External skeletal fixation versus cement fixation in the treatment of redislocated Colles' fracture.

In a prospective investigation in 1984 and 1985, all Colles' fractures (not comminuted or intraarticular) that redisplaced after two closed reductions were allocated at random to one of two groups: Group I (n = 23), in which the dorsal bone deficiency was filled with bone cement (methylmethacrylate), and Group II (n = 25), in which the fracture was treated with external fixation. All were followed for one year and examined with respect to anatomy of the fracture, wrist movement, strength, appearance, pain, function, and complications. Roentgenograms, 12 months after treatment, showed that all

Anterior chondral buttress plating for complex coronal shear fractures of the distal humerus. A retrospective case series.

BACKGROUND: Internal fixation of complex coronal shear distal humerus fractures is challenged by the absence of metaphyseal osseous extension often limiting fixation strategies to intra-articular screws or wires alone. Complex injury patterns involving the entire articular surface, presence of comminution and impaction, may need additional fixation. Supplemental plating of these injuries is currently limited to the posterolateral distal humeral surface, which is biomechanically not favorable to provide an efficient buttress fixation. Anterior plating provides the best possible neutralization o

Risk Factors for Postoperative Infection and Associated Outcomes After Pilon Fracture Fixation: A Propensity-Matched Cohort Study.

BACKGROUND: Pilon fractures are complex, often involving severe comminution and soft tissue compromise. Risk factors for infection after ankle fracture definitive fixation have been associated with open and/or comminuted fractures and poorly managed diabetes. However, infection risk following pilon fractures have not been well studied, and the effect of infection on other postoperative outcomes have not been widely discussed. This study aims to identify risk factors for developing infection following pilon fracture and assess the effect of infection on postoperative outcomes. METHODS: This st

Locking plates for distal fibula fractures in young and elderly patients: A retrospective study.

BACKGROUND: Ankle fractures are common injuries in the young and elderly populations. To prevent post-traumatic arthritis, an anatomic reconstruction of the ankle structure is mandatory. Open reduction and internal fixation is the treatment of choice among orthopaedics. Conventional plates allow stability of the fracture if bone quality is present. Locking plates might offer an advantage for the treatment of lateral malleolar fracture in patients with comminution, severe instability, distal fractures, or osteoporotic bone. Our hospital introduced a new locking plate for fracture of the distal

Surgical technique: static intramedullary nailing of the femur and tibia without intraoperative fluoroscopy.

BACKGROUND: On a recent mission directed at definitive care for victims of the Haitian earthquake, the orthopaedic team developed a technique for freehand distal locking of femoral and tibial nails without intraoperative fluoroscopy or proximally mounted targeting jigs. DESCRIPTION OF TECHNIQUE: After performing open antegrade or retrograde nailing by standard techniques, the freehand lock must be obtained before doing standard outrigger locking. This allows the surgeon to control the nail and deliver the locking hole in the nail to a unicortical drill hole in the femur. Before nail insertion

Use of the Minimally Invasive Reduction Instrumentation System for Facilitating Alignment and Reduction When Performing Minimally Invasive Plate Osteosynthesis in Three Dogs.

The Minimally Invasive Reduction Instrumentation System (MIRIS) was utilized to facilitate minimally invasive plate osteosynthesis (MIPO) of distal limb diaphyseal comminuted fractures (2 crural, 1 antebrachial) in three dogs. The MIRIS facilitated efficient MIPO in all three fractures. Radial and tibial lengths were restored within 2% of the length of the intact bone and postoperative frontal and sagittal plane angulation were within 3° of the normal contralateral limb for each of the fractures. Fixation failed in one of the tibial fractures when the plates bent a week following surgery. The

Recent advances in the treatment of gunshot fractures of the humeral shaft.

The increase in violent crime has lead to an increase in gunshot-induced fractures in the United States. Injuries to the upper extremity are common. The treatment of gunshot injuries depends on the extent of soft tissue disruption and the type and location of fracture. Most of these injuries are a result of low-energy gunshot wounds that may be treated conservatively under the appropriate conditions. Low-energy fractures that require operative stabilization may be done with predictable results for achieving union with intramedullary fixation or compression plating. Complex open fractures from

Management of frontal sinus fractures. Changing concepts.

Since the turn of the century, surgeons have handled frontal sinus fractures with a variety of different procedures. The optimal management procedure remains controversial. We have presented a graduated anatomic algorithm for treatment of frontal sinus fractures based on the degree of fracture displacement and nasofrontal duct involvement and presence of CSF leak. Nondisplaced fractures are best handled conservatively, without operative intervention. However, the majority of frontal sinus fractures require operative correction. Uncomplicated anterior table displacement with an aesthetic deform

Measurement of globe position in complex orbital fractures. II. Patient evaluation utilizing a modified exophthalmometer.

Globe position was assessed by both the Hertel exophthalmometer and the modified external auditory canal (EAC)-fixated device in 27 patients with complex orbital fractures (18 tripod and 9 Le Fort fractures). Although 94% of the patients with tripod fractures had relative exophthalmos on the fracture side or no difference between eyes by Hertel exophthalmometry, greater than 30% of the same patients showed relative enophthalmos when measured by the EAC-fixated device. In three of four patients undergoing surgical repair of the orbital floor, modified exophthalmometry showed exophthalmos of gre

[The incidence, therapy and results of combined ankle joint and tibial shaft fractures].

