Effects of microenvironment in osteosarcoma on chemoresistance and the promise of immunotherapy as an osteosarcoma therapeutic modality.

Osteosarcoma (OS) is one of the most common primary malignant tumors originating in bones. Its high malignancy typically manifests in lung metastasis leading to high mortality. Although remarkable advances in surgical resection and neoadjuvant chemotherapy have lengthened life expectancy and greatly improved the survival rate among OS patients, no further breakthroughs have been achieved. It is challenging to treat patients with chemoresistant tumors and distant metastases. Recent studies have identified a compelling set of links between hypoxia and chemotherapy failure. Here, we review the ev

Premature physeal closure after tibial diaphyseal fractures in adolescents.

Seven cases of premature physeal closure secondary to diaphyseal fractures of the tibia in adolescents between 12 and 15 years of age are presented. At the time of the accidents, there was no evidence of physeal lesion in any of the patients. After 4 to 13 months of follow-up (mean, 9 months), early closure was observed in the radiologic controls of one or more physes of the affected leg: distal femoral and proximal and distal tibial in three cases, isolated distal femoral physis in three cases, and both tibial physes without femoral damage in one case. Physeal closure was always central, and

Complications of the Grice-Green operation.

A case presentation involving complications resulting from errors in surgical technique with the Grice-Green procedure is discussed in this report. The patient was treated 18 years after bilateral tendo achillis lengthenings and Grice-Green extra-articular subtalar arthrodeses with autogenous tibial bone grafts. The complications encountered were severe adductovarus deformity of the feet with multiple painful plantar callosities, in-toed gait with weightbearing on the lateral aspect of the feet, inversion ankle instability, and tripping over the feet. Deformity and symptoms were worse on the r

An Alternative Treatment Option for Blauth III B Thumb Hypoplasia-Thumb Stabilization with Iliac Crest Bone Graft and Intermetacarpal Arthrodesis.

Thumb hypoplasia modified Blauth III B is usually treated by pollicization or, less commonly, by toe transfer. Both procedures always result in the resection of a body part, but with good cosmesis and acceptable function. We describe an intermetacarpal I/II arthrodesis with autologous bone graft augmentation to lengthen and stabilize the loose thumb. Clinical data were collected from nine patients, median age at surgery 3 years 8 months, with more than 7 years of follow-up. The results showed a grip strength on the Jamar dynamometer of approximately 61% of the unoperated hand. The Quick-DASH s

[Motor nerves of the face. Surgical and radiologic anatomy of facial paralysis and their surgical repair].

Motor innervation of the face depends on the facial nerve for the mobility of the face, on the mandibular nerve, third branch of the trigeminal nerve, which gives the motor innervation of the masticator muscles, and the hypoglossal nerve for the tongue. In case of facial paralysis, the most common palliative surgical techniques are the lengthening temporalis myoplasty (the temporal is innervated by the mandibular nerve) and the hypoglossal-facial anastomosis. The aim of this work is to describe the surgical anatomy of these three nerves and the radiologic anatomy of the facial nerve inside the

Orthopedic aspects of skeletal dysplasia in children.

Significant advances in the management of skeletal dysplasia have occurred during the past year. A revision of the Paris classification of constitutional disorders of bone has been proposed. New mutations have been identified in several disorders that confirm genetic heterogeneity within similar phenotypes. New clinical data of growth, fracture rates, and incidence of major skeletal and extraskeletal changes as well as the natural history and treatment of spinal deformities in osteogenesis imperfecta have been reported. Pilot studies of short-term growth hormone therapy in patients with achond

Safety range for acute limb lengthening in primary total hip arthroplasty.

PURPOSE: There is no documented maximum amount that an extremity can be safely lengthened in primary total hip arthroplasty (THA) without neurologic or soft tissue complications. We retrospectively reviewed patients who underwent primary THA with acute limb lengthening and investigated the safety range for acute limb lengthening in primary THA. METHODS: This study included 61 hips in 52 patients who underwent primary THA with acute limb lengthening (more than 2.5 cm) without femoral shortening osteotomy. The amount of lengthening was measured from pre-operative and post-operative X-ray films

[Ilizarov techniques of leg lengthening. Problems and results].

