In utero effects of opioids on fetal growth.

BACKGROUND: Studies indicate antenatal opioid use is associated with birth size deficits, as evidenced by reductions in birth weight and head circumference. However, there remains a limited understanding of how early this growth restriction occurs, and what specific parameters are affected. This novel study evaluated global and specific growth deficits associated with prenatal opioid exposure between 18-22 weeks' gestation as assessed during anatomy ultrasounds. METHODS: Pregnant women who completed an anatomy ultrasound were identified via electronic medical records from a large academic obs

Evaluating the Growth Assessment Protocol for stillbirth prevention: progress and challenges.

Many stillbirths are associated with fetal growth restriction, and are hence potentially avoidable. The Growth Assessment Protocol (GAP) is a multidisciplinary program with an evidence based care pathway, training in risk assessment, fetal growth surveillance with customised charts and rolling audit. Antenatal detection of small for gestational age (SGA) has become an indicator of quality of care. Evaluation is essential to understand the impact of such a prevention program. Randomised trials will not be effective if they cannot ensure proper implementation before assessment. Observational stu

Immune dysfunction in developmental programming of type 2 diabetes mellitus.

Intrauterine growth restriction (IUGR) is a common complication of pregnancy and increases the risk of the offspring developing type 2 diabe … ry to nutrient and oxygen deprivation. Pancreatic-resident macrophages populate the pancreas during fetal development, and their phenotype is dynamic through the neonatal period. Furthermore, macrophages in the islets are instrumental in islet development as they influence β-cell proliferation and islet neogenesis. In addition, cytokines, derived from β-cells and m …

Placental mitochondria adapt developmentally and in response to hypoxia to support fetal growth.

Mitochondria respond to a range of stimuli and function in energy production and redox homeostasis. However, little is known about the developmental and environmental control of mitochondria in the placenta, an organ vital for fetal growth and pregnancy maintenance in eutherian mammals. Using respirometry and molecular analyses, the present study examined mitochondrial function in the distinct transport and endocrine zones of the mouse placenta during normal pregnancy and maternal inhalation hypoxia. The data show that mitochondria of the two zones adopt different strategies in modulating thei

The impact of chronic intrauterine inflammation on the physiologic and neurodevelopmental consequences of intermittent umbilical cord occlusion in fetal sheep.

OBJECTIVE: To determine the effect of intrauterine inflammation on fetal responses to umbilical cord occlusion (UCO). STUDY DESIGN: In pregnant sheep, lipopolysaccharide (LPS) or saline (SAL) was infused intra-amniotically for 4 weeks from 80 days of gestation (d). At 110 d, fetuses were instrumented for UCOs (5 × 2-minutes, 30-minute intervals: LPS + UCO, n = 6; SAL + UCO, n = 8) or no UCO (sham, n = 6) on 117 and 118 d. Tissues were collected at 126 d. RESULTS: Fetal physiological responses to UCO were similar between LPS + UCO and SAL + UCO. Histologic chorioamnionitis and increased amnio

Individualized growth charts for ultrasound measurements can significantly improve fetal monitoring.

OBJECTIVE: For monitoring pregnancies it is useful to reduce unnecessary examinations due to wrongfully assumed growth retardation in cases with a small fetal growth potential. It also makes sense to improve the detection of objectively retardated children in order to a disproportionately high growth potential. It was the aim of this study to modify the standard growth charts for ultrasound parameters according to parental influences to better display the individual growth potential. MATERIAL AND METHODS: In a prospective trial 1,390 ultrasound measurements of 174 completed pregnancies were o

Growth of the tibia and fibula bones in human fetuses.

The growth of the long bones has been a subject of great importance lately, because it provides non invasive diagnosis of prenatal malformations. This study examines 116 tibia and fibula bones of Brazilian fetuses (17 males and 12 females) with gestational age ranging from the 16th to the 34th week postconception. Only fetuses with no visible congenital malformations and with normal biometric parameters were considered. The bones were measured with a 0.1 mm precision caliper and their Total Length (TL), the Ossified Diaphysis Length (ODL), the Diaphysis width (DW), the Proximal Epiphyss length

Metabolic control of feto-placental development and pregnancy outcomes.

