A case of Stevens-Johnson Syndrome (erythema multiforme major) due to Mycoplasma pneumoniae infection in a 45-year-old man is presented. This association has been previously reported only in children and young adults. A brief review of erythema multiforme, including the Stevens-Johnson syndrome, is described. M. pneumoniae should be added to the list of causes of the Stevens-Johnson syndrome in adults with pneumonia.

We report the case of a 16-year-old Indian girl who presented with multiple small papular lesions and larger erythema multiforme-like plaques in a Christmas tree pattern over her body for 3 weeks. Histopathologic examination was performed, and she was diagnosed with pityriasis rosea presenting with two atypical morphological variants-generalized papular and erythema multiforme-like. A brief review of literature of these two rare types is also presented.

A 47-year-old woman presented with a long-standing history of an erythema multiforme-like eruption in association with lupus erythematosus. Unusual laboratory and immunologic findings were consistent with a diagnosis of Rowell's syndrome, which includes lupus erythematosus in association with erythema multiforme-like skin lesions, a speckled antinuclear antibody pattern, and a positive rheumatoid factor. We believe that our patient meets the criteria for this rarely reported entity.

Stevens-Johnson syndrome (SJS) is a severe and sometimes lethal form of erythema multiforme most often associated with a drug hypersensitivity reaction. HIV-infected patients suffer a higher incidence of SJS than the general population. This article details a case of nevirapine-induced SJS in an HIV-seropositive man and reviews the clinical features of erythema multiforme, SJS, and their association with drugs commonly used in the treatment of HIV infection.

A subset of patients with lupus erythematosus develops erythema multiforme-like skin lesions with speckled pattern of antinuclear antibodies, positive rheumatoid factor, anti-Ro (SS-A), and anti-La antibodies (SS-B), which known as Rowell's syndrome. We report an adolescent boy presented with erythema multiforme-like skin lesions and pericardial effusion; he fulfilled criteria of both Rowell's syndrome and systemic lupus erythematosus (SLE). Such initial presentation of SLE is rarely reported in literature.

A case of erythema multiforme-like allergic contact dermatitis due to midysalb, an ointment containing mephenesine is described. Patch testing showed marked sensitization to mephenesine, with positive reactions to different dilutions down to 0.1%. It is unlikely that the weak reaction to a further component, ethylene glycol monosalicylic acid ester, is responsible for the allergy. There are several other reports in the literature, so that mephenesine must be classed as one of the few allergens that cause reactions reminiscent of erythema multiforme.

BACKGROUND: No data exist as to Th2 chemokines in erythema multiforme (EM) and Stevens-Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN).
OBJECTIVE: To evaluate thymus- and activation-regulated chemokine (TARC), macrophage-derived chemokine (MDC) and regulated upon activation, normal T-lymphocyte-expressed and secreted chemokine (RANTES) expression in EM and SJS/TEN and to correlate with the serum levels of the Th1 promoter interleukin (IL)-12 and soluble Fas ligand (sFasL).
MATERIALS AND METHODS: IL-12, sFasL, TARC, MDC and RANTES expression were analyzed by ELISA techniques in 31 unt…

This paper draws attention to the relationship between the clinical and biological picture of SLE and the immune mechanisms of this disease. The presence, in the same patient, of erythema multiforme-like skin lesions and erythemato-squamous lesions specific for SLE together with a characteristic immune picture (speckled antinuclear antibodies (ANAs), positive anti-Ro antibodies, positive rheumatoid factor) raise the question of a relationship between the immune mechanisms in SLE and the clinical picture. A case of Rowell's syndrome is discussed: systemic lupus erythematosus diagnosed on the oc…

Non-genital herpes simplex virus in immunocompetent hosts causes a variety of primary infections--gingivostomatitis, keratoconjunctivitis, herpetic whitlow, and encephalomyelitis. Recurrent infections with orolabialis are very common, but are usually mild and self-limiting. Cutaneous complications of herpes simplex virus infections include eczema herpeticum and erytherma multiforme. Systemic treatment with acyclovir is indicated in encephalomyelitis, progressive eczema herpeticum, and frequent severe erythema multiforme. Chronic, suppressive acyclovir treatment may be helpful in severe recurre…

A 19-year-old male military recruit developed erythema multiforme 20 days after receiving a triad of vaccinations: smallpox (vaccinia virus), anthrax, and tetanus. Over the course of a few days, the erythema multiforme evolved into Stevens-Johnson syndrome, associated with widespread bullae, stomatitis, conjunctivitis, and fever. After 7 days of conservative management, the patient's signs and symptoms improved. This case serves as a timely reminder of a severe and potentially life-threatening complication of smallpox vaccination.

In immunocompetent patients, infection by the measles (rubeola) paramyxovirus produces fever, cough, coryza, Koplik's spots, and, on the skin, a macular erythema that can become confluent. The erythema has a striking cephalocaudal spread and clearing. The diagnosis of measles on a skin biopsy and the distinction from an erythema multiforme type of drug eruption can be difficult. We studied a skin biopsy from a patient with the acquired immunodeficiency syndrome (AIDS) who presented with measles. In contrast to erythema multiforme, the measles biopsy has necrosis of clusters of keratinocytes in…
