Efficacy and safety of a modified killed-whole-cell oral cholera vaccine in India: an interim analysis of a cluster-randomised, double-blind, placebo-controlled trial.

BACKGROUND: Oral cholera vaccines consisting of killed whole cells have been available for many years, but they have not been used extensively in populations with endemic disease. An inexpensive, locally produced oral killed-whole-cell vaccine has been used in high-risk areas in Vietnam. To expand the use of this vaccine, it was modified to comply with WHO standards. We assessed the efficacy and safety of this modified vaccine in a population with endemic cholera. METHODS: In this double-blind trial, 107 774 non-pregnant residents of Kolkata, India, aged 1 year or older, were cluster-randomis

Psychological and somatic distress profiles in young adults in and out of conflict settings: a cluster-based analysis of three population-based studies in Ukraine and Switzerland.

BACKGROUND: Young adulthood is a developmental period characterized by major life transitions and increased exposure to stress, during which vulnerability to psychological and somatic symptoms may be heightened. These symptoms frequently co-occur, posing challenges for diagnostic and treatment approaches. Despite the mental and physical symptom burden among young adults, population-based data on symptom patterns in this age group remains sparse. This study investigates psychological and somatic symptom patterns among young adults in three populations with differing socio-geographic and war-rel

The clinical picture of ICANS after CAR-T cell therapy in critically ill patients: A retrospective clustering analysis of a multicentre multinational cohort study.

CAR T-cell therapy carries significant risks, including ICANS. There is limited data on ICANS presentation and outcomes in patients admitted to ICU and it is still challenging to predict those who will have severe ICANS. This is a pre-planned substudy of an international, multicenter, observational cohort study of 241 CAR T-cell recipients for hematological malignancies, admitted to the ICU, focusing on those who developed ICANS. We aimed for a detailed description of symptoms and hierarchical clustering analysis was used to identify distinct patterns of neurological symptom presentation. ICAN

Clustering analysis of acute mountain sickness susceptibility among young adults during rapid ascent using low-altitude step test data.

Acute mountain sickness (AMS) compromises health and work efficiency after rapid ascent. This study aimed to explore a low-altitude-data-based method for classifying AMS-susceptible young adults. Forty-four low-altitude residents (18-31 years) completed a step test at 50 m. Measurements included maximal oxygen uptake (V̇O2max), peripheral oxygen saturation at rest (re_SpO2), exercise (ex_SpO2), and recovery (rec_SpO2), heart rate at rest (re_HR), exercise (ex_HR), and recovery (rec_HR), and step index. Participants ascended to 3650 m within 6 h, and AMS was assessed using the 2018 Lake Louise

Low vitamin D status and cardiometabolic trait clustering in a primary-care adult cohort: association analysis and exploratory explainable machine-learning classification.

BACKGROUND: Low serum 25-hydroxyvitamin D (25(OH)D) commonly co-exists with obesity and adverse cardiometabolic traits. However, its cross-sectional association with dyslipidaemia, body composition and glycaemic markers in primary-care adults remains incompletely characterised. This study examined these associations as the primary objective and, as a secondary exploratory objective, assessed whether routine clinical variables could classify concurrent vitamin D deficiency using interpretable machine-learning models. METHODS: In this cross-sectional study, adults attending routine health check

Concomitant cannabidiol does not impact safety and effectiveness of diazepam nasal spray for seizure clusters: Post hoc analysis of a phase 3 safety study.

People with epilepsy may experience episodes of frequent seizure activity (seizure clusters, acute repetitive seizures), and benzodiazepines are the cornerstone of rescue treatment. Cannabidiol (CBD) can be used as an adjunctive treatment for epilepsy, and it may interact with other antiseizure drugs, such as benzodiazepines. Here, we examined the safety and effectiveness of intermittent use of diazepam nasal spray in patients with seizure clusters who also received CBD treatment. This analysis included data from patients aged 6 to 65 years enrolled in a phase 3, long-term safety study of diaz

Assessing the sustainability of the Systems Analysis and Improvement Approach to increase HIV testing in family planning clinics in Mombasa, Kenya: results of a cluster randomized trial.

BACKGROUND: In Kenya, HIV incidence is highest among reproductive-age women. A key HIV mitigation strategy is the integration of HIV testing and counseling (HTC) into family planning services, but successful integration remains problematic. We conducted a cluster-randomized trial using the Systems Analysis and Improvement Approach (SAIA) to identify and address bottlenecks in HTC integration in family planning clinics in Mombasa County, Kenya. This trial (1) assessed the efficacy of this approach and (2) examined if SAIA could be sustainably incorporated into the Department of Health Services
