Management of Large Bone Defects in Diaphyseal Fractures by Induced Membrane Formation by Masquelet's Technique.

INTRODUCTION: Management of the large gap in long bone fractures is a challenging problem after compound injuries. A novel technique called as Masquelet's technique of "induced membrane formation", is used to bridge a gap of more than 5 cm using bone cement as a spacer in first stage and autologous cancellous bone graft to fill the gap in second stage. CASE PRESENTATION: We present two different and difficult cases with bone defects after open injuries associated with long bone fractures in this paper. First case is a 50-year-old lady with grade IIIa open fracture right distal femur with intr

[Treatment of bilateral distal tibial nonunions. Use of an intramedullary compression nail and a modified locking compression plate osteosynthesis].

Operative treatment of distal tibial fractures remains a challenge for the surgeon even today. The soft tissues demand atraumatic operative techniques, although an anatomical reduction of the articular fracture component is mandatory. The nonunion rate increases with disturbed local blood supply, widened fracture gap, unstable fixation. If a nonunion occurs, an individual treatment concept is required, so that even difficult situations can be successfully managed, as described in our case with bilateral tibial nonunions.

Radiological assessment of traumatic bone injuries secondary to motorcycle accidents in Ghana: a multi-center cross-sectional study.

INTRODUCTION: Motorcycle accidents are a leading cause of traumatic injuries, including bone fractures, dislocations, and soft tissue damage. Despite the growing concern about motorcycle-related injuries, there is a paucity of research. This study assessed traumatic bone injuries secondary to motorcycle accidents in Ghana. METHODS: This multi-center quantitative study employed a cross-sectional design to examine plain X-ray reports of 1736 patients with traumatic bone injuries due to motorcycle accidents from January 2020 to December 2023. Data collected were analyzed using SPSS version 25. S

Definitive treatment of persistent frontal sinus infections: elimination of dead space and sinonasal communication.

BACKGROUND: Frontal sinus injury involving nasofrontal outflow tract obstruction is routinely managed by obliteration or cranialization; however, a small percentage of patients develop persistent indolent infections despite routine measures. The authors discuss the course of persistent infection following frontal sinus fractures and propose a novel treatment that definitively obliterates and separates the nasofrontal outflow tract from the cranium in these patients. METHODS: Seven consecutive patients with persistent indolent infections associated with frontal sinus fractures were identified

The tuning fork test--a useful tool for improving specificity in "Ottawa positive" patients after ankle inversion injury.

The Ottawa ankle rules are a clinical instrument calibrated towards a high sensitivity in order not to miss any fractures of the ankle or mid-foot. This is at the cost of a mean specificity of around only 32%. The aim of this study was to determine the suitability of tuning fork testing in combination with existing Ottawa guidance for increasing the specificity in detecting fractures of the lateral malleolus. A prospective pilot study was carried out, in which a single trained investigator examined all patients with already "Ottawa positive" findings for possible lateral malleolus injury by ap

Prevalence of Cozen's Phenomenon of the Proximal Tibia.

BACKGROUND: There has been little investigation into the epidemiology of Cozen's phenomenon, genu valgum as a complication of proximal tibial metaphysis fractures. In this study, we present a large case series of proximal tibia fractures in children to describe the prevalence, epidemiology, and characteristics associated with Cozen's phenomenon following proximal tibial fracture in children. METHODS: We searched the orthopedic database at our institution for all patients aged 1 to 8 years old with proximal tibia fractures presenting for initial fracture management between January 1, 2014 and

Intercalary Diaphyseal Endoprosthetic Reconstruction.

Intercalary endoprosthetic devices are a diaphyseal segmental reconstructive option for both primary tumors and skeletal metastases, most used for pathological fractures or failure of internal fixation. Numerous designs have been employed with varying success. These implants require adequate quality and length bone stock, both proximal and distal, to be employed. Alternative reconstructions may include vascularized fibula autograft, allograft reconstruction, acute shortening, and fixation with cement spacer augmentation with planning staged procedures. The reported functional results and compl

Evaluation of complications and flap losses in mandibular reconstruction with microvascularized fibula flap.

The mandibular defects caused by mandibulectomies can involve esthetic and functional sequelae in patients. The fibula presents positive aspects and can be used as an option for mandibular reconstruction after tumor resections or extensive traumas. Furthermore, this retrospective study of the patients who have passed through a mandibular reconstruction with a microvascular fibular flap over the last 17 years describes the experience of the Oral and Maxillofacial SurgeryService at the ErastoGaertner Hospital, Curitiba/PR. The use of this flap type has a complication rate of 32.3%, which include

Reconstruction of skeletal defects in the femur with 'two-strut' free vascularized fibular grafts.

Large skeletal defects of the femur caused by infected nonunions remain a challenge to orthopedic surgeons. Conventional bone grafting may not succeed when the recipient bed is not ideal. Single fibular grafts were proven very useful in bridging this type of defect, yet were complicated by the high incidence of refracture and the need for protection of long duration. By dividing the fibula into two struts connected by the nutrient vessels, we doubled the effective cross-sectional area of bone grafting with only one set of vascular anastomoses. Five cases of infected nonunion of the femur after

[When the leg is the victim of the foot].

