Strength-Based Exercise Is Not Associated With Ventricular Arrhythmia in Arrhythmogenic Cardiomyopathy-Towards Safer Exercise Options.

BACKGROUND: Arrhythmogenic cardiomyopathy (ACM) is a genetic heart disorder characterized by fibrofatty replacement of ventricular myocardium, sustained ventricular arrhythmia (VA), and elevated risk of sudden death. Current guidelines restrict high-intensity aerobic exercise and competitive sports in individuals with ACM or at risk due to pathogenic/likely-pathogenic (P/LP) gene variants; however, the impact of strength-based exercise is poorly understood. OBJECTIVES: This study sought to evaluate the risk of VA in individuals with ACM and/or P/LP ACM gene variants exposed to strength-based

Glucagon-Like Peptide-1 Receptor Agonists Across the Heart Failure Spectrum: A Systematic Review and Meta-Analysis.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have shown cardiovascular (CV) benefits, but their effects across the heart failure (HF) spectrum remain uncertain. We synthesized randomized evidence comparing GLP-1 RAs with placebo in adults with HF across ejection fraction phenotypes by searching PubMed, Cochrane CENTRAL, and ClinicalTrials.gov through February 2026. The primary outcome was the composite of CV death and first HF hospitalization, and random-effects meta-analysis used restricted maximum likelihood (REML) estimation with the Hartung-Knapp-Sidik-Jonkman (HKSJ) adjustment. Therapeutic potential of ELABELA in alleviating hereditary hypertrophic cardiomyopathy.

INTRODUCTION: Hereditary hypertrophic cardiomyopathy (HCM) is a common genetic heart disease associated with heart failure and sudden cardiac death. Current therapies are limited, and the myosin inhibitor mavacamten is constrained by safety concerns and restricted applicability. ELABELA (ELA), an endogenous ligand of the apelin receptor (APJ), plays an important role in cardiovascular regulation, but its involvement in HCM remains unclear. OBJECTIVES: This study aimed to determine the role of ELA in hereditary HCM and to assess the therapeutic potential of restoring ELA signaling. METHODS: E

Risk factors for sudden cardiac death or sudden unexplained death in patients treated with clozapine: systematic review.

BACKGROUND: Clozapine is the gold-standard treatment for treatment-resistant schizophrenia but carries serious cardiac risks, including sudden cardiac death (SCD). Specific risk factors for SCD in clozapine-treated patients remain poorly defined. AIMS: To systematically identify and synthesise evidence on risk factors for SCD and sudden unexplained death (SUD) in clozapine-treated patients, to guide clinical monitoring. METHOD: We conducted a systematic review following Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines (PROSPERO, no. CRD420250646384). Five databas

Current concepts and challenges in pediatric diastolic dysfunction and heart failure with preserved ejection fraction.

Heart failure with preserved ejection fraction (HFpEF) has transitioned from an underrecognized and poorly characterized condition to a recognized, increasingly prevalent form of heart failure in the adult population, accounting for approximately half of all heart failure cases. Improved diagnostic capabilities and the growing burden of risk factors, including aging, hypertension, diabetes, and obesity, drive this rise. In contrast, pediatric HFpEF remains largely under-characterized, arising in a heterogeneous context that includes congenital heart disease and restrictive cardiomyopathies. Th

Vascular changes in hypertrophic cardiomyopathy.

AIMS: Hypertrophic cardiomyopathy (HCM) is characterized by left ventricular hypertrophy along with (micro)vascular abnormalities. These include cardiac and peripheral vascular dysfunction, as well as structural vascular changes. This review summarizes the modalities for assessing (micro)vascular function and explores how vascular changes relate to disease progression, including structural remodelling, fibrosis, and clinical outcomes. METHODS AND RESULTS: A systematic search of multiple databases was conducted to identify studies investigating vascular changes in patients with HCM, varying in

A Multicenter Study of Contemporary Long-Term Tafamidis Outcomes in Transthyretin Amyloid Cardiomyopathy.

BACKGROUND: Tafamidis improved survival and decreased cardiovascular hospitalizations in the ATTR-ACT trial. Due to improved recognition and earlier diagnosis, the epidemiology of transthyretin amyloid cardiomyopathy (ATTR-CM) is rapidly evolving. OBJECTIVES: The authors sought to evaluate the contemporary long-term outcomes of patients with ATTR-CM treated with tafamidis. METHODS: Patients with ATTR-CM who received at least 1 dose of tafamidis between 2018 and 2021 at 5 amyloidosis centers in the United States were enrolled. Primary outcome was all-cause mortality. RESULTS: Among 624 patie

Echocardiographic diastolic function thresholds indicative of increased mortality risk.

BACKGROUND: Currently recommended echocardiographic diastolic dysfunction thresholds are mostly derived from healthy adult populations rather than outcomes-based metrics. We identified thresholds of diastolic function parameters associated with increased long-term mortality risk. METHODS: Retrospective analysis of consecutive patients between 2011 and 2021. Repeat examinations, patients <18 years, critically ill, with left ventricular dysfunction or significant mitral annular calcification or stenosis, or with known or suspected restrictive or dilated cardiomyopathies were excluded. Clinical phenotypes and outcomes associated with improved left ventricular ejection fraction after biventricular pacing.

BACKGROUND: In some patients who receive a cardiac resynchronization therapy (CRT) device, the left ventricular ejection fraction (LVEF) does not improve. METHODS: We analyzed patients enrolled in the REVERSE, MADIT-CRT, and BLOCK-HF trials, restricting the analysis to those who received CRT. Characteristics of patients with or without improved LVEF were compared using two sample t-tests and Pearson's chi-square tests. Kaplan-Meier survival curves were constructed to display time-to-event data. A log-rank test was used to compare event rates for patients with or without improved LVEF. Mixed e

Granulopoietic Dysregulation in a Patient-Tailored Mouse Model of Barth Syndrome.

