Reoperations after decompression with or without fusion for L4-5 spinal stenosis with or without degenerative spondylolisthesis: a study of 6,532 patients in Swespine, the national Swedish spine register.

Background and purpose - There are different opinions on how to surgically address lumbar spinal stenosis with concomitant degenerative spondylolisthesis (DS). We investigated reoperation rates at the index and adjacent levels after L4-5 fusion surgery in a large cohort of unselected patients registered in Swespine, the national Swedish spine register.Patients and methods - 6,532 patients, who underwent surgery for L4-5 spinal stenosis with or without DS between 2007 and 2012, were followed up to 2017 to identify reoperations at the index and adjacent levels. The reoperation rates for decompre

Compression Neuropathies: Revisions and Managing Expectations.

Most compression neuropathies can be reliably treated with surgical decompression; however, in approximately 25% of the cases, this release fails, requiring revision surgery. Defining the correct diagnosis after a failed nerve decompression (ie, persistent, recurrent, or new symptoms) is of the utmost importance and guides toward the optimal treatment. This article describes the clinical categorization of secondary carpal tunnel syndrome and cubital tunnel syndrome, intraoperative principles of revision surgery and treatment options that are currently available.

Symptomatic Rathke cleft cysts: extent of resection and surgical complications.

OBJECT: Rathke cleft cysts (RCCs) are benign masses arising from the embryological Rathke pouch, and are commonly treated by transsphenoidal surgery. The authors retrospectively compared RCC extent of resection-either gross-total resection (GTR) or decompression-to the primary outcome measure, which was recurrences resulting in repeat surgery, and the secondary outcome measure, which was complications. METHODS: Seventy-four patients presenting to the neurosurgical department with RCC were analyzed retrospectively. Sixty-eight patients had a total of 78 surgical procedures, with the diagnosis

Effect of spinal decompression on back pain in lumbar spinal stenosis: a Canadian Spine Outcomes Research Network (CSORN) study.

BACKGROUND CONTEXT: Surgical decompression is usually offered for improvement of neurogenic claudication in patients with symptomatic lumbar canal stenosis. These patients often have associated low back pain (LBP) and little is known about the effect of decompression on this symptom. PURPOSE: The goal of the present study is to specifically quantify the improvement in LBP following surgical decompression for lumbar canal stenosis and to identify factors associated with changes in LBP in this population. STUDY DESIGN: This is a multicenter, retrospective review of consecutive spine surgery pa

Safety and efficacy of microsurgical decompression for 6 arachnoid bands.

BACKGROUND: Dorsal spinal arachnoid bands are a rare cause of focal spinal cord compression and thoracic myelopathy that alter cerebrospinal fluid dynamics and may lead to progressive neurological deficits and syringomyelia. Nevertheless, the clinical outcomes of surgical treatment remain incompletely characterized. Here, we evaluated the safety and clinical outcomes of 6 microsurgical decompressions for dorsal thoracic arachnoid bands in a single-center cohort. METHODS: Six patients underwent surgery for dorsal thoracicspinal arachnoid bands with/without syringomyelia (2021-2025) at a single

Surgical Timing After Spinal Cord Injury: A Narrative Review of Current Evidence and Perspectives.

Traumatic spinal cord injury (SCI) causes severe neurological and functional disability with major socioeconomic impact. The primary trauma initiates ischemia, excitotoxicity, and inflammation, leading to progressive neuronal loss and glial scarring. Early surgical decompression is a key strategy to limit secondary injury and promote recovery, though optimal timing remains debated. Preclinical data consistently favor early intervention, while clinical studies define "early" variably (<8-72 hours). Evidence from multicenter trials and meta-analyses indicates that decompression within 24 hours o

Optimal interval from decompression to semi-elective operation in sigmoid volvulus.

BACKGROUND/AIMS: The initial non-operative decompression and subsequent semi-elective operation is a common strategy in managing sigmoid volvulus. However, the optimal interval between decompression and operation is still unclear. METHODOLOGY: From 1994 to 2004, 25 patients operated on for sigmoid volvulus at the National Taiwan University Hospital were enrolled. The treatment strategy and clinical outcome in these patients were retrospectively reviewed and analyzed. RESULTS: Twenty-one patients underwent successful non-operative decompression initially, and the remaining 4 patients underwen

Possible role of an endovascular provocative test in the diagnosis of glossopharyngeal neuralgia as a vascular compression syndrome.

We utilized endovascular provocative techniques to identify the indications for microvascular decompression surgery in a serious case of glossopharyngeal neuralgia. This is the first reported case in which an endovascular provocative test was applied for diagnosis of glossopharyngeal neuralgia as a vascular compression syndrome. A 68-year-old woman presented with severe paroxysmal facial pain which could not be controlled by medical therapy. Partial effectiveness to carbamazepine led us to wonder whether or not the selection of microvascular decompression surgery would be appropriate. Pre-oper

Acute thyroid eye disease (TED): principles of medical and surgical management.

The active inflammatory phase of thyroid eye disease (TED) is mediated by the innate immune system, and management is aimed at aborting this self-limited period of autoimmune activity. In most patients with TED, ocular and adnexal changes are mild and management involves controlling thyroid dysfunction, cessation of smoking, and addressing ocular surface inflammation and exposure. In patients with acute moderate disease, this being sufficient to impair orbital functions, immunosuppression reduces the long-term sequelae of acute inflammation, and adjunctive fractionated low-dose orbital radioth