Man (homo-erectus, plantigrade) is in constant contact with the ground via his feet which are platforms on which the legs and, above all, the entire body rest. When the body moves, the only segment which remains stable and fixed is the foot, which forms the point of departure of all the muscular actions of this moving body. The slightest deformity of this platform, this foot, will lead to skeletal, musculotendinous and neurovascular problems in the legs, and to pain. Pain due to skeletal problems may be related to fatigue fractures (tibia, fibula) and to posterior tibial periostitis. Pain may

Primary Ankle Fracture Dislocation Is Not a Negative Prognostic Factor for the Surgical Treatment of Syndesmotic Injury-A Retrospective Analysis of 246 Patients.

Background/Objectives: Acute ankle sprains are common injuries that significantly affect both sports and daily activities. Syndesmotic injuries, a specific type of ligamentous damage, can occur as a part of a sprain or alongside fractures, affecting approximately 20% of ankle fractures. The aim of this study was to evaluate negative prognostic factors influencing surgical outcomes in tibiofibular syndesmotic injuries associated with ankle fractures. Methods: Data from 246 patients were analyzed to examine the impact of initial ankle dislocation, fracture type, and fixation method on postoperat

Microbial Profile and Antibiotic Sensitivity Patterns in Open Fractures: A Retrospective Study from a Tertiary Care Center.

INTRODUCTION: Open fractures are orthopedic emergencies with a high risk of infection due to soft-tissue compromise and bone exposure. Early antibiotic administration and wound debridement are critical to reducing infection-related complications. However, microbial profiles and antibiotic sensitivity patterns vary regionally, warranting local evaluation. MATERIALS AND METHODS: This retrospective study was conducted over 24 months at a tertiary care center in North India. A total of 67 patients aged ≥18 years with open fractures were included. Wound swabs were obtained either within or after 6

Changes in pediatric fracture patterns presenting to US emergency departments before, during, and after the COVID-19 pandemic.

The purpose of this study was to analyze the demographics of pediatric fracture patients before, during, and after the COVID pandemic using US national emergency department (ED) data. The National Electronic Injury Surveillance System (NEISS) data for the years 2018 through 2021 was extracted for those <16 years of age, and organized into 24 consecutive bimonthly groups. There was a decrease in the number of ED visits for fractures in 2020 and returned to pre-COVID levels by March/April of 2021, except for small hospitals which demonstrated an earlier rebound beginning in late 2020. During the

Ankle fractures in the elderly: Treatment and results in 477 patients.

INTRODUCTION: In the elderly, ankle fractures are likely to cause specific complications and have a major impact on their autonomy. The goal of this multicentre study was to assess these outcomes in a geriatric population treated operatively. MATERIAL AND METHODS: This retrospective study included 477 patients with ankle fractures treated surgically between 2008 and 2014. The minimum age was 60years for women and 70 for men. Patients with a tibial pilon fracture or less than 3months' follow-up were excluded. Functional (autonomy and comorbidities) and radiological assessments were performed b

Novel reconstruction of an open pilon fracture with significant bone loss.

This is a case of how a polytrauma patient was managed successfully. This 47-year-old woman was a victim of a detonated improvised explosive device in a terror attack. She had multiple injuries and underwent damage control surgery. She suffered significant bone loss with an open distal tibia pilon fracture, which we reconstructed with novel techniques. About 18 months postoperative, she is mobilising without the use of any aids.

Triplane fractures of the distal tibial epiphysis.

Triplane distal tibial fractures can occur as two-, three-, or four-part fractures with or without a fibular fracture. Diagnosis of the particular anatomy of each fracture is ascertained by plain radiographs; if the fracture is displaced 2 mm or more on any view, anteroposterior and lateral tomograms and, if possible, a limited computerized tomography (CT) scan should be done. A plaster cast in situ for non-displaced fractures or closed reduction for displaced fractures should be attempted first by internal rotation and anterior movement of the fibular metaphyseal piece. Failure to obtain and/

[Surgery of bimalleolar fractures. Results of a series of 70 cases with a follow up of 3 to 8 years (author's transl)].

The authors analyse a series of 70 bimalleolar fractures treated surgically with a follow up of 3 to 8 years. The late functional and radiological result was favourable in 85.7% of cases. They were able to determine from this series a certain number of unfavourable factors on which surgery has a better influence than immobilisation: e.g. fractures with a large displacement, impaction of the roof of the mortar, open fractures. Complications were rare; in 50% they were the result of technical errors avoidable with experience. The indications are noted; the authors consider that certain fracture

Treatment of pediatric tibial shaft nonunion using ipsilateral free non-vascularized fibular graft.

BACKGROUND: Although tibial shaft fractures are the third most common long bone fractures in children after the forearm and femur, nonunion of these fractures are rare in the pediatric population. CASE REPORT: Despite seldom seen, tibial nonunion is very complex and it is also a devastating complication of tibial fracture especially when infected. Numerous methods have been employed to treat pediatric tibial nonunion, but there is no consensus. Here, we present a case of a child with right tibial shaft fracture nonunion. We treated this patient with ipsilateral free non-vascularized fibular g
