Pain levels after injection of corticosteroid to hand and elbow.

The goal of this study was to examine temporal pain response after injection of corticosteroid to the upper extremity (hand and elbow). Visual analog pain questionnaires were prospectively distributed to 133 consecutive patients, and pain levels were recorded for 5 days after injection. Ninety-four patients (71%) completed the questionnaire. Forty-seven (50%) of the 94 patients did not have increased pain after injection; the other 47 patients had increased pain after injection. The increased pain in 39 (83%) of the 47 patients resolved by day 5, lasting a mean of 1.2 days, and these 39 patien…

Fractures of the elbow's lateral column radial head and capitellum.

Fractures of the lateral column of the elbow involve radial head fractures or capitellum fractures. As the biomechanics of the elbow have become better understood, the importance of the radiocapitellar joint in stabilizing the elbow has been emphasized. Preservation of the radial head has gained importance. For nondisplaced radial head fractures, good results are generally obtained with a short period of immobilization followed by early range of motion. In managing displaced radial head fractures, improved techniques and more versatile instrumentation have helped make preservation more feasibl…

Indications, alternatives, and complications of external fixation about the elbow.

External fixation around the elbow is a concept as complex as the pathology that it treats. A myriad of potential pitfalls await the surgeon who casually applies a frame to any extremity, let alone a portion of one as complicated as the elbow. A careful understanding of the anatomy, specific technique, applications of the various devices commercially available, and rational alternatives to each problem is crucial for the judicial use of external fixation around the elbow. The potential benefits of the external fixator, however, are worthy of the surgeon's time to understand and pass on to his …

Nonhealing ulcerative mass of the elbow: do not forget tuberculosis.

Skeletal tuberculosis is a rare condition and is seen in only 1 to 2% of all cases of tuberculosis. The authors present a 69-year-old white woman with tuberculosis of the left elbow joint whose disease was suspicious for synovial sarcoma. Skeletal tuberculosis usually conflicts with neoplastic or inflammatory diseases. One should remember the possibility of bone-joint tuberculosis and send biopsy specimens for culture to determine the presence of acid-fast bacilli. However, neither culture nor polymerase chain reaction nor histological examination may be capable of showing the bacilli. Only a …

Evaluation of the strength of elbow flexors in patients with neuromuscular diseases.

In planning the optimum treatment for patients with neuromuscular diseases (NMD), it is essential to know as much as possible about their fu … ighest torque. For this purpose an eletronic brace enabling isometric measurements of torque during elbow flexion was designed. The patients produced three maximum voluntary elbow flexions that lasted about 3 s and separated by a pause of about 3 s. Force development was rapid with continuous build-up and isometric. About 15 s later the patients produce …

Anterior intramuscular transposition with ulnar nerve decompression at the elbow.

The effectiveness of decompression and anterior intramuscular transposition of the ulnar nerve for treatment of severe cubital tunnel syndrome was evaluated. A consecutive series of 39 anterior intramuscular transpositions were reviewed. One surgeon performed the transpositions between 1993 and 1997 in 34 patients who presented with clinically severe cubital tunnel syndrome. Clinical outcome and satisfaction with surgery were assessed. The results showed early clinical improvement of 77% of patients (mean followup, 3.34 months). With repeated assessments later, the same group of patients had c

Posterolateral dislocation of the elbow joint. Relationship to medial instability.

BACKGROUND: Dislocation of the elbow joint is the second most common dislocation in the upper extremity, dislocation of the shoulder being the most common. It has been reported that uncomplicated dislocation of the elbow joint may be associated with a decreased range of motion, degenerative changes in the elbow joint, ectopic calcification, or neurological deficits. As the medial collateral ligament complex can be completely disrupted during dislocation, we evaluated the association between the long-term results of treatment of simple posterolateral dislocation of the elbow and the presence of…

Percutaneous release of the common extensor origin for tennis elbow.

Thirty patients (32 elbows) who had surgery for tennis elbow were examined. Symptoms were present an average of 18 months before surgery. The operation was done percutaneously through an incision just distal to the lateral epicondyle, through which the common extensor origin was released. The followup period was 6 to 61 months, with an average of 26 months. Twenty-nine elbows had good or excellent results. In these 29, the pain was relieved an average of 9 weeks after surgery. Three elbows did not improve significantly, and treatment was considered a failure. The grip strength improved from an…

Transarticular fixation with the capacity for motion in fracture dislocations of the elbow.

Post-traumatic stiffness of the elbow joint is a frequent result of immobilisation leading to severe disability in the use of the upper extremity. Recognition of the tendency to stiffness leads to the assumption that the strong self-healing forces of the capsule and ligament apparatus converts the initial instability of the joint after ligament disrupture, into a high-grade undirected stability following immobilisation. Directed stability as it is produced by the natural ligament apparatus of the joint on the other hand produces a guided movement of the joint in one direction. These theoretica…

Conservative and surgical treatment of tennis elbow: a study of outcome.

