Many astronauts exhibit post-spaceflight orthostatic hypotension due to inadequate norepinephrine release when in an upright posture. We hypothesized that an alpha1-adrenergic agonist, midodrine, would be an effective countermeasure. A female astronaut, who had problems with postflight orthostatic hypotension after a previous flight, consumed 10 mg midodrine after a subsequent flight, prior to her tilt test. Hemodynamic variables were compared between the two flights. Midodrine prevented severe falls in stroke volume, cardiac output and systolic pressure, and severe increases in heart rate wit

INTRODUCTION: Exposure to spaceflight or simulations of microgravity reduce human postflight orthostatic tolerance. Exercise training and volume loading can reduce associated losses of plasma volume and muscle strength, but are not successful in maintaining postflight orthostatic tolerance. A preliminary study (16) indicated that short bouts of artificial gravity (AG) training on a centrifuge could increase orthostatic tolerance in healthy, ambulatory volunteers. We tested the same AG protocol for its tolerance effect on 14 men who underwent a 3-wk exposure to Gz acceleration training on NASA-

INTRODUCTION: There is disagreement regarding the impact of dietary sodium on alterations in extracellular volume during head-down bed rest (HDBR). The primary purpose of this study was to assess the effects of salt intake on extracellular volume (ECV) during HDBR.
METHODS: We performed whole-body bioimpedance spectroscopy with controlled sodium intake during 4 d of ambulation and 8 d of -6 degrees HDBR in 10 normotensive men. Each subject performed an initial 12-d familiarization run with moderate sodium (246 +/- 12 mmol x L(-1) x d(-1) excreted) during which no measurements were made. They

BACKGROUND: Future fighter aircraft will include three-dimensional sound signals as part of the human-machine interface. The reduction in cerebral vascular flow associated with maneuvering acceleration (+Gz) may affect a pilot's ability to perceive and interpret such aural cues. We hypothesized that vascular deprivation along the cochlea produced by +Gz would raise hearing thresholds either globally or specifically at 1000 Hz.
METHODS: We compared hearing thresholds for pure tones at 250, 1000, 6000 and 10,000 Hz during exposure to +1 Gz vs. +4 Gz. Experiments were conducted with steady noise

Motion sickness is not only elicited by certain kinds of self-motion, but also by motion of a visual scene. In case of the latter, optokinetic drums are often used and a visual-vestibular conflict is assumed to cause the sickness. When the rotation axis is Earth vertical however, different studies show different results. Here, we propose that visual-vestibular conflicts per se do not cause sickness whereas subjective vertical mismatch theory can reconcile the disparate findings. The theory attributes the nausea induced by horizontal optokinetic stimulation to the subjects self-inducing pseudo-

INTRODUCTION: The purpose of this study was to examine whether acupressure and acustimulation prevent motion sickness, taking into consideration whether or not the acupressure and acustimulation are administered properly. These techniques claim to reduce nausea through stimulation of the P6/Neiguan acupuncture point by applying acupressure or electrical acustimulation.
METHODS: The Acuband and ReliefBand were used to administer acupressure and acustimulation, respectively. There were 77 subjects who were assigned to 1 of 5 conditions: Acuband trained or untrained; ReliefBand trained or untrai

Square-wave jerks (SWJs) are small, involuntary saccades that disrupt steady fixation. We report the case of an astronaut (approximately 140 d on orbit) who showed frequent SWJs, especially postflight, but who showed no impairment of vision or decrement of postflight performance. These data support the view that SWJs do not impair vision because they are paired movements, consisting of a small saccade away from the fixation position followed, within 200 ms, by a corrective saccade that brings the eye back on target. Since many returning astronauts show a decrement of dynamic visual function du

BACKGROUND: It is commonly assumed that car passengers who suffer from motion sickness will benefit from being able to see the road ahead.
HYPOTHESIS: It was hypothesized that variations in the external view (including restrictions to forward view and the provision of an artificial real-time video view) would affect the development of sickness during a 30-min suburban car journey.
METHOD: Motion sickness was investigated in 4 experiments using 15 different conditions, with 20 subjects in each condition. The conditions involved artificial restrictions to view, restrictions arising from differ

UNLABELLED: Sopite syndrome is a disturbance caused by motion characterized by drowsiness and mood changes that may occur without the classic symptoms of motion sickness. Because sopite symptoms are often subtle, it is important to assess for their presence, and to distinguish true sopite symptoms from changes in mood and drowsiness that are due to non-motion related factors such as fatigue and boredom. We compared sopite symptoms in the optokinetic drum (OKD) with symptoms in a control condition in which the drum's motion cues were removed.
METHODS: There were 48 Introductory Psychology stud