The intricate processes of feto-placental development are regulated by both genetics and the nutritional environment. The placenta mediates nutrient transfer to the fetus and waste transfer to the maternal circulation to facilitate proper fetal cell fate specification. Disruptions in these fate-regulatory programmes can lead to maternal and fetal complications. We explore how metabolic-epigenetic mechanisms programme feto-placental development, discussing the roles of metabolic sensing mechanisms. This Review covers the roles of nutrient metabolite-dependent protein modifications, lipid signal

Hemodynamic changes in pregnancies with impaired fetal growth: A systematic review and meta-analysis.

INTRODUCTION: Abnormalities in central hemodynamic functions before and throughout pregnancy may antedate impaired fetal growth. We aimed to assess cardiac output (CO) and total peripheral vascular resistance (TPVR) trajectories throughout singleton pregnancies with and without impaired fetal growth by systematic review and meta-analysis. MATERIAL AND METHODS: PubMed and Embase were systematically searched (inception - July 2023), and reference lists were screened. Studies reporting CO and TPVR during singleton pregnancies complicated by impaired fetal growth were included. Studies measuring

The structure of the porcine uterine-conceptus interface is associated with gestational day, fetal size and sex.

This study aimed to characterize histological changes of the maternal-conceptus interface in feto-placental units associated with fetal weight and sex throughout pregnancy. Pregnant Large-White X Landrace gilts(n=18) were euthanized and hysterectomized on gestational days[GDs] 30(n=3), 45(n=5), 60(n=5), and 90(n=5). Intact cross-sections of fetoplacental interface associated with the lightest[LW] and normally-grown[NW] littermates were collected on GD30(n=4 per size). On GDs 45, 60 and 90, interactions between fetal size and sex were investigated in light-weight males[LWM] and females[LWF]; no

A conditional standard for the customized fetal growth trajectory in twin pregnancy.

BACKGROUND: The difference in the birth weights between twins and singletons grows with advancing gestation. Although many fetal weight standards based on ultrasonographic measurements have been created for tracking fetal growth in twin pregnancies, their applicability to other groups is limited by the fact that they are population specific. OBJECTIVE: This study aimed to generate conditional centiles for growth assessment of twin fetuses and to compare them with other population-based growth standards for singleton and twin fetuses. STUDY DESIGN: This was a retrospective, longitudinal study

Placental plastics in young women from general population correlate with reduced foetal growth in IUGR pregnancies.

Constant exposure to plastics particulates has raised concerns against human health, particularly when it comes to birth outcomes. The present study explores the first appraisal of plastic particles in fresh human placenta and its association with foetal growth in neonates. Specifically, 43 pregnant women from general population were selected and their placentas were analyzed by digital microscopy and Raman microspectroscopy for microplastics (MPs <5 mm). We used regression analysis to estimate associations between MPs count in placenta and neonatal anthropometric measurements. MPs were found

Maternal asthma and early fetal growth, the MAESTRO study.

BACKGROUND: Several maternal conditions can affect fetal growth, and asthma during pregnancy is known to be associated with lower birth weight and shorter gestational age. OBJECTIVE: In a new Swedish cohort study on maternal asthma exposure and stress during pregnancy (MAESTRO), we have assessed if there is evidence of early fetal growth restriction in asthmatic women or if a growth restriction might come later during pregnancy. METHODS: We recruited women from eight antenatal clinics in Stockholm, Sweden. Questionnaires on background factors, asthma status and stress were assessed during pr

Genetic syndromes associated with isolated fetal growth restriction.

Early onset fetal growth restriction (FGR) may be due to impaired placentation, environmental or toxic exposure, congenital infections or genetic abnormalities. Remarkable research, mainly based on retrospective series, has been published on the diverse genetic causes. Those have become more and more relevant with the improvement in the accuracy of the analysis techniques and the rising of breakthrough genomewide methods such as the whole genome sequencing. However, no publication has presented an integrated view of management of those fetuses with an early and severe affection. In this review

Ghrelin to obestatin ratio in maternal serum in pregnancies complicated by intrauterine growth restriction.

Ghrelin, an endogenous for the growth hormone secretagogue receptor, has been shown to participate in fetal growth. Obestatin, encoded by the same gene as ghrelin, is described as a physiological opponent of ghrelin. This study was designed to determine the changes of ghrelin/obestatin ratio in maternal serum in pregnancies with intrauterine growth restriction (IUGR). The authors found that the ghrelin levels in maternal serum were significent lower in IUGR group than in control group (236.34 ± 14.58 pg/ml vs. 321.49 ± 18.19 pg/ml, p = 0.003). However, the difference of obestatin levels in mat
