BACKGROUND: The purpose of the study was to investigate the feasibility of laparoscopic paraaortic lymphadenectomy in the restaging of ovarian carcinomas.
METHODS: Nine patients in a referral center seen initially with ovarian (eight patients) or tubal (one patient) carcinoma who had experienced substandard staging during a previous laparotomy or laparoscopy underwent laparoscopic paraaortic lymphadenectomy as part of a surgical staging procedure that included peritoneal fluid sampling and multiple staging biopsies. The topographical distribution of oestrogen and progesterone receptors in the human endometrium and Fallopian tube was investigated by an immunocytochemical technique. A gradient of positively stained cells was observed: the highest oestrogen and progesterone receptor content was noted in the fundal part of the uterine cavity and the ampullar region of the Fallopian tube. The observed gradient is in keeping with biological and pathological events that occur in the human mullerian tract, e.g. fecundation, implantation and carcinogenesis.

Several studies have shown that gonadotrophin-releasing hormone analogue (GnRHa) offers a promising medical approach in the treatment of uterine leiomyomas. Medical management is very important especially when fertility is desired. We report on a case with mechanical infertility, in which the right tube was obstructed by cornual myoma and the left tube was resected due to a ruptured ectopic pregnancy. The myoma was reduced in size by GnRHa treatment and the patient subsequently conceived.

In 25 women with primary or secondary infertility, primary hysterosalpingography (HSG) or laparoscopy with chromopertubation indicated 33 proximal and 2 distal tubal obstructions. Four salpingectomies had been performed earlier. All patients were considered for transcervical catheter salpingoplasty (TCSP) to reconstruct tubal patency. Ovarian function and fertility was studied in 46 female New Zealand White rabbits after isthmic-isthmic reanastomosis of the oviduct. Postoperative ovarian function was slightly suppressed and returned to normal after two month. No significant differences were found between the control side and the side of anastomosis. Fertility was reduced directly after surgery on the side of anastomosis as well as on the control side. The outcome of ZIFT and IVF-ET was compared in consecutive nontubal factor patients in a prospective fashion. Groups did not differ in characteristics and were matched by the number of prezygotes/pre-embryos transferred. Overall, implantation, pregnancy, miscarriage, and ongoing PRs were not statistically different. These results suggest that ZIFT offers no significant advantage over IVF-ET for the treatment of nontubal infertility.

Over a 13-month period, 14 patients with proximal tubal obstruction underwent transcervical fallopian tube recanalization under fluoroscopic guidance in an outpatient setting at the hospital of the University of Pennsylvania. Malignant mixed Müllerian tumors are usually found in the endometrium, vagina, cervix, and ovary. It is extremely rare for this tumor to arise in the fallopian tube, and to date only 37 tubal cases have been reported. We recently experienced 2 such cases. From 1984 to 1988, 136 patients with ectopic pregnancy were treated by endoscopic means. In 29 cases the fallopian tube was removed. In 107 patients conservative treatment was undertaken. The salpingotomy was performed with the endocoagulator and with scissors or using lasers. The postoperative status of the tube was controlled in 54 patients by hysterosalpingography (n = 17) or by second-look laparoscopy (n = 37). In 38 cases the patency test was positive. The incidence of postoperative adhesions was minimal after laser salpingotomy (16%). The complication rate was low (2%).

The origin of gamma-aminobutyric acid (GABA) in the rat oviduct was investigated by measuring GABA levels in the oviduct after selective ablation of the extrinsic oviductal innervation. The literature reveals a total of only 25 cases of malignant mixed Müllerian tumor of the fallopian tube. Another case is presented; the patient is well and without evidence of disease more than 5 years after the operative treatment. She also received postoperatively adjuvant cytostatic treatment with cyclophosphamide, vincristine, adriamycin, and external radiation therapy. An aggressive mode of treatment is recommended in this rare disease due to the usually very poor prognosis.

We present a case of a one month old female infant who presented with left inguinal swelling. She was prematurely born at 32 weeks of gestational age.
