Nocardia asteroides cerebral abscesses are rare but challenging intracranial lesions. Early diagnosis, institution of appropriate antimicrobial therapy, lack of underlying systemic disease and limited intracranial disease are recognized factors leading to good outcome. However, the optimal treatment approach has not been established and nocardial brain abscesses have been managed either conservatively, with steroetactic aspirations or with open craniotomy and enucleation. We present three cases of Nocardia asteroides cerebral abscesses cured only after neurosurgical enucleation, and discuss th…

The acute onset of orbital and periorbital inflammation presents the clinician with a diagnostic and therapeutic dilemma, the consequence of which may be very serious. The progression from the initial stage of cellulitis to orbital abscess, cavernous sinus thrombosis, brain abscess, meningitis, or visual loss may be quite rapid at times. CT, ultrasonography, and MR imaging play an important role in clinical management of these patients. Indeed, it is the information obtained through orbital imaging that directs the clinician to use the correct therapeutic modality.

Brucellosis is a zoonosis that is transmissible to humans. It is a disease with multi-systemic involvement caused by the genus Brucella. Neurological complications, including meningitis, meningo-encephalitis, myelitis-radiculoneuritis, brain abscess, epidural abscess and meningo-vascular syndromes, are rarely encountered. We present a patient presenting with acute onset myositis. This kind of presentation has not previously been reported in the English language literature. We conclude that the diagnosis of neuro-brucellosis should be considered in patients presenting with muscle weakness.

Klebsiella pneumoniae is known to cause pneumonia, urinary tract infections, bacteremia, meningitis, and liver abscesses in immunocompromised individuals. A distinct invasive syndrome has been reported in Southeast Asia in the past two decades, whereby K. pneumoniae results in liver abscesses and extrahepatic complications in immunocompetent individuals. We present a case of disseminated K. pneumoniae infection, with metastatic infections of midbrain abscess, liver abscess, and osteomyelitis. A 48-year-old female with a history of chronic alcohol intake, hypertension, and gout presented to our…

INTRODUCTION: Helcococcus kunzii is a commensal bacterium found in the human skin that seldom causes infection. While there is documentation of abscesses; such as foot, breast, and brain abscesses; caused by H. kunzii, there are no reports of its causing umbilical abscesses.
CASE PRESENTATION: A case of scar tissue infection caused by H. kunzii in a diabetic patient is described. A single pathogen was isolated from a culture of the abscess extract and was identified as H. kunzii using matrix-assisted laser desorption/ionization-time of flight (MALDI-TOF) and 16S rRNA gene analysis. The patien…

Neonatal Escherichia coli meningitis is a serious disease with high mortality and poor outcome. Ventriculitis, brain abscess and subdural empyema are frequent, with no homogeneous recommendations available for these complications. The case of a newborn infant who developed sepsis and meningitis caused by E. coli is presented. During intravenous treatment with ampicillin, cefotaxime and gentamycin in recommended doses, the patient developed severe subdural abscesses detected on MRI. After consequent antibiotic therapy over 2 months with fosfomycin, amikacin and meropenem the patient improved cl…

A unique case of a large intradiploic abscess involving posterior fossa osseous structures is reported. A 16-year-old boy presented with a hard mass in the calvarium of posterior fossa region, fever and confusion. Radiological examination revealed an intradiploic collection with compression to cerebellum, fourth ventricle and brain stem, resulting in syringomyelia in cervical and thoracic spinal cord. After drainage and resection of the abscess wall, closure of a round dural defect was performed. The medical history of the patient and the intraoperative observations support the contention that

TWO HUNDRED CASES OF INTRACRANIAL SUPRATENTORIAL ABSCESS HAVE BEEN REVIEWED: 100 spanned the years 1951 to 1957, and 100 the years 1962 to 1967. The mortality rate of 40% was the same in both series, chiefly owing to continuing inaccurate localization of the abscess and inadequate use of antibiotics. The hazards of lumbar puncture are real, and this procedure is contra-indicated when convulsions, signs of a hemisphere disturbance, or papilloedema are present. Of the methods for localizing intracranial abscess the site of E.N.T. sepsis, ventriculography, and brain scanning were found to be the

Amoebic infections of the brain can cause brain abscesses and meningoencephalitis. Brain abscess is a rare form of extraintestinal amoebiasis caused by hematogenous spread from intestinal infection with Entamoeba histolytica, which occurs through oral intake of contaminated water and food or fecal-oral transmission. It is mostly associated with amoebic liver abscesses and can be effectively treated with metronidazole. Meningoencephalitis caused by pathogenic free-living amoebae is extremely rare. Primary amoebic meningoencephalitis (PAM), caused by Naegleria fowleri infection of the olfactory …

