Low-Volume Versus Standard-Volume Pericapsular Nerve Group Block with and Without Dexamethasone for Postoperative Analgesia After Total Hip Arthroplasty: A Randomized Controlled Trial.

Background: The pericapsular nerve group block provides motor-sparing analgesia after total hip arthroplasty, but the optimal injectate volume and the role of dexamethasone remain unclear. Methods: In this prospective, randomized, quadruple-blinded trial, 120 patients undergoing total hip arthroplasty under spinal anesthesia were allocated to receive ultrasound-guided pericapsular nerve group block with 20 mL or 10 mL of 0.2% ropivacaine, with or without 4 mg dexamethasone. The primary outcome was time to first rescue opioid within 24 h. Secondary outcomes included opioid consumption, pain sco

Effectiveness of nature-based group intervention in loneliness and health-related quality-of-life in lonely older adults living in assisted living facilities-a randomised controlled trial.

INTRODUCTION: Nature experienced with peers may mitigate the harmful outcomes of loneliness on health and wellbeing. The H2020 RECETAS 'Friends in Nature (FiN)-Helsinki' group intervention for lonely older adults in Helsinki assisted living facilities (ALFs) aimed to investigate the effects on participants' loneliness and health-related quality of life (HRQoL). We also examined factors influencing effects between the intervention and the outcomes. METHODS: Lonely participants were recruited from 22 ALFs in Helsinki area, Finland and randomised into two groups: (i) nature-based group intervent

Effectiveness of a mobile application-based program for enhancing independent menstrual management skills in adolescent girls with mild intellectual disabilities: A parallel-group randomized controlled trial.

BACKGROUND: Adolescent girls with mild intellectual disabilities often face challenges in achieving independence in menstrual management, highlighting the need for structured and accessible educational interventions. This study evaluated the effectiveness of a mobile application-based education program in improving independent menstrual management skills. METHODS: A parallel-group randomized controlled trial was conducted with 90 adolescent girls with mild intellectual disabilities and their mothers. Participants were randomly assigned (1:1) to the intervention or control group using a comput

[Controlled Donation after Circulatory determination of Death (cDCD) : A position paper by the DCD working group of the German Interdisciplinary Association for Intensive Care and Emergency Medicine (DIVI)].

In Germany at present, patients with a deleterious brain damage must be denied their wish for organ donation if the full clinical picture of "brain death" (DNC) does not occur. This significantly limits the individual patient's autonomy at the end of life. Intensive care may and must be withdrawn if the patient refuses its continuation. Following withdrawal of life-sustaining measures (WLST), the patient then dies - whilst receiving palliative care - as a result of cardiocirculatory arrest. In many countries, a patient's death following cardiocirculatory arrest may - under strictly regulated c

Effects of group-based music therapy combined with speech therapy on language and social skills in preschool children with autism spectrum disorder: An exploratory randomized controlled trial.

BACKGROUND: Autism spectrum disorder (ASD) is characterized by early difficulties in language and social communication. While speech therapy is a core early intervention, music therapy has been increasingly applied to support social-emotional development. However, evidence regarding the additive effects of group-based music therapy combined with speech therapy in preschool children with ASD remains limited. METHOD: This exploratory randomized controlled trial enrolled (RCT) preschool children aged 3-5 years with ASD and language delay. Participants were randomly assigned to a Music Therapy gr

Ultrasound-guided pericapsular nerve group block versus intravenous morphine for pain management in older adults with hip fractures: a randomised controlled trial in the emergency department.

BACKGROUND: Hip fracture pain is severe and is commonly managed with intravenous opioids that cause adverse effects in older adults. Although the use of regional anaesthesia is increasing, high-quality evidence remains limited, with insufficient randomised controlled trial (RCT) evaluating the effectiveness of the pericapsular nerve group (PENG) block in the emergency department (ED). This study aimed to compare the analgesic efficacy of ultrasound-guided PENG block with intravenous morphine for acute hip fracture pain in older adults presenting to the ED. METHODS: This open label, 1:1 parall

Effectiveness of a resilience, gender equity and mental health group intervention for young people living in informal urban communities in North India: a cluster randomized controlled trial.

BACKGROUND: Mental health problems are the leading cause of disease burden among young people in India. While evidence shows that youth mental health and resilience can be improved with group interventions in school settings, such an intervention has not been robustly evaluated in informal urban settings. OBJECTIVE: This study aimed to evaluate whether the Nae Disha 3 group intervention could improve youth resilience, mental health and gender equal attitudes among disadvantaged young people from low-income urban communities in India. METHODS: This cluster randomised controlled trial used an

The effect of adding lateral femoral cutaneous nerve block to pericapsular nerve group (PENG) block in hip surgery on postoperative morphine consumption: A randomized controlled trial.

BACKGROUND: The lateral approach is commonly used in hip fracture surgery. Pericapsular nerve group (PENG) block cannot block the lateral femoral cutaneous nerve (LFCN), which is involved in sensing the skin incision during the lateral approach. Therefore, we compared the effect of adding the LFCN block to the PENG block on opioid consumption and pain scores in hip fracture operations under spinal anesthesia. METHODS: In this prospective randomized-controlled study, patients undergoing hip fracture surgery under spinal anesthesia were randomized into 3 groups: PENG, PENG + LFCN, and CONTROL g

Clinical and cost-effectiveness of lithium versus quetiapine augmentation for treatment-resistant depression: a pragmatic, open-label, parallel-group, randomised controlled superiority trial in the UK.

BACKGROUND: Lithium and quetiapine are first-line augmentation options for treatment-resistant depression; however, few studies have compared them directly, and none for longer than 8 weeks. We aimed to assess whether quetiapine augmentation therapy is more clinically effective and cost-effective than lithium for patients with treatment-resistant depression over 12 months. METHODS: We did this pragmatic, open-label, parallel-group, randomised controlled superiority trial at six National Health Service trusts in England. Eligible participants were adults (aged ≥18 years) with a current episode

Clinical benefits of modifying the evening light environment in an acute psychiatric unit: A single-centre, two-arm, parallel-group, pragmatic effectiveness randomised controlled trial.

BACKGROUND: The impact of light exposure on mental health is increasingly recognised. Modifying inpatient evening light exposure may be a low-intensity intervention for mental disorders, but few randomised controlled trials (RCTs) exist. We report a large-scale pragmatic effectiveness RCT exploring whether individuals with acute psychiatric illnesses experience additional benefits from admission to an inpatient ward where changes in the evening light exposure are integrated into the therapeutic environment. METHODS AND FINDINGS: From 10/25/2018 to 03/29/2019, and 10/01/2019 to 11/15/2019, all

Restrictive use of Restraints and Delirium Duration in the Intensive Care Unit (R2D2-ICU): protocol for a French multicentre parallel-group open-label randomised controlled trial.

INTRODUCTION: Physical restraint (PR) is prescribed in patients receiving invasive mechanical ventilation in the intensive care unit (ICU) to avoid unplanned removal of medical devices. However, it is associated with an increased risk of delirium. We hypothesise that a restrictive use of PR, as compared with a systematic use, could reduce the duration of delirium in ICU patients receiving invasive mechanical ventilation. METHODS AND ANALYSIS: The Restrictive use of Restraints and Delirium Duration in ICU (R2D2-ICU) study is a national multicentric, parallel-group, randomised (1:1) open-label,
