Glucocorticoid receptor antagonism in major depressive disorder with childhood trauma: a randomized controlled trial.

Childhood trauma (CT) is a key risk factor for major depressive disorder (MDD) onset and persistence. Hypothalamic-pituitary-adrenal (HPA) axis dysregulation may underlie this link, and preclinical studies suggest glucocorticoid receptor (GR) antagonism can reverse early life stress effects. This study tested whether the GR antagonist mifepristone reduces depressive symptoms in adults with MDD and CT. The RESET-medication study was a randomized, double-blind, placebo-controlled trial evaluating a 7-day course of mifepristone (1200 mg/day) or placebo in 158 adults with MDD and CT, assessed at b…

Elevated endothelial cell signatures in major depressive disorder: A two-cohort transcriptomic concordance study.

Major Depressive Disorder (MDD) lacks objective molecular biomarkers and is diagnosed clinically. Bulk blood transcriptomics shows immune-inflammatory dysregulation in MDD but obscures the cell populations driving disease, and recent work highlighting an elevated endothelial signature has lacked independent validation. We conducted a two-stage study using public datasets, performing cell-type deconvolution with marker panels for seven cell types in a discovery cohort (GSE54564, n = 42) to nominate candidates, then tested these for concordance in a larger independent cohort (GSE98793, n = 192) …

Unveiling diverse clinical symptom patterns and neural activity profiles in major depressive disorder subtypes.

BACKGROUND: The heterogeneity of major depressive disorder (MDD) significantly hinders its effective and optimal clinical outcomes. This study aimed to identify MDD subtypes by adopting a data-driven approach and assessing validity based on symptomatology and neuroimaging. METHODS: A total of 259 patients with MDD and 92 healthy controls were enrolled in this cross-sectional study. Latent profile analysis (LPA) was used to identify MDD subtypes based on validated clinical symptoms. To examine whether there were differences between these identified MDD subtypes, network analysis was used to te…

Molecular pathways linking the serotonin transporters (SERT) to depressive disorder: from mechanisms to treatments.

The serotonin transporter (SERT) plays a key role in the regulation of serotonin levels in synapses and is significantly involved in the pathophysiology of major depressive disorder (MDD). In this review, molecular pathways connecting SERT dysfunction associated with depression are defined, including genetic polymorphisms (e.g., 5-HTTLPR), epigenetics (e.g., SLC6A4 methylation), and environmental interactions through stress and inflammation. Using a serotonergic pharmacophore, oxidative/nitrosative stress, cytokines, and neuroendocrine factors act via the hypothalamic-pituitary-adrenal (HPA) a…

Do immune system and microbiome-gut-brain axis interactions associate with major depressive disorder?

Major depressive disorder (MDD) is a leading psychiatric disorder with increasing global prevalence, yet its underlying pathogenesis remains inadequately elucidated. Increasing evidence highlights the complex interplay between the immune system, gut microbiota, and their bidirectional crosstalk with the central nervous system. Gut microbiota dysbiosis affects neuroimmune and intestinal immune homeostasis, driving bidirectional peripheral-central immune responses through immune-to-brain and gut-to-brain communication. This process involves impaired intestinal barrier integrity (bacterial transl…

The association of plasma glutamate with food addiction in individuals with major depressive disorder.

BACKGROUND: Food addiction (FA) is more prevalent in individuals with major depressive disorder (MDD) and may contribute to the increased risk of obesity in MDD. Peripheral glutamate, an amino acid found in the blood and tissues, has been linked to mental health symptoms, substance use and obesity. It is implicated in problematic eating however its relationship to FA has been underexpored. This study investigated associations between glutamate and FA in individuals with and without MDD. METHODS: Sixty-one individuals with MDD and sixty controls were recruited and sub-categorised based on crit…

Transcranial temporal interference stimulation for treating bipolar disorder with depressive episodes: a feasibility Study.

Bipolar depression (BD-D) is a significant clinical challenge associated with high disease burden. Transcranial temporal interference stimulation (tTIS), a novel and noninvasive approach for targeting deep brain structures, was investigated for its efficacy and safety in BD-D patients in this trial. Thirty-six patients were recruited for a single-arm, open-label trial, and 25 completed the 5-day intervention consisting of 10 tTIS sessions targeting the left nucleus accumbens. Each session lasted 20 min, with a maximum current intensity of 2 mA and an envelope stimulation frequency of 40 Hz. Si

Electroconvulsive therapy induces cortical and subcortical structural changes in adolescents with major depressive disorder.

BACKGROUND: Although electroconvulsive therapy (ECT) is an effective intervention for adolescents with major depressive disorder (MDD), the underlying neuroanatomical mechanisms remain poorly understood. This structural magnetic resonance imaging (sMRI) study investigated cortical and subcortical morphology in adolescents with MDD and suicidal ideation before and after ECT. METHODS: The study included 27 adolescent MDD patients with suicidal ideation and 31 demographically similar healthy controls (HC). Cortical thickness, surface area, and subcortical volumes were quantified from MRI scans u…

Subcortical functional connectivity alterations in first-episode medication-naïve adolescents with major depressive disorder.

Major depressive disorder (MDD) demonstrates significant alterations in intrinsic functional connectivity (FC) of the subcortical networks (SCN). However, the neurobiological mechanisms underlying these changes in adolescents with MDD and their association with stressors and sleep disturbances remain poorly understood. Using seed-based functional connectivity analysis, we investigated changes in intrinsic connectivity of the SCN in 83 first-episode medication-naïve adolescents with MDD (aMDD) and 59 healthy controls (HC). We then examined the correlation between the SCN connectivity alteration…

A network-based approach reveals higher plasticity levels in bipolar than major depressive disorder.

BACKGROUND: Bipolar disorder (BD) and major depressive disorder (MDD) are often misdiagnosed due to overlapping symptomatology, leading to inappropriate treatments and poor outcomes. Thus, novel approaches for effective early differential diagnosis are warranted. Plasticity - the ability to modify brain functioning and mental state - is increasingly recognized as a crucial process in psychiatry, as it underlies transitions from psychopathology to wellbeing. Accordingly, enhanced plasticity is reported to increase mental state transitions. Since BD patients experience transitioning between depr…

Novel screening tool for psychiatric samples with major depressive disorder: a two-centre study.

BACKGROUND: Major depressive disorder (MDD) poses a significant global public health challenge. Although the Patient Health Questionnaire-9 (PHQ-9) is widely used for MDD screening, its validation in psychiatric samples is limited. Additionally, variability in cut-off values across populations may lead to diagnostic inaccuracies. This study aims to develop a highly accurate nomogram tailored to improve MDD screening precision in psychiatric samples. METHODS: From March 2024 to February 2025, participants were consecutively recruited from psychiatric outpatient clinics at Xijing Hospital and t…

Initial and Sustained Attentional Bias Toward Emotional Faces in Patients with Major Depressive Disorder.

Major depressive disorder (MDD) represents a prevalent mental health condition characterized by prominent attentional biases, particularly toward negative stimuli. While extensive research has established the significance of negative attentional bias in depression, critical gaps remain in understanding the temporal dynamics and valence-specificity of these biases. This study employed eye-tracking technology to systematically examine the attentional processing of emotional faces (happy, fearful, sad) in MDD patients (n = 61) versus healthy controls (HC, n = 47), assessing both the initial orien…