Barth syndrome (BTHS) is an X-linked recessive disorder characterized by cardiomyopathy, skeletal muscle myopathy and fatigue, growth restriction, and neutropenia. Neutropenia increases the risk of life-threatening bacterial infections, a major cause of death in individuals with BTHS. Currently, there is no curative treatment for BTHS or associated neutropenia. The development of therapeutic strategies to correct BTHS-associated neutropenia has been hindered by a limited understanding of the underlying molecular mechanisms involved. BTHS is caused by a mutation in the Tafazzin gene encoding a

Multi-Organ Transplantation in Adult Congenital Heart Disease: Navigating the Unique Challenges of a Distinct Patient Population.

The prevalence of adult congenital heart disease (ACHD) is increasing, with heart failure being the leading cause of death. For many ACHD patients, heart transplantation is the only treatment option for advanced heart failure, though significant extracardiac involvement may require multi-organ transplantation. Despite the rising number of ACHD transplants, multi-organ transplants in this population remain challenging, and a substantial gap remains between those in need and those who receive a transplant. While short-term outcomes may be worse for ACHD patients, long-term outcomes are comparabl

Position statement of Polish Cardiac Society experts on cardiomyopathy.

Cardiomyopathies (CMs) are a very broad group of diseases, including genetically determined and acquired, and their classification is based on phenotypic characteristics. There is always a need to search for the etiology (often also to try to identify the genetic cause), which may determine the appropriate choice of clinical management. The geographical distribution of genetic variants varies as does the prevalence across populations, ethnic groups, regions, and countries. The most reliable data on the distribution of individual genetic variants come from developed countries. The phenotypic cl

Utility of native T1 mapping and myocardial extracellular volume fraction in patients with nonischemic dilated cardiomyopathy: A systematic review and meta-analysis.

BACKGROUND: Cardiac magnetic resonance imaging (CMR) based T1 mapping and extracellular volume fraction (ECV) are powerful tools for identifying myocardial fibrosis. This systematic review and meta-analysis aims to characterize the utility of native T1 mapping and ECV in patients with non-ischemic cardiomyopathy (NICM) and to clarify the prognostic significance of elevated values. METHODS: A literature search was conducted for studies reporting on use of CMR-based native T1 mapping and ECV measurement in NICM patients and their association with major adverse cardiac events (MACE), ventricular

Uric acid elevation in pediatric patients with dilated cardiomyopathy and prediction of mortality.

BACKGROUND AND AIMS: Pediatric dilated cardiomyopathy (DCM) is a primary cause of heart failure, highlighting the urgent need for effective prognostic markers. METHODS: We performed a single-center retrospective study involving 145 children diagnosed with DCM, with a median follow-up period of 4.0 months (interquartile range: 6.2-108.4 months). The relationship between serum uric acid (SUA) levels and all-cause mortality was assessed using Kaplan-Meier survival curves, multivariate Cox proportional hazard models, and restricted cubic spline (RCS) models. RESULTS: Of the 145 children with DCM

Assessing Cardiovascular Health in Paediatric Hypertrophic Cardiomyopathy Using the CANHEART Health Index.

BACKGROUND: Paediatric hypertrophic cardiomyopathy (HCM) is associated with sudden cardiac death, often resulting in restriction of strenuous physical activity. High levels of inactivity and a rise in youth obesity may increase risk of poor cardiovascular (CV) health in these patients. We aim to compare the CV disease risk profile of paediatric patients with HCM to Canadian youth. METHODS: Patients with HCM (10-19 years) were recruited from 10 paediatric cardiac centres in Canada. The CV disease risk profile was determined using the Healthy Hearts School Program questionnaire and Cardiovascul

Cathelicidins Target HSP60 To Restrict CVB3 Transmission via Disrupting the Exosome and Reducing Cardiomyocyte Apoptosis.

Cathelicidin antimicrobial peptides (mouse, CRAMP; human, LL-37) have broad-spectrum antiviral activities against enveloped viruses, but their mechanisms of action against nonenveloped viruses remain to be elucidated. Coxsackievirus B3 (CVB3), a member of nonenveloped virus belonging to the Enterovirus genus of Picornaviridae, is an important pathogen of viral myocarditis and dilated cardiomyopathy. Here, we observed that cardiac CRAMP expression was significantly upregulated in mice after CVB3 infection. The administration of CRAMP or LL-37 markedly suppressed CVB3 infection in mice, and CRAM

Only one beer can be mortal: a case report of two sisters with cardiac arrest due to a homozygous mutation in PPA2 gene.

We report the long way to the correct diagnosis in two teenage sisters who developed a cardiac arrest after consuming minimal amounts of alcohol. The older girl dramatically survived two cardiac arrests at the age of 14 and 15 years. She underwent an extensive examination that revealed isolated cardiac abnormalities including fibrosis, dilated cardiomyopathy and inflammation. The younger girl also had a cardiac arrest at the age of 15 and died suddenly after consuming 1-2 beers, 3 years after her sister´s first incident. Autopsy of the heart revealed acute myocarditis without structural altera

Prenatal Diagnosis of Fetal Heart Failure.

Fetal heart failure (FHF) is a condition of inability of the fetal heart to deliver adequate blood flow for tissue perfusion in various organs, especially the brain, heart, liver and kidneys. FHF is associated with inadequate cardiac output, which is commonly encountered as the final outcome of several disorders and may lead to intrauterine fetal death or severe morbidity. Fetal echocardiography plays an important role in diagnosis of FHF as well as of the underlying causes. The main findings supporting the diagnosis of FHF include various signs of cardiac dysfunction, such as cardiomegaly, po
