[Combined dorsal injuries of the thumb. A review of treatment possibilities].

In complex dorsal injuries of the thumb with an intact CM-joint, the reconstruction of a stable post for grip is of greater importance than restoring mobility to the MP- or IP-joint. The basis of reconstruction is primary stable internal fixation of the fractured bones, in some cases necessitating an arthrodesis or interposition of cortico-cancellous bone grafts. Extensor tendons are repaired or reconstructed if distal joint function is to be expected. In order to prevent neuroma formation, direct nerve repair is favoured, but nerve-grafting is not recommended. In exclusively dorsal lesions, t

A rare presentation of a complex Galeazzi fracture-dislocation and a review of literature on complications: dislocation and interposition of the deep flexor of the fifth finger.

Fractures of the distal third of the radius often involve instability of the distal radioulnar and secondary stabilizers. In some cases, reduction of the distal radioulnar is prevented by the interposition of other dislocated structures. Dislocation and interposition of the deep flexor of the fifth finger is rarely observed in this type of fracture. The authors present a review of the literature and the case of a 36-year-old female athlete who presented to the emergency room after a trauma to the wrist, during a barbell exercise. A first radiographic examination showed a fracture of the distal

[Arthrogryposis multiplex congenita--a rare congenital stiff joints syndrome].

INTRODUCTION: Arthrogryposis multiplex congenita is not a disease but a term describing multiple congenital contractures. Etiological factors include neurological and primary myogenic diseases. This rare syndrome is present at birth and is characterized by reduced mobility of many joints. The contractures involve two or more joints with ankylosis. The accompanying musculature is hypoplastic, but multiple pterygia are also present. Arthrogryposis multiplex congenita is a heterogeneous group of disorders with the incidence rate of 6.2/100000 liveborn infants. The true incidence cannot be establi

Assessment of body-powered 3D printed partial finger prostheses: a case study.

BACKGROUND: Traditional prosthetic fabrication relies heavily on plaster casting and 3D models for the accurate production of prosthetics to allow patients to begin rehabilitation and participate in daily activities. Recent technological advancements allow for the use of 2D photographs to fabricate individualized prosthetics based on patient anthropometrics. Additive manufacturing (i.e. 3D printing) enhances the capability of prosthesis manufacturing by significantly increasing production speed and decreasing production cost. Existing literature has extensively described the validity of using

One-Stage Extension Shortening Osteotomy for Syndromic Camptodactyly.

Syndromic camptodactyly often affects multiple fingers, and severe deformities are common compared to idiopathic camptodactyly. This study aimed to evaluate the use of a one-stage extension shortening osteotomy of the proximal phalanx for patients with syndromic camptodactyly without tendon surgery. Forty-nine cases of syndromic camptodactyly were included. Forty fingers (81.6%) were associated with arthrogryposis multiplex congenita, and nine (18.4%) with other syndromes. Six fingers presented with a moderate form (30° to 60°) of camptodactyly, whereas 43 fingers manifested the severe form (>

Predictors of outcome after primary flexor tendon repair in zone 1, 2 and 3.

UNLABELLED: We retrospectively reviewed the outcomes of flexor tendon repairs in zones 1, 2 and 3 in 356 fingers in 291 patients between 2005 and 2010. The mean (standard deviation) active ranges of motion of two interphalangeal joints of the fingers were 98° (40) and 114° (45) at 8 weeks postoperatively and at the last follow-up (mean 7 months, range 3-98), respectively. Using the Strickland criteria, 'excellent' or 'good' function was obtained in 95 (30%) out of 322 fingers at 8 weeks and 107 (48%) out of 225 fingers at the last follow-up. A total of 48 (13%) fingers required reoperation bec

The association between Turner's syndrome and juvenile rheumatoid arthritis.

BACKGROUND: Turner's syndrome (TS) is a chromosomal disease frequently associated with autoimmune conditions including thyroid disease, inflammatory bowel disease, and diabetes. Recent reports have described an association with juvenile rheumatoid arthritis (JRA) and psoriatic arthritis. We describe three additional cases of TS associated with JRA. OBJECTIVE: The objective of this report is to describe the radiographic and clinical features of TS associated with JRA to heighten awareness of this association and alert radiologists to recognize the superimposition of radiographic bony changes o

Free vascularized whole joint transfer in children.

Reconstruction of the traumatized finger joint with epiphyseal destruction has long been problematic. Since free vascularized whole joint transfer was introduced as a treatment for joint and epiphyseal destruction, this procedure has been selected as an alternative treatment because it may provide a growing epiphysis. We have reviewed our series of 19 joint transfers. Mean age at operation was 6.2 years (range 3 to 12). Average active range of motion was 31 degrees/61 degrees for the group with posttraumatic reconstruction (n = 12) and 21 degrees/43 degrees for the group with reconstruction of…

[Comparison of functional results after reconstruction of the thumb].

Between 1980 and 2000, 26 pollicizations of the index finger and 32 transplantations of the second toe were performed for reconstruction of the thumb after traumatic loss. Twenty-three patients with index finger pollicization and 26 patients with second toe transplantation could be reviewed after an average follow-up of 6.4 years. Vascular complications occurred in two toe transplantations and one index finger pollicization. One transplanted toe was lost after vascular complications. Mobility of the interphalangeal joints was reduced after both procedures. Pinch grip was achieved in 20 of 23 p

Swing traction versus no-traction for complex intra-articular proximal inter-phalangeal fractures.

