Repair of partial atrioventricular septal defect: a 37-year experience.

OBJECTIVES: Partial atrioventricular septal defect (pAVSD) is routinely repaired with a low mortality. However, limited data are available on the long-term follow-up of these patients. The current study was designed to determine long-term survival and morbidity of a large cohort of patients operated on at a single institution. METHODS: From 1975 to 2012, 249 consecutive patients underwent pAVSD repair at the Royal Children's Hospital. The follow-up data were obtained from hospital records, correspondence with cardiologists and primary care physicians, patient surveys and the state death regis…

Heparin-induced thrombocytopenia in the cardiac patient: 10 points to help the physician.

CASE 1: A 65-year-old man underwent triple coronary artery bypass surgery. Postoperative atrial fibrillation lengthened his ICU stay. On postoperative day 6, it was noted that the platelet count, which had begun to rise from a postoperative nadir of 105,000, had fallen again to 90,000. The cardiologist indicated suspicion of heparin-induced thrombocytopenia (HIT); he stopped all heparin and ordered a heparin antibody test. Platelets were 98,000 the next day when the cardiologist wrote, "I am considering calling Hematology, but they would likely anticoagulate the patient; his stool guaiac is po

Hand-carried ultrasound performed by hospitalists: does it improve the cardiac physical examination?

OBJECTIVE: The traditional physical examination of the heart is relatively inaccurate. There is little information regarding whether cardiac hand-carried ultrasound performed by noncardiologists adds to the accuracy of physical examinations. The purpose of this study was to determine whether hand-carried ultrasound can add to the accuracy of hospitalists' cardiac physical examinations. METHODS: During a focused training program in hand-carried echocardiography, 10 hospitalists performed cardiac examinations of 354 general medical inpatients first by physical examination and then by hand-carri…

Doppler echocardiography and the early diagnosis of carditis in acute rheumatic fever.

BACKGROUND: The incidence of acute rheumatic fever in New Zealand remains relatively high. Reliable early diagnosis of carditis is difficult and important in management. AIM: To determine if Doppler echocardiography contributed to the early diagnosis of carditis in acute rheumatic fever. METHODS: Forty-seven patients admitted to hospital with suspected acute rheumatic fever and 19 control patients, with a febrile illness due to a documented non-cardiac bacterial infection, were assessed two days and two weeks following admission. Presence or absence of clinical carditis was determined by a c

Ethnicity and socioeconomic status: impact on the diagnosis of congenital heart disease.

OBJECTIVES: The purpose of this study was to estimate the prevalence rates of specific cardiac defects for three ethnic groups and to determine the effects of ethnicity, family income and household education level on the timing of referral for pediatric cardiac care. BACKGROUND: Previous studies examining ethnic differences in rates of congenital heart disease were based on hospital referrals or were limited to diagnoses made in the 1st year of life. These limitations may lead to potential biases in the ascertainment of cases. The present study is population based and includes patients diagno

Use of Photovoice to Explore Pediatric Patients with Hypertrophic Cardiomyopathy and their Parents' Perceptions of a Heart-Healthy Lifestyle.

Background Heart-healthy lifestyles promote lifelong cardiovascular health. However, patients with hypertrophic cardiomyopathy are often advised to avoid strenuous exercise because of the risk of sudden cardiac death. Given these restrictions, this study explored youth and parent perceptions of a heart-healthy lifestyle and the barriers and facilitators to this lifestyle. Methods and Results Youth and parents were purposefully recruited at 2 Canadian hospitals for this photovoice project. Participants were given cameras and took pictures of everyday heart-healthy or heart-unhealthy choices. Ph

Process Factors Affecting Reperfusion Time in Patients With ST-Segment Elevation Myocardial Infarction.

OBJECTIVE: To determine process factors impacting myocardial infarction reperfusion time. BACKGROUND: An ST-segment elevation myocardial infarction (STEMI) is a life-threatening condition that necessitates emergent medical treatment; longer reperfusion times are associated with negative patient outcomes. Therefore, time is critical in the treatment of a STEMI. METHODS: Admissions data from Las Vegas hospitals were analyzed via multivariate regression analysis to determine predictors of reperfusion times. The analysis was based on 618 patients presenting with a diagnosis of STEMI at participa

The inductor role of cardiac consultation in the pre-anesthetic evaluation of asymptomatic patients submitted to non-cardiac minor and intermediate-risk surgery: a cross-sectional study.

INTRODUCTION: Asymptomatic patients with moderate functional capacity do not require Coronary Artery Disease (CAD) workup in the preoperative period of non-cardiac surgeries, especially when scheduled for minor and intermediate-risk surgeries. The workup is inappropriate because it promotes over diagnosing and pointless treatments. Moreover, those patients usually undergo cardiology assessment, in addition to pre-anesthetic evaluation. OBJECTIVE: Investigate the role of cardiology consultation as mediator in inappropriate assessment of CAD for preoperative of non-cardiac surgeries. METHOD: R

Acupuncture-Induced Tension Pneumothorax Presenting as Acute Heart Failure.

Takotsubo syndrome (TSS) is a reversible, acute cardiomyopathy with transient heart failure, often secondary to other disorders. A 64-year-old woman, with no history of ischemic heart disease, was admitted to the emergency department after developing sudden-onset dyspnea after a planned acupuncture treatment for back pain. Acute echocardiography showed decreased left ventricular function with basal hypercontraction and apical akinesia and was interpreted, and treated, as acute heart failure. When the attending cardiologist arrived, the patient still had dyspnea with a declining blood pressure