BACKGROUND: Pilots who fly jet fighters or helicopters frequently experience vertebral problems due to acceleration and vibration, wearing helmets and other headgear, and sitting in suboptimal postures.
METHODS: We looked for spondylarthritic or spondylitic changes in 4-view radiographs (AP, lateral, left and right oblique) of the cervical and lumbar vertebrae of 732 pilots and 202 non-flying controls. The subjects included 91 F-16 jet pilots, 363 other jet pilots, 119 transport pilots, and 159 helicopter pilots.
RESULTS: The prevalence of cervical changes in helicopter pilots was 19%, signi

Three otherwise healthy private pilots were licensed with the restriction to fly "as or with safety pilot" because of a left bundle branch block (LBBB). Whether a pilot with this conduction disturbance is allowed to fly unrestricted depends on invasive diagnostic procedures. Above age 40, cardiovascular risk in subjects with LBBB becomes high enough that a flying license should be withheld. A protocol for licensing in LBBB cases under age 40 is given, relying on non-invasive diagnostic procedures, taking into account intermittent occurrence, electrical axis, and duration of QRS complex.

INTRODUCTION: We hypothesized that repeated respiratory straining maneuvers (repeated SM) designed to elevate arterial BPs (arterial baroreceptor loading) would acutely increase baroreflex responses.
METHODS: We tested this hypothesis by measuring cardiac baroreflex responses to carotid baroreceptor stimulation (neck pressures), and changes in heart rate and diastolic BP after reductions in BP induced by a 15-s Valsalva maneuver in 10 female and 10 male subjects at 1, 3, 6, and 24 h after performing repeated SM. Baroreflex responses were also measured in each subject at 1, 3, 6, and 24 h at t

Recent evidence has identified a risk of neck injury resulting from exposure to high sustained G force during operation of fast jet military aircraft. This review considers the risk of acute soft tissue neck injury when exposure to acceleration is unexpected by the individual. In a military jet, this might occur to a back seat occupant such as an instructor or weapons operator/navigator, but unexpected forces may also be encountered by roller coaster riders and human centrifuge users under certain conditions. The mechanisms of injury in this situation are examined, the effect of acceleration a

An understanding of the consequences of sustained operations involving prolonged sleep deprivation is important to the military. Losses in cognitive performance of the order of 30% after one night and 60% after two nights of sleep loss have been shown to occur in several studies. Napping strategies have been proposed as one coping strategy for these performance decrements. An alternate solution is the use of stimulants. Modafinil may offer a safer alternative to more commonly used psychostimulants. It has been shown to counteract the effects of sleep deprivation with fewer side effects than am

BACKGROUND: Under optokinetic drum conditions, a stationary participant views the patterned interior of a rotating drum. Quickly, most participants perceive illusory self-rotation in the direction opposite to the drum's true rotation (vection). It has been documented that up to 60% of participants experience motion sickness-like symptoms under optokinetic conditions perhaps because of conflicting sensory information from the visual and vestibular systems.
METHODS: Keeping rotation speed constant (10 RPM), drum tilt relative to the axis of rotation was systematically manipulated (0 degrees, 5

BACKGROUND: In many environments associated with motion sickness there are low frequency motions in several axes, including roll. Roll motion has often been assumed to be a cause of motion sickness, either alone or in combination with motions in other axes. However, there have been no systematic studies of the effects of roll frequency on sickness.
HYPOTHESIS: It was hypothesized that sickness caused by roll oscillation would depend on the frequency of roll.
METHOD: There were 100 male subjects (aged 18 to 26 yr) who participated in a laboratory study. Prior to experiencing motion, all subje

INTRODUCTION: The U.S. Navy is considering placing flight simulators aboard ships. It is known that certain types of flight simulators can elicit motion adaptation syndrome (MAS), and also that certain types of ship motion can cause MAS. The goal of this study was to determine if using a flight simulator during ship motion would cause MAS, even when the simulator stimulus and the ship motion were both very mild.
METHODS: All participants in this study completed three conditions. Condition 1 (Sim) entailed "flying" a personal computer-based flight simulator situated on land. Condition 2 (Ship)

BACKGROUND: It is generally understood that whole-body movement up and down can produce motion sickness. A standard reference, published in this journal in 1974, found that a frequency of 6 cycles per minute was most productive of motion sickness. The 1974 study used a vertical oscillator carrying two subjects at a time. The investigators were, of course, aware that the sight, sound, or smell of another person being ill might itself contribute to sickness. Accordingly, the two subjects were located in completely separate compartments, each soundproofed and with its own independent air-conditio
