Effect of a mobile health intervention optimised with artificial intelligence on blood pressure and other cardiovascular risk factors in adults with high blood pressure: rationale and design of My Intelligent Cardiac Assistant (MICArdiac) randomised controlled trial.

INTRODUCTION: Control of blood pressure (BP) continues to be a challenge globally. Clinical trials have shown home BP monitoring and text-message interventions to lower BP. Integrating these to personalise supportive messaging in response to changing BP and activity through leveraging artificial intelligence (AI) could improve BP control. The aim of this trial is to examine the impact on BP, compared with a text-message only programme, of the My Intelligent Cardiac Assistant (MICArdiac) programme. MICArdiac is a 6-month programme comprising tracking of BP, physical activity and heart rate info

Does the exercise environment matter? Indoor versus outdoor aerobic exercise for adolescents with subthreshold depression: a randomised controlled trial protocol.

INTRODUCTION: Aerobic exercise, defined as rhythmic and continuous physical activity performed at moderate intensity to improve cardiorespiratory fitness, is widely recognised as an effective non-pharmacological intervention for improving the mental health of adolescents with subthreshold depression. However, whether the environment in which exercise is performed-such as indoor versus outdoor settings-modulates its psychological benefits remains insufficiently understood, as empirical evidence is still scarce. This study aims to compare the psychological effects of aerobic exercise conducted i

DIAGnosis using NOvel technology for Subtypes In Stroke (DIAGNOSIS study): performance evaluation protocol.

INTRODUCTION: Timely and accurate identification of stroke subtypes, particularly intracerebral haemorrhage (ICH) and large vessel occlusion (LVO), is essential for improving stroke outcomes. Biomarkers have shown potential in identifying these subtypes. Glial fibrillary acidic protein (GFAP), which is elevated in ICH, and D-dimer, which is associated with LVO, are promising for rapid, point-of-care identification. This study evaluates whether combining GFAP and D-dimer with clinical features improves the diagnostic accuracy for ICH and LVO. METHODS AND ANALYSIS: This is a multicentre, observ

Evaluating the effectiveness and acceptability of free door-to-door transport to increase the uptake of breast screening appointments in Yorkshire: a cluster randomised GP feasibility trial (DOORSTEP protocol).

INTRODUCTION: Breast screening uptake remains low in parts of the UK, partly due to barriers including limited transport access. Offering free transport to screening appointments may help address this and improve uptake. This general practitioner (GP) cluster-randomised feasibility trial will assess whether offering free door-to-door transport alongside routine screening invitations increases attendance. METHODS AND ANALYSIS: Eight general practices in Yorkshire will be randomised to either the intervention (routine invitation plus information about booking free door-to-door transport) or con

Study protocol for a hybrid I randomised clinical trial to evaluate an audit and feedback and a pharmacist-led intervention to reduce potentially inappropriate medications in older adults: the AIM study.

INTRODUCTION: Potentially inappropriate prescriptions (PIPs) in older adults, such as long-term use of benzodiazepines, proton pump inhibitors without indication or antipsychotics in dementia, are associated with adverse events and increased healthcare utilisation. Despite clinical guidelines discouraging their use, PIPs remain frequent in primary care. An audit and feedback (A&F) intervention of PIPs to general practitioners (GPs), led by pharmacists, may reduce the prescription of PIPs in primary care. METHODS AND ANALYSIS: A two-arm, pragmatic, controlled trial will be conducted to evaluat

On the crossroads of interdisciplinary medicine in amyloidosis - study protocol for a single-center interdisciplinary registry study.

BACKGROUND: Systemic amyloidosis comprises a heterogeneous group of rare diseases characterised by extracellular deposition of misfolded protein fibrils, leading to progressive organ dysfunction. Due to the variability in clinical presentation and course, collection of system-specific and longitudinal data is essential for understanding disease progression, treatment response and patient outcomes. At the Amyloidosis Center Charité Berlin (ACCB), a prospective amyloidosis registry has been established to systematically collect clinical, laboratory, imaging and patient-reported data with the aim

Timing of anticoagulation after emergency endovascular therapy for acute ischaemic stroke with atrial fibrillation (TIMERS): protocol for a randomised controlled trial.

INTRODUCTION: The optimal timing for direct oral anticoagulants (DOACs) initiation after acute ischaemic stroke in patients with atrial fibrillation (AF) remains unclear, particularly in those undergoing endovascular thrombectomy (EVT), who are at higher risk of both haemorrhagic transformation and early embolic recurrence. Patients undergoing thrombectomy have been under-represented in previous randomised trials, resulting in limited evidence for guiding management in this high-risk population. The TIMERS trial aims to evaluate the efficacy and safety of early versus delayed initiation of DOA

Diagnostic yield and safety of probe-based confocal laser endomicroscopy-guided pleural biopsy via semirigid thoracoscope in diagnosing patients with unknown pleural effusion: a protocol for a multicentre, randomised controlled trial (COLLABORATION-I).

INTRODUCTION: Semirigid thoracoscopy plays an important role in the diagnosis of pleural diseases. However, its diagnostic performance remains unsatisfactory particularly in terms of the negative likelihood ratio. Therefore, more effective supplementary diagnostic tools are required. Probe-based confocal laser endomicroscopy (pCLE), which allows live tissue imaging at the cellular level, can discriminate between malignant and benign pleura during medical thoracoscopy. However, the clinical relevance of pCLE in pleural disease remains unclear. This protocol describes a randomised controlled tri

Open-label, randomised controlled, multicentre study evaluating higher valency pneumococcal vaccines (PCV15/PCV20) compared with PCV13 given in homologous schedules at 3 months and 12 months ('1+1' schedule) and 2 months, 4 months and 12 months ('2+1' schedule) in infants: protocol.

BACKGROUND: Most recent childhood invasive pneumococcal disease in the UK is caused by serotypes not included in the currently used pneumococcal conjugate vaccine (PCV13). There is potential for additional protection with newer PCVs that have wider serotype coverage but there are no data on these vaccines being given in the 1+1 schedule used in the UK. This study will determine whether the PCV13 serotype-specific antibody responses to PCV15 and PCV20 given at 3 and 12 months (1+1 schedule) are non-inferior to PCV13 serotype-specific antibody responses to PCV13 given in the same schedule. It wi

Digital screening tool for the assessment of cognitive impairment in unsupervised setting-digiDEM-SCREEN: study protocol for a validation study.

INTRODUCTION: Dementia is one of the most relevant widespread diseases, with a prevalence of currently 55 million people with dementia worldwide. However, about 60-75% of people with dementia have not yet received a formal diagnosis. Asymptomatic screening of cognitive impairments using neuropsychiatric tests has been proven to efficiently enhance diagnosis rates. Digital screening tools, in particular, provide the advantage of being accessible without spatial or time restrictions. The study aims to validate a digital cognitive screening test (digiDEM-SCREEN) as an app in the German language.

Protocol for a prospective observational study linking research domain criteria-based phenotypes with clinical and care-related outcomes in psychiatric patients and healthy controls: the VeRDoC study.

INTRODUCTION: The Research Domain Criteria (RDoC) approach initiated by the National Institute of Mental Health provides a comprehensive framework for guiding research on mental illness and health. Since retrospective studies have indicated associations between RDoC characteristics and clinically relevant as well as care-relevant outcomes, there is a need for prospective, theory-driven investigations that systematically link a priori defined assessments of RDoC constructs to clinically and care-relevant outcomes in a transdiagnostic psychiatric sample. METHODS AND ANALYSIS: This prospective o
