Human immunodeficiency virus, genital ulcers and the male foreskin: synergism in HIV-1 transmission.

Epidemiologic studies in Nairobi and elsewhere in Africa, have shown that men infected with HIV-1 more commonly have a history of genital ulcer disease compared to uninfected men. In one study, HIV infected men were three times as likely to have a recent history of genital ulcers. In a prospective study of seronegative men, those presenting with chancroid had a five-fold risk of seroconversion during follow-up compared to men presenting with urethritis. Uncircumcised men had an increased risk of seroconversion which was independent of their risk of genital ulcer disease. Over 95% of attributab

Glanzmann thrombasthenia: a multi-center study of demographics, clinical spectrum, and treatment efficacy.

Glanzmann thrombasthenia (GT) is a rare inherited bleeding disorder caused by quantitative or qualitative defects of platelet integrin GPIIb/IIIa (CD41 and CD61). This study intended to describe the demographics, clinical features, and treatment response of GT patients in several hematology centers in Egypt. A cross-sectional multi-center study involved 87 children (<18 years old) diagnosed with GT. Clinical data were recorded as bleeding manifestations, response to treatment modalities, and laboratory results. The study included eighty-seven children (61 females and 26 males) with a median ag

What will it take for an injectable ARV to change the face of the HIV epidemic in high-prevalence countries? Considerations regarding drug costs and operations.

INTRODUCTION: The proven effectiveness of injectable cabotegravir (CAB-LA) is higher than that of any other HIV prevention intervention ever trialled or implemented, surpassing medical male circumcision, condoms and combination antiretroviral treatment. Based on our own analyses and experience with the South African oral pre-exposure prophylaxis (PrEP) programme, we review the supply and demand side factors that would need to be in place for a successful rollout of CAB-LA, and delineate lessons for the launch of other long-acting and extended delivery (LAED) antiretroviral drugs. DISCUSSION:

Impact of a bottom-up community engagement intervention on maternal and child health services utilization in Ghana: a cluster randomised trial.

BACKGROUND: Ghana is among African countries not likely to achieve the Sustainable Development Goal (SDG) three (3) target of reducing maternal mortality to 70 per 100,000 live births by the year 2030 if maternal and child health services utilization are not improved. Community engagement in health is therefore advocated to help address this challenge. This study evaluated the impact of a community engagement intervention on maternal and child health services utilization in Ghana. METHODS: This study was a cluster randomised trial among primary healthcare facilities (n = 64) in the Greater Ac

Risky sexual behaviors among Malay adolescents: a comparison with Chinese adolescents in Singapore.

OBJECTIVE: Malays, with majority of the individuals being Muslim, form the largest ethnic group in Southeast Asia. This region is experiencing a rising incidence of HIV infections. Due to circumcision and prohibition of sex outside marriage, being Muslim was argued to be a protective factor against sexually transmitted infections (STI) and Human Immunodeficiency Virus (HIV). However, Malay adolescents were found to be more likely to contract chlamydia and gonorrhea than non-Malay adolescents in Singapore. DESIGN: Using a cross-sectional survey, we examined and compared safer sex knowledge, at

The Impact of Behavioural Economics-Informed Interventions on the Care Cascade for Sexually Transmitted and Blood-Borne Infections: A Systematic Review of Randomized Controlled Trials.

INTRODUCTION: While behavioural economics principles are widely used for public health interventions, a comprehensive understanding of their impact across the sexually transmitted and blood-borne infections (STBBIs) care cascade remains limited. We systematically evaluated the effectiveness of behavioural economics-informed interventions for an STBBI care cascade. METHODS: We systematically searched five major literature databases from their inception to 25 April 2024, with the search updated on 22 March 2025. We included randomized controlled trials (RCTs) evaluating behavioural economics-in

Untapped potential of post-exposure prophylaxis in sub-Saharan Africa: a comparative analysis of PEP implementation planning in Kenya, Mozambique, Nigeria, Uganda and Zambia.

INTRODUCTION: In 2023, over 210,000 new HIV acquisitions occurred in Kenya, Mozambique, Nigeria, Uganda and Zambia. While uptake of oral pre-exposure prophylaxis (oral PrEP) and coverage of voluntary medical male circumcision increased significantly over the past decade, post-exposure prophylaxis (PEP) has received less attention and remains an underused HIV prevention intervention. In 2024, the World Health Organization (WHO) released new guidance emphasizing the need for timely access to PEP, including through community-based channels and task-sharing to mitigate barriers such as stigma and

Harnessing digital health data for person-centred HIV prevention monitoring: a survey of national health information systems.

