The occurrence of multiple sclerosis in clustered groups of cases that are often related to others in time and place has been observed on several occasions in the last 50 years. Selected clusters are here reviewed in relation to suspected sources of heavy metal (mercury, lead) poisoning as background for the analysis of the 1983-1985 "outbreak" of 30-40 cases of multiple sclerosis in Key West, Florida. Evidence is presented that the time-place clustering resulted from environmental pollution stemming from a nearby dump pile of rocky debris. The probable mechanism is discussed.

Spread of shigellosis within day care centers is difficult to control, and illness may spread into the community. We investigated two sequential outbreaks of diarrhea caused by multiply resistant Shigella sonnei in children at day care centers in 2 neighboring counties in Florida. We used plasmid analysis to identify a single epidemic strain causing both outbreaks and to assess the control measures used. One of the control measures, keeping ill children from attending the center, did not prevent spread of illness in the community.

University Medical Center in Jacksonville initiated a screening program for maternal serum alphafetoprotein (MSAFP) in October 1987. Women with both high and low values were recalled for testing. During the first two years, neural tube and other birth defects including cystic hygroma and gastroschisis were detected. No chromosomal abnormalities were found. The program meets the recommendation of the American College of Obstetrics and Gynecology to offer MSAFP testing to all pregnant women. Pregnant women in other areas of Florida could receive services through similar programs.

A study by hemagglutination-inhibition test showed that 19 strains of eastern equine encephalitis virus grouped themselves in two main types, which have been designated North American and South American. The former consists of ten strains from the eastern half of the United States, from Massachusetts to Florida; Jamaica, the Dominican Republic, and, subject to confirmation, Thailand. The South American type comprises nine strains from Panama, Trinidad, British Guiana, Brazil, and Argentina. The strains were isolated from different natural hosts over a period of 30 years.

OBJECTIVES: Nursing home residents are especially vulnerable to adverse outcomes after a hurricane. Prior research suggests that emergency department (ED) visits increase among community-residing older adults after natural disasters. However, little is known about the impact of hurricanes on the large population of older adults residing in assisted living (AL) settings, particularly the influence of storms on the rates and causes of ED visits.