BACKGROUND: Tennis elbow is one of the most common lesions of the arm. The first description is attributed to Runge in 1873 but the name derives from 'Lawn Tennis Arm' described by Morris in 1882. The majority of cases are believed to be caused by a musculo-tendinous lesion of the common extensor origin at or near the attachment to the lateral epicondyle. While there are some comprehensive reports in the literature of patients treated by one technique or another, there are no comprehensive reports of groups of patients for whom various techniques were employed in a logical sequence until a res…

Compressive ulnar neuropathies at the elbow: I. Etiology and diagnosis.

Ulnar nerve compression at the elbow can occur at any of five sites that begin proximally at the arcade of Struthers and end distally where the nerve exits the flexor carpi ulnaris muscle in the forearm. Compression occurs most commonly at two sites-the epicondylar groove and the point where the nerve passes between the two heads of the flexor carpi ulnaris muscle (i.e., the true cubital tunnel). The differential diagnosis of ulnar neuropathies at the elbow includes lesions that cause additional proximal or distal nerve compression and systemic metabolic disorders. A complete history and a tho…

Osteochondritis dissecans of the elbow managed by Herbert screw fixation.

Seven patients (eight elbows) with osteochondritis dissecans were treated with cancellous bone grafts and internal fixation of the fragment using a Herbert screw. Average follow-up was 32 months. All patients were pain-free and returned to previous sporting activities. The postoperative range of motion of the elbow increased by an average of 18 degrees. Radiographs showed complete reossification of the capitular cyst and normally contoured joint surface.

Os supratrochlear dorsale of the elbow: a report of two cases.

The os supratrochlear dorsale, an accessory ossicle of the elbow, has rarely been described in the literature. Rather than being caused by trauma, it is subject to trauma by elbow extension with secondary chondrometaplasia, resulting symptomatology. The aim of this report is to remind readers that its presence is crucial for both diagnostic and therapeutic purposes. Arthroscopic removal may be required to relieve persistent pain, catching and limited range of motion.

[Sports-induced acute epicondylitis of the elbow and conservative therapy].

48 patients with an acute lateral and/or medial epicondylitis of the elbow, induced by sporting activities, were treated in a prospective, controlled, double-blind, and randomised clinical study over a period of one to two weeks in a standard manner, either with taping and the oral NSAID proglumetacine b.i.d. (26 patients: 8 women, 18 men; mean age of 37.8 years) or with taping and an oral placebo b.i.d. (22 patients: 11 women, 11 men; mean age of 40.0 years). The course of complaints was followed on the basis of a functional index (created according to Lequesne). Taking into account the prior…

Fixation strength of the ulnar component of total elbow replacement.

After clinical failure of ulnar component fixation by olecranon fracture occurred, this study examined different component designs to find the strongest type. Components requiring square cuts or rounded cuts to prepare the proximal ulna with short or long stems of polyethylene or metal were implanted in paired cadaver bones and tested to failure. Loads were applied through the articular surface in an anterior-posterior direction, causing bending fracture of the olecranon. The main findings were a higher strength if the bone was prepared to take a rounded rather than a square-shaped prosthesis

A new case of hairy elbows syndrome (Hypertrichosis cubiti).

Hypertrichosis cubiti, also named hairy elbows syndrome (HES), is an uncommon variety of congenital, circumscribed hypertrichosis in which a remarkable amount of long vellus hair is localized on the extensor surfaces of the upper extremities. Usually, this condition appears in infancy, increases during early childhood, and regresses spontaneously at puberty. The mode of inheritance is not elucidated, and the condition presents in a sporadic or familial form. In most patients HES is not associated with any other physical abnormalities, although short stature and other developmental disorders ha…

Synovial Osteochondromatosis of the elbow presenting with ulnar nerve neuropathy.

Synovial chondromatosis is an uncommon disorder in which cartilaginous material is formed within synovial tissue. The cartilaginous nodules may undergo enchondral ossification, described as synovial osteochondromatosis. The nodules may be shed from the synovium and become intra-articular loose bodies. The presenting symptoms are usually diffuse discomfort in the affected joint and decreased range of motion with an accompanying gritty or locking sensation. The authors present the case of a young man with elbow synovial osteochondromatosis associated with ulnar nerve neuropathy.

Arthroscopic treatment for posterior impingement in degenerative arthritis of the elbow.

Twenty-one patients underwent arthroscopic treatment for posterior impingement associated with degenerative elbow arthritis. Anterior debridement and removal of loose bodies was performed first. The posterior procedure consisted of three parts: removal of posterior loose bodies, removal of the posterior olecranon osteophyte, and removal of the osteophytes in the olecranon fossa to the point of fenestration. Patients were assessed for pain, strength, motion, stability, and function. They were classified as excellent, good, fair, or poor. Twenty-one of 25 patients were followed an average of 35 …

Compression of the ulnar nerve at the elbow in association with synovial cysts.

Compression neuropathies around joints in association with synovial cyst formation are rare events. A case of a posttraumatic ulnar nerve palsy in a patient with osteoarthritis of the elbow is presented. At operative exploration of the cubital tunnel, the ulnar nerve was found to be compressed by a synovial cyst. Cyst formation should be considered in determining the etiology of compression neuropathies in patients with posttraumatic and degenerative processes occurring around the joints.