Forty combined tibial shaft and ankle fractures were admitted to our Trauma Department between 1980 and 1985; these cases were followed up, and the results are presented. Using the classification scheme of Johner/Wruhs for lower-leg fractures and that of Lauge-Hansen for ankle fractures, we found 25 pronation-eversion and 14 supination-eversion lesions combined with pure spiral fractures (A1, 9 cases), spiral bending-type fractures (B1, 25 cases), comminuted spiral lesions (C1, 5 cases), and 1 crush fracture (C3). Although no fibular or ligamentous reconstruction had been done and various meth

Scapulothoracic dissociation: a rare variant: a case report.

ABSTRACT: Scapulothoracic dissociation is a rare injury involving separation of scapula from the thorax along with the upper extremity. Majority of the patients have concomitant neurovascular injury and the prognosis is uniformly poor in such cases. We present a case of scapulothoracic dissociation with comminuted fracture of scapula and acromioclavicular joint disruption without neurovascular deficit. There were associated avulsion fractures of the spinous processes of vertebrae (T3-T5). Such presentation is rare in an already rare scapulothoracic dissociation injury. A discussion regarding t

Basilar skull fracture in a Thoroughbred colt: radiography or computed tomography?

A two-year-old Thoroughbred colt was presented to the Equine Clinic, Onderstepoort Veterinary Academic Hospital for head trauma after rearing and falling backwards, hitting his head on the ground. Following medical therapy for acute onset neurological impairment secondary to a suspected basilar skull fracture, the horse was anaesthetised and computed tomography of the skull was performed. A diagnosis of a comminuted basilar skull fracture was made and skull radiographs were taken for comparison. The horse was subsequently euthanased owing to the poor prognosis; necropsy findings were compatibl

A new modification of corrective osteotomy for treatment of distal radius malunion.

A new modification of corrective osteotomy for distal radius malunion, which minimizes the need for bone grafting and addresses the constraining aspect of the extensor retinaculum, is described. This new modification uses a volar and dorsal approach to perform an angled step-cut osteotomy, release of the extensor retinaculum, and volar plating. The typically dorsally extruded comminuted fracture fragments from the distal, dorsal compression fracture component of the distal radius fracture is mobilized with the distal fragment and acts as a dorsal strut graft to span the opening wedge, correcti

Combined intramedullary Kirschner wire and intra-osseous wire loop for fixation of finger fractures.

An intramedullary Kirschner wire was combined with an intra-osseous wire loop for fixing finger fractures. The mechanical stability of this technique to resist a bending moment in the sagittal plane was compared with four other conventional methods: single loop, loop and an oblique Kirschner wire, crossed Kirschner wires and dorsal plating. It was found that when the applied force was small, the technique was much more stable than the crossed Kirschner wires and was similar to the others. The ability to withstand mechanical failure was much better than most of the methods except the dorsal pla

Risk factors for post-traumatic osteoarthritis following surgical treatment of acetabular posterior wall fractures: a retrospective study.

Posterior wall fractures, commonly caused by high-energy mechanisms like motor vehicle collisions or falls, are frequently associated with hip dislocation and complex intra-articular injuries. Despite surgical advances, post-traumatic osteoarthritis (PTOA) remains a prevalent complication, often leading to significant functional impairment and eventual joint replacement. Identifying potentially modifiable risk factors may help improve long-term outcomes. This study aimed to identify independent risk factors associated with the development of post-traumatic osteoarthritis (PTOA) following surgi

Immediate weight bearing as tolerated (WBAT) correlates with a decreased length of stay post intramedullary fixation for subtrochanteric fractures: a multicenter retrospective cohort study.

BACKGROUND: Subtrochanteric femur fractures associate with a relatively high complication rate and are traditionally treated operatively with a period of limited weight bearing. Transitioning from extramedullary to intramedullary implants, there are increasing biomechanical and clinical data to support early weight bearing. This multicenter retrospective study examines the effect of postoperative weight bearing as tolerated (WBAT) for subtrochanteric femur fractures. We hypothesize that WBAT will result in a decreased length of stay (LOS) without increasing the incidence of re-operation. METH

Biomechanical Assessment of the Dorsal Spanning Bridge Plate in Distal Radius Fracture Fixation: Implications for Immediate Weight-Bearing.

BACKGROUND: The goal of this study was to compare the biomechanical stability of a 2.4-mm dorsal spanning bridge plate with a volar locking plate (VLP) in a distal radius fracture model, during simulated crutch weight-bearing. METHODS: Five paired cadaveric forearms were tested. A 1-cm dorsal wedge osteotomy was created to simulate an unstable distal radius fracture with dorsal comminution. Fractures were fixed with a VLP or a dorsal bridge plate (DBP). Specimens were mounted to a crutch handle, and optical motion-tracking sensors were attached to the proximal and distal segments. Specimens w

Prognostic value of computed tomography classification systems for intra-articular calcaneus fractures.

BACKGROUND: There are several published computed tomography (CT) classification systems for calcaneus fractures, each validated by a different standard. The goal of this study was to measure which system would best predict clinical outcomes as measured by a widely used and validated musculoskeletal health status questionnaire. METHODS: Forty-nine patients with isolated intra-articular joint depression calcaneus fractures more than 2 years after treatment were identified. All had preoperative CT studies and were treated with open reduction and plate fixation using a lateral extensile approach.

Skin Tenting Due to Displaced Mid-shaft Clavicle Fracture in an Adolescent.

Sport-related clavicle fractures are a relatively common injury in the pediatric population. However, neurovascular complications at initial presentation are rare in adolescents. We report the case of a 14-year-old female skier who presented with a left shoulder injury and visible skin tenting. Plain radiographs demonstrated a comminuted and displaced mid-shaft clavicle fracture. Due to concern for impending necrosis and potential conversion to an open fracture, she was promptly referred to a nearby facility with surgical capabilities. She underwent surgical fixation shortly after arrival, wit
