Brain Tumor Vaccines: Next Steps.

High-grade gliomas remain among the most challenging malignancies despite extensive advances in molecular characterization and immunotherapy. This review highlights that while cancer vaccines consistently induce robust systemic immune responses, their clinical efficacy is limited by profound tumor heterogeneity and a highly immunosuppressive tumor microenvironment. Across peptide, dendritic cell, and mRNA-based platforms, outcomes are more strongly influenced by tumor biology than vaccine design. Emerging evidence supports a shift toward combinatorial strategies. Future progress will depend on

Right Brain: Language Without Speech.

During a neurology rotation as a second-year internal medicine resident, I encountered a preschool-aged child whose absence of spontaneous speech initially framed my clinical thinking. Although she did not answer direct questions, she demonstrated an unexpected facility with written language, reading words from books with ease and engaging with the world through visual and textual cues. Over subsequent encounters, I witnessed the gradual evolution of her communication and the adjustments made by her mother, whose daily life had become shaped by therapies, routines, and the uncertainties surrou

Neuropalliative care in brain cancer.

People with brain cancer experience substantial symptom burden, rapid functional decline, complex decision-making needs, and high care partner distress. Neuropalliative care offers a framework for integrating palliative care principles with neuro-oncology expertise across the disease trajectory, from diagnosis and treatment through recurrence, later deterioration, end-of-life care, and bereavement. This CME article summarises practical recommendations for embedding palliative care in routine brain cancer care, including early communication about goals and values, individualised symptom assessm…

Brain Fitness in Combat Sports.

Combat sports athletes inherently challenge their nervous system and put strain on its reserve. Brain fitness is a growing concept and field of research in neurology but particularly in sports neurology for the purpose of improving athlete performance and longevity. The ways in which we can optimize brain fitness of combat sports athletes involves the categories of safe training/rest schedules, prefight and postfight examinations, nutrition/vitamin supplementation, and hydration. When sports neurologists consider at least these areas, it contributes to safe participation for these neurological…

Brain Architecture of Punishment Learning.

Learning from punishment allows animals to suppress actions that produce adverse consequences while maintaining other rewarded behaviors. However, the brain mechanisms of this learning are poorly understood. Here, we combined instrumental behavioral analysis, whole-brain Fos mapping, spatial transcriptomics, computational network analysis, and chemogenetic inhibition in male and female mice. A within-subject yoking procedure showed that suppression depended on the instrumental response-punisher contingency rather than matched shock exposure or embedded pavlovian stimulus-shock relations. Whole…

Photobiomodulation for traumatic brain injury.

There is a notable lack of therapeutic alternatives for what is fast becoming a global epidemic of traumatic brain injury (TBI). Photobiomodulation (PBM) employs red or near-infrared (NIR) light (600-1300 nm) from lasers or LEDs to stimulate healing, protect tissue from dying, increase mitochondrial function, improve blood flow and tissue oxygenation. PBM can also act to reduce swelling, increase antioxidants, decrease inflammation, protect against apoptosis, and modulate microglial activation state. All these mechanisms of action strongly suggest that PBM delivered to the head should be benef…

[Emergent Surgery for Brain Tumors].

Brain tumors are relatively rare when compared with other malignancies. However, because the central nervous system governs the functions of the entire body, brain tumors can lead to significant morbidity and, ultimately, mortality if they are not treated appropriately and in a timely manner. These conditions are referred to as "neuro-oncologic emergencies," in which neurosurgeons play a central role in management. This review outlines the pathophysiological mechanisms underlying neuro-oncologic emergencies and discusses the role of emergent surgical intervention in supratentorial tumors, infr…

Contrastive learning in brain imaging.

Contrastive learning is a type of deep learning technique trying to classify data or examples without requiring data labeling. Instead, it learns about the most representative features that contrast positive and negative pairs of examples. In literature of contrastive learning, terms of positive examples and negative examples do not mean whether the examples themselves are positive or negative of certain characteristics as one might encounter in medicine. Rather, positive examples just mean that the examples are of the same class, while negative examples mean that the examples are of different

Brain interleukins and Alzheimer's disease.

The central nervous system (CNS) is immune-privileged by several immuno-modulators as interleukins (ILs). ILs are cytokines secreted by immu … eukins and their roles throughout the disease progression and interaction with other systems in the brain may provide promising therapeutic targets for the prevention or treatment of AD.

Malaria monoclonals block brain binding.

In Plasmodium falciparum malaria, infected cells accumulate in blood vessels of organs, including the brain. Recently, Reyes et al. identified monoclonal antibodies that stop infected cells from binding to the endothelial protein C receptor (EPCR) in a model of brain blood vessels. EPCR-blocking monoclonals could form the basis of new treatments for severe malaria.

Focused ultrasound for brain metastases.

Brain metastases (BMs) increasingly represent a significant cause of morbidity and mortality in cancer patients. The efficacy of systemic therapies for BMs, in contrast to extracranial metastases (EMs), remains limited secondary to a host of challenges. These include insufficient drug delivery due to the blood-brain barrier and blood-tumor barrier, the unique immunological milieu in the tumor microenvironment and cerebrospinal fluid, the diversity of immunogenomic landscapes in BMs across genetically distinct malignancies, the branching evolution of BM from EMs, and the challenges in longitudi…

Neuroglia and brain energy metabolism.

The glial control of energy homeostasis is of crucial importance for health and disease. Astrocytes in particular play a major role in controlling the equilibrium among adenosine 5'-triphosphate (ATP), adenosine 5'-diphosphate (ADP), adenosine 5'-monophosphate (AMP), and adenosine. Any energy crisis leads to a drop in ATP, and the resulting increase in adenosine is an evolutionary ancient mechanism to suppress energy-consuming activities. The maintenance of brain energy homeostasis, in turn, requires the availability of energy sources, such as glucose and ketones. Astrocytes have assumed an im…