Leg lengthening procedures have developed rapidly in the last 20 years, especially distraction epiphyseolysis and corticotomy. We have used both methods since 1977 and carried out 22 distraction epiphyseolyses (20 patients) and 33 corticotomies (28 patients). The average lengthening distance was 8.25 cm and 7.8 cm respectively (minimum 4 cm, maximum 18 cm). The time per cm lengthening (lengthening index) was 45.8 days/cm for epiphyseolysis and 52.2 days/cm for corticotomy. The complication rates were similar in both methods (1.18 and 1.21 pro lengthening procedure). Both techniques are prefera

Deformity progression in congenital posteromedial bowing of the tibia: a report of 44 cases.

BACKGROUND: congenital posteromedial bowing of tibia (CPMBT) is a very rare birth defect, characterized by shortened bowed leg and ankle deformity. We described a single institution experience in the management of CPMBT. METHODS: we identified 44 CPMBT in 44 children. The age at presentation was 5.5 ± 5.6 years and the mean age at the final review was 10.1 ± 4.8 years. Radiographic evaluation included the antero-posterior and lateral inter-physeal angle (AP-IPA and L-IPA), the limb length discrepancy (LLD), the morphology of the distal tibia and the lateral distal tibial angle (LDTA). Humeral lengthening in patients with achondroplasia and in patients with post-septic shortening: comparison of procedure efficiency and safety.

PURPOSE: Although humeral lengthening in patients with achondroplasia is an accepted procedure for improving functional status, there is still a paucity of information about the effectiveness of the method. Therefore, the aim of this study was to evaluate the efficacy and safety of humeral lengthening using monolateral fixators in patients with achondroplasia and unilateral shortening. METHODS: Twenty-one patients (31 humeri) were included in this study. The study group consisted of eight patients with achondroplasia (16 segments). The control group consisted of 13 patients with post-septic s

Forearm deformities in hereditary multiple exostosis: clinical and functional results at maturity.

BACKGROUND: Thirty to 60% of hereditary multiple exostoses patients have forearm deformities. There is no consensus regarding optimal therapy. This long-term retrospective study is the first to compare radiologic and clinical data with patient assessments, to define more precise surgical indications. METHODS: All children presenting with hereditary multiple exostose during the period from 1990 to 2010 were retrospectively analyzed. Masada forearm deformity classification, treatment, clinical course, and radiologic data were included from the preoperative period to the last follow-up. Operated

Pediatric deformity correction using a multiaxial correction fixator.

UNLABELLED: Circular fixators have been used successfully to correct multiplanar deformities but are often cumbersome and may be difficult to apply. We determined whether a monolateral fixator, which allows for correction of angular deformity and displacement in three planes, can correct lower extremity deformities to within normal radiographic means (anatomic lateral distal femoral angle, anatomic medial proximal tibial angle, and tibial femoral angle). We retrospectively reviewed the clinical records and radiographs of 22 consecutive patients (25 limbs) who underwent deformity correction usi

Functional outcome after lengthening with and without deformity correction in polio patients.

Poliomyelitis is one of the causes of limb length discrepancy. The aim of lengthening and deformity correction in such patients is to improve the functional mobility of the patient. This study aims to find out whether or not improvement of limb length inequality with or without deformity correction affects or improves ambulation. This prospective study included 32 skeletally mature patients managed using the Ilizarov technique and external fixation for limb lengthening with or without deformity correction. Functional Mobility Scale scoring was used for assessment of ambulation before lengtheni

[Mandibular micrognathism as a sequela of early childhood capitulum fractures and their treatment using distraction osteogenesis].

