organs and the ineligibility of most CHF patients for this therapy. Approved heart-assist devices are cumbersome and costly. Therefore, these devices are currently used mainly in tertiary care centers in a limited number of patients. CRT has been rapidly evolving as a viable and beneficial therapy that is universally appli

Ebstein's anomaly is a rare congenital cardiac disease initially described by Wilhelm Ebstein in 1866. The primary pathology involves significant apical displacement of the septal tricuspid valve leaflet and the presence of a redundant, elongated, anterior tricuspid valve leaflet. This congenital malformation has multiple known associated morphological and electrophysiological cardiac pathologies. In the present article, we report, to the best of our knowledge for the first time in the literature, a case of Ebstein's anomaly with multiple accessory bypass tracts, dual AV nodal physiology, non-