INTRODUCTION: Traction orthoses are thought to optimize recovery from intra-articular finger fractures by restoring joint space and allowing early motion. Evidence to date has, however, consisted only of case series. PURPOSE OF THE STUDY: To compare swing traction versus no-traction management of complex fractures of proximal inter-phalangeal (PIP) finger joints. We hypothesized that there is no long-term (i.e. >12 month) difference between swing traction and no-traction (with or without surgical fixation) in terms of motion, pain, function, patient satisfaction, or treatment cost. METHODS: …

[Results of the reconstruction of the middle phalangeal base of the finger through an osteochondral autograft from the hamate bone].

OBJECTIVE: The fractured base of the middle phalanx was reconstructed in 13 patients using an osteochondral transplant from the carpometacarpal joint surface of the hamate bone. The goal was to restore joint stability with preservation of mobility. Indications were acute and missed isolated destruction of the palmar middle phalanx base ≥ 30 %. Contraindications were destruction of the head of the proximal phalanx, advanced chondropathy of the head of the proximal phalanx, and extensive soft tissue injury with loss of skin coverage for the proximal interphalangeal joint. Progression of Dupuytren Contracture: A Randomized Controlled Trial Comparing Surgery, Needle Fasciotomy, and Collagenase Injection.

BACKGROUND: Dupuytren contracture can be treated by surgery, needle fasciotomy, or collagenase injection, but how these treatments affect disease progression in both treated and untreated fingers is poorly understood. This study aimed to compare the progression of Dupuytren contracture across these 3 treatments over a 2-year follow-up and identify patients at highest risk for progression and retreatment. METHODS: This finger-level analysis used data from DETECT (Dupuytren Treatment Effectiveness Trial) and included 302 participants with 423 initially treated fingers. Progression was defined a

Imaging Helps Guide Physical Therapy Treatment in a Patient With Diffuse Systemic Sclerosis (Scleroderma).

A 50-year-old woman with an 8-year history of diffuse systemic sclerosis, a form of scleroderma, was referred by her rheumatologist to physical therapy for decreased finger range of motion (ROM) and pain that adversely affected her dexterity and ability to perform activities of daily living. To determine whether joint mobilization would be appropriate for this patient, posterior-to-anterior and modified lateral radiographs of both hands were requested by the physical therapist. Images showed significant bone loss in the distal phalanges of both thumbs and in the left third and fourth digits, a

Hemi-hamate arthroplasty for pilon fractures of finger.

BACKGROUND: Injury following proximal interphalangeal joint fracture dislocation is determined by the direction of force transmission and the position of the joint at the time of impact. Dorsal dislocations with palmar lip fractures are the most frequently encountered. The degree of stability is directly determined by the amount of middle phalangeal palmar lip involvement. MATERIALS AND METHODS: Hemihamate arthroplasty procedure was used in the reconstruction in five cases with comminuted, impacted fractures of the proximal end of middle phalanx of the finger. Three patients were presented wi

Acute calcific periarthritis of proximal interphalangeal joint in a professional golfer 's hand.

This report describes a 22-yr-old professional golfer with acute calcific periarthritis of the proximal interphalangeal joint of the 4th finger. We considered that the major cause for this condition may be the repeated minor traumas to the affected site. Also, since this condition is self-limiting, a correct diagnosis is required to avoid unnecessary tests and surgery.

Primary shortening of the pollicized long flexor tendon in congenital pollicization.

Primary shortening of the flexor digitorum profundus tendon to the pollicized index finger was routinely performed during all congenital pollicizations over a 3-year period. Interphalangeal joint flexion of the pollicized digit was observed to begin within weeks after pollicization. The rebalancing of the pollicized long flexor musculotendinous unit has the potential for greater long-term strength.

Thyromental distance measurement--fingers don't rule.

Thyromental distance (TMD) measurement is commonly used to predict difficult intubation. We surveyed anaesthetists to determine how this test was being performed. Comparative accuracy of ruler measurement and other forms of measurement were also assessed in a meta-analysis of published literature. Of respondents, 72% used fingers for TMD measurement and also considered three finger widths the minimum acceptable TMD. In terms of distance, the minimum acceptable TMD was felt to be 6.5 cm by 55% of respondents. However, the actual width of three fingers was (range) 4.6-7.0 cm (mean 5.9 cm), with

A Novel Relative Motion Splint Fabrication Technique Using Low-cost and Readily Available Materials.

This article described the fabrication and clinical application of a simple relative motion extension orthosis constructed using readily accessible materials such as a wooden tongue depressor, an aluminum strip, and a cohesive bandage. Initially developed to facilitate early active mobilization following extensor tendon repairs, the relative motion extension orthosis is applied by positioning the metacarpophalangeal joint of the injured finger 15-20 degrees more extended relative to the adjacent fingers. The proposed orthosis, produced with low-cost and easily obtainable materials, is consider