We report about our experience in five patients who suffered from a mandibular micrognathia after ancylosis of the temporomandibular joint. At first the ancylosis was removed using a costochondral graft. About one year later, the mandible was cut by an osteotomy behind the last visible toothbud and a distractor was inserted. The mandible was to a slide overcorrection. This has to be taken into account to compensate a certain relapse. At first we used an unidirectional device, for one and a half years we have used a bidirectional device (Normed from Tuttlingen/Germany) which enables us to corre

Calcaneo-Cuboid-Cuneiform Osteotomy for the Treatment of Planovalgus Feet in Patients with Spastic Cerebral Palsy.

BACKGROUND: The calcaneo-cuboid-cuneiform (triple-C) osteotomy is indicated for the correction of symptomatic flexible planovalgus foot deformity. This procedure allows correction of all of the varied components of the planovalgus foot deformity in a single operation1,2. DESCRIPTION: The patient is positioned in a floppy lateral position2. The calcaneus is exposed via an oblique lateral incision along the peroneal tendons. The osteotomy is performed in an extra-articular fashion beginning posterior to the posterior articular facet and extending distally and anteriorly to the inferior surface

Limb lengthening and deformity correction of congenital and acquired deformities in children using the Taylor Spatial Frame.

Background and purpose - Hexapod ring fixators such as the Taylor Spatial Frame (TSF) have shown good outcomes. However, there have only been a few studies comparing the use of TSF with various etiologies of the deformity. We compared the use of TSF in congenital and acquired deformities in children. Patients and methods - We reviewed 213 lower extremity reconstructive procedures with the TSF in 192 patients who were operated between October 2000 and October 2015. 128 procedures (67 proximal tibiae, 51 distal femora, and 10 distal tibiae) in 117 children (median age 14 (4-18) years; 59 girls)

Talocalcaneal tarsal coalitions and the calcaneal lengthening osteotomy: the role of deformity correction.

BACKGROUND: Surgical resection of persistently painful talocalcaneal tarsal coalitions may not reliably relieve symptoms in patients with large coalitions associated with excessive hindfoot valgus deformity and subtalar posterior facet narrowing. Since 1991, calcaneal lengthening osteotomy, with or without coalition resection, has been used at our institution to relieve symptoms and to preserve motion at the talonavicular and calcaneocuboid joints. METHODS: We retrospectively reviewed the records for eight patients with thirteen painful talocalcaneal tarsal coalitions who had undergone a calc

Surgical treatment of wrist deformity in hereditary multiple exostosis.

Wrist deformity represents a unique problematic entity in patients with Hereditary Multiple Exostosis (HME). We report our experience in the treatment of wrist deformities due to HME using three surgical procedures and the outcome of 12 wrist surgical corrections by comparing preoperative, postoperative and last follow-up radiographic values of the carpal slip, radial articular angle and ulnar shortening. Eight out of 12 forearms did show a postoperative improvement of the radiographic parameters, although recurrence of the deformity occurred frequently. Two forearms showed no change in postop

Hammer Toe Correction with Proximal Interphalangeal Joint Arthrodesis.

BACKGROUND: First described by Soule in 1910, arthrodesis of the proximal interphalangeal joint is a common operative method of treatment of hammer toe, or fixed-flexion deformity of the proximal interphalangeal joint of the lesser toes1. The deformity is often caused by imbalance in intrinsic and extrinsic muscle function across the interphalangeal joint and metatarsophalangeal joint2,3, which can be effectively addressed through proximal interphalangeal joint straightening and arthrodesis in conjunction with soft-tissue balancing of the metatarsophalangeal joint. DESCRIPTION: Following long

[Valgus osteotomy of the first metatarsal by a bone graft.].

The authors describe their experience of valgus osteotomy of the first metatarsal in hallux valgus - metatarsus primus varus (HV-MTPV). It is based on the Stamm technique consisting in the resection of one third of the base of the proximal phalanx of the great toe. From the resected part they make a wedge-shaped bone graft. Subsequently, they extend the section and perform a two-third transversal osteotomy of the first metatarsal (MT) 1 cm distal to the cuneometatarsal joint. The rest of the bone is broken into valgus position by pressure. Afterwards, the bone graft is inserted in the open ost