INTRODUCTION: Measuring HIV prevention impact is challenging because prevention is started and stopped as needed, and individual-level data availability has been suboptimal or not collected. WHO's 2022 Consolidated guidelines on person-centred HIV strategic information aim to bridge this gap by recommending a minimum dataset for HIV prevention monitoring. METHODS: We surveyed the availability of 42 HIV prevention data elements collected on an individual from WHO's recommended minimum dataset in 21 countries' national health information systems during a Prevention Outcome Monitoring Workshop h

Understanding effective post-test linkage strategies for HIV prevention and care: a scoping review.

INTRODUCTION: Following HIV testing services (HTS), the World Health Organization recommends prompt linkage to prevention and treatment. Scale-up of effective linkage strategies is essential to achieving the global 95-95-95 goals for maintaining low HIV incidence by 2030 and reducing HIV-related morbidity and mortality. Whereas linkage to care including same-day antiretroviral therapy (ART) initiation for all people with HIV is now routinely implemented in testing programmes, linkage to HIV prevention interventions including behavioural or biomedical strategies, for HIV-negative individuals re

Evidence Gaps in Economic Evaluations of HIV Interventions Targeting Young People: A Systematic Review.

PURPOSE: Young people living with HIV (YPLWH) face the burden of navigating the unique physical, psychological, and social challenges of adolescence while coping with a stigmatized infectious disease that requires lifelong care. Consequently, YPLWH experience worse HIV outcomes compared to children and adults. This systematic review seeks to collate evidence on the health and economic impact of HIV interventions targeting YPLWH and to identify gaps in the available evidence that may inform future economic evaluations of interventions for YPLWH. METHODS: We searched the MEDLINE, Embase, Scopus

Assessing the efficacy of an integrated intervention to create demand for fishermen's schistosomiasis and HIV services (FISH) in Mangochi, Malawi: Study protocol for a cluster randomized control trial.

BACKGROUND: Both HIV and schistosomiasis are major public health problems worldwide with 1.8 million new HIV infections, and up to 110 million untreated schistosomiasis cases globally. Although a causal link has not been established, there are strong suggestions that having schistosomiasis increases onward transmission of HIV from co-infected men to women. With both HIV and schistosomiasis treatment readily available in Malawi, there is a need to investigate the feasibility, acceptability and health impacts of joint management of these two hazards, with special focus on health education and de

Modelling cervical cancer elimination using single-visit screening and treatment strategies in the context of high HIV prevalence: estimates for KwaZulu-Natal, South Africa.

INTRODUCTION: In settings with high HIV prevalence, cervical cancer incidence rates are up to six-fold higher than the global average of 13.1 cases per 100,000 women-years. To inform strategies for global cervical cancer elimination, we used a dynamic transmission model to evaluate scalable screening and treatment strategies, accounting for HIV-associated cancer risks and weighing prevention gains against overtreatment. METHODS: We developed a dynamic model of HIV-HPV co-infection and disease progression, which we calibrated to KwaZulu-Natal, South Africa. Our baseline scenario reflects the c

Implementation research approaches to promoting universal health coverage in Africa: a scoping review.

BACKGROUND: Implementation research has emerged as part of evidence-based decision-making efforts to plug current gaps in the translation of research evidence into health policy and practice. While there has been a growing number of initiatives promoting the uptake of implementation research in Africa, its role and effectiveness remain unclear, particularly in the context of universal health coverage (UHC). Hence, this scoping review aimed to identify and characterise the use of implementation research initiatives for assessing UHC-related interventions or programmes in Africa. METHODS: The r

Religion, faith, and spirituality influences on HIV prevention activities: A scoping review.

INTRODUCTION: Strategies to increase uptake of next-generation biomedical prevention technologies (e.g., long-acting injectable pre-exposure prophylaxis (PrEP)) can benefit from understanding associations between religion, faith, and spirituality (RFS) and current primary HIV prevention activities (e.g., condoms and oral PrEP) along with the mechanisms which underlie these associations. METHODS: We searched PubMed, Embase, Academic Search Premier, Web of Science, and Sociological Abstracts for empirical articles that investigated and quantified relationships between RFS and primary HIV preven