Streptococcus intermedius is an opportunistic pathogen capable of causing rapidly progressive, life-threatening cerebral abscesses. Diagnosis can be difficult because clinical features are nonspecific, cerebrospinal fluid findings may be negative, and biopsy results take time. Neuroimaging therefore plays a crucial role. While most reported S. intermedius abscesses present as a single lesion, we describe an immunocompetent young man with unusual multifocal brain involvement initially mistaken for neurocysticercosis. A 33-year-old male presented with fever, headache, and rapidly progressive con

BACKGROUND: No universal protocol exists for treating cerebral abscesses in Down syndrome. An illustrative case supplemented with a systematic literature review on brain abscesses in Down syndrome is presented, comprising a total of 16 cases. Preoperative infectious disease workups, cardiac examinations including echocardiography, as well as reported surgical and antibiotic treatments were correlated in the reported cohorts.
OBSERVATIONS: Overall, 18.8% of cases (n = 3) had no reported cardiac evaluation. The majority of cases were treated surgically (n = 8), with aspiration (n = 3), drainage…

BACKGROUND: The treatment of infectious oral lesions is an important prophylactic procedure preceding immunsuppressive therapy. The case reported underlines the importance of this treatment by showing severe complications of an infective dental focus in an immunocompetent patient.
CASE REPORT: A 65 year old patient was referred to hospital due to neurological disorders including hypaesthesia, movement deficits, nausea and falls. Because of progressive aggravation with hemiplegia a craniotomy and biopsy of a suspicious area in the pons was performed. A brain stem abscess caused by Streptococcu

Prenatal, neonatal meningoencephalitis and infections of the brain in older infants are often associated with serious complications which can be diagnosed by sonography through the open fontanelles. Most frequently postmeningitic hydrocephalus and subdural effusions occur. Rarer complications are brain abscesses and ventriculitis which are caused by gram negative bacteria such as E. coli, Serratia marcescens, Proteus and Enterobacter. A serious complication after ventriculitis is the development of compartment hydrocephalus. Multifocal small echogenic lesions scattered all over the brain are s…

INTRODUCTION: Abscesses associated with tumors are a rare entity. Imaging to differentiate abscess from other entities is often non-diagnostic, and often the source of infection is unknown. We present an unusual case of peritumoral abscess infected with both gram-negative and gram-positive bacteria.
METHODS: A 70-year-old, previously healthy male presented with a 1-day history of right-sided facial weakness sparing the forehead, as well as concomitant right upper and lower extremity numbness. A homogenously enhancing mass with adjacent rim-enhancing lesion with diffusion restricting cavity se

Tuberculosis remains prevalent in developing countries. Central nervous system tuberculosis often occurs secondary to pulmonary tuberculosis, transmitted through the bloodstream, and has a high mortality rate. Meningitis is the most common presentation of central nervous system tuberculosis, followed by tuberculoma, tuberculous brain abscess, and miliary tuberculosis. In this report, we present a case of miliary tuberculosis in a 3 month-old boy. The patient had a fever and was breathless for 1 month. The patient appeared cyanotic, experienced a seizure, and became comatose. Chest computed tom…

BACKGROUND: The development of a slowly enlarging epidermoid cyst (EC) near a scalp incision following titanium mesh cranioplasty (TMC), with subsequent progression to an epidural abscess after resection, represents an exceedingly rare iatrogenic complication.
CASE DESCRIPTION: A 68-year-old male, who had undergone decompressive craniectomy followed by staged bilateral TMC 5 years earlier for traumatic brain injury, presented with a solitary scalp mass that had been noted 1 year prior. Six months after the mass was resected, the patient developed an epidural abscess. Pathological examination

In recent years, an increasing number of reports have described invasive infections caused by bacteria from Streptococcus anginosus group (SAGs). S. intermedius seems to be more related with pleuropulmonary infections and abscess of the brain and deep soft tissues, and it is more likely to cause suppurative and non-bacteremic infections compared to other members of the same genus. We present two clinical cases of invasive S. intermedius infections in pediatric patients: a liver abscess case and a pansinusitis case associated with bilateral otomastoiditis and parapharyngeal abscess complicated

Rhinogenous complications may manifest in the region of the orbits, bone or soft parts of the wall of the frontal sinus, or endocranially. With regard to orbital complications, a differentiation is made between edema, periostitis, subperiosteal abscess and phlegmon--depending on severity and extent. A possible sequela of frontal sinusitis may be osteomyelitis. If the frontal bone is involved, there is a danger that the infection may spread to the endocranium via medullary spaces and blood vessels. The diagnostic basis for deciding appropriate treatment is CT or MRI. While orbital edema and per
