[Surgery for hyperostotic cranial orbital meningiomas].

The paper analyzes surgical techniques for removal of hyperostotic cranial orbital meningiomas in 36 patients operated on in 1998 to 2000. In 19 cases hyperostosis extends to the upper and lower lid slits without involving the optic canal. It also spreads to the ethmoidal sinus in 6 cases to the frontal sinus in 3, and to the maxillary one in 2 patients. In 3 patients, hyperostosis was beyond the wing of the basic bone, by involving the temporal and frontal squamous. In 19 cases, hyperostosis was resected and the tumor was removed without creating any additional bone flaps. To make an addition

One-piece Craniofacial Bone Fracture Reduction With an Additional Single Osteotomy at the Zygomaticomaxillary Buttress: A Successful Reduction of Frontoorbitozygomaticomaxillary Fracture Associated With a Temporoparietal Bone Fracture.

A frontoorbitozygomaticomaxillary fracture, also known as a complex midface fracture or "quadripod fracture," is a type of facial fracture involving multiple midface region bones. This includes the frontal bone (forehead), orbitozygomatic complex (which includes the zygomatic bone or cheekbone and the orbital bones around the eye), and the maxillary bone (upper jaw). These complex midface fractures associated with temporoparietal bone fractures can occur in clinical practice. However, a single-piece fracture of these has not been reported before. This study reports a 44-year-old male patient w…

Percentile-based assessment of craniosynostosis.

Perioperative assessment of craniosynostosis is based mostly on subjective scores. In this study, we sought to find an objective method to assess cranial deformation based on normative craniofacial percentiles. Anthropometric datasets from 104 (79 males, 25 females) patients with craniosynostoses were included. Anthropometric data were compared with normative age-dependent percentiles. Deviations above the 90th or below the 10th percentile were defined as significant cranial deformation. The cohort comprised 69 children with sagittal, 22 metopic, nine coronal, two bicoronal, one lambdoid, and

Lengthening the nose: a lesson from craniofacial surgery applied to posttraumatic and congenital deformities.

Various methods of nasal lengthening presented in the literature are discussed, including extensive skin undermining, provision of extra skin by forehead flaps, bone grafts, cartilage grafts to the nasal tip, extensive dissection of nasal lining, and buccal flaps to provide additional nasal lining. This paper describes a method that entails a nasofrontal osteotomy and mobilization of previous fractures in posttraumatic foreshortened noses and a perinasal osteotomy (Le Fort II minus Le Fort I) in congenitally short noses such as seen in Binder's syndrome. Nine posttraumatic and two congenital s

[A new case of fronto-nasal dysplasia associated with craniosynostosis].

The Authors report a case of frontonasal dysplasia, a rare congenital syndrome, accompanied by multiple cranial synostosis. According to Cohen, frontonasal dysplasia associated with craniosynostosis may be considered as a separate syndrome and Cohen himself has described its occurrence in members of two families as "Craniofrontonasal Dysplasia". The Authors describe a sporadical case presenting various minor abnormality reported by Cohen, in addition to the hypoplasia of the corpus callosum, demonstrated by the Cranial Ultrasonography and by the NMR study of the brain.

Subtotal neonatal calvariectomy for severe craniosynostosis.

This report sets forth an example of a new mode of management of severe craniosynostosis in the neonate: subtotal calvariectomy. An infant with synostosis of sagittal, coronal, and lambdoidal sutures who had signs of increased intracranial pressure with impending neurologic complications was operated on at 13 days of age. Wide excision of the bony calvarium from the underlying dura was accomplished from the supraorbital ridge to near the foramen magnum; the entire area of the coronal sutures was included. Regeneration of the calvarium occurred over two to 12 weeks and was accompanied by the pr

The biomechanics of maxillofacial trauma.

Current advances in vehicle design include nonlacerative windshields and energy-absorbing instrument panels and interior structures. In order to prevent more severe injuries, it may be necessary to accept minor soft tissue trauma and nasal fractures, but it should be possible to prevent almost all other facial trauma at reasonable impact speeds.

Imaging Diagnosis: Atypical CT Appearance of a Multilobular Osteochondrosarcoma in a Dog.

A 9-year-old female spayed boxer presented for a dorsal cranial mass. Cytology diagnosed a sarcoma. Contrast-enhanced computed tomography (CT) of the head revealed a symmetrical, soft tissue attenuating mass with subtle mineralization and a contrast-enhancing capsule at the dorsal calvarium. There was adjacent frontal bone lysis and contrast enhancement of the parietal lobe. This supported a sarcoma of bone or cartilage origin. Postmortem examination additionally revealed a pulmonary tumor embolus. A final diagnosis of grade III multilobular osteochondrosarcoma (MLO) was made. This is the firs…

Intra-osseous meningioma of the orbit: an unusual presentation (2007: 5b).

Primary calvarial or intra-osseous meningiomas are uncommon lesions often confused preoperatively with other primary or secondary bone tumors of the skull. We present details of a patient with a primary calvarial meningioma involving the frontal, zygomatic, sphenoid and temporal bones that was radiographically diagnosed as fibrous dysplasia and initially treated conservatively for many years. Progressive vision loss and significant proptosis, with difficulty of eye closure, prompted surgical removal with cranioplasty and a 3D reconstructed prosthetic flap. Histological examination revealed int

Psammomatoid juvenile ossifying fibroma of frontal sinus - surgical and reconstructive approach.

Psammomatoid juvenile ossifying fibroma (PJOF) is a benign fibro-osseous lesion that mainly affects the paranasal sinuses and periorbital bones. It may cause significant esthetic and functional impairment. Herein, we describe the diagnosis and surgical approach of an extensive PJOF arising in the frontal sinus of a young male. After complete lesion removal and histopathological confirmation, the bone defect was repaired with a customized polymethylmethacrylate implant. PJOF may present aggressive clinical behavior. The excision of extensive PJOF in the orbitofrontal area can result in signific

Intraosseous hemangiomas.

Intraosseous hemangiomas are classified as benign tumors of vascular nature. Some authors describe them as hamartomas. They originate and expand inside bone structures. They are usually congenital, rarely of posttraumatic origin. In the Maxillo-Facial Surgery departments of the Universities of Rome "La Sapienza" and "Tor Vergata," from 1990 to 2004, 11 cases of intraosseous hemangioma have been diagnosed. In 6 cases, the neoplasm localized in the zygomatic region; in 3 cases, at the mandible level; in 1 patient, in the maxillary site; and in 1 patient, in the frontal bone. Literature review an…

[Chondromyxoid fibroma: radiologic and radioisotope aspects].

Chondromyxoid fibroma is a relatively rare benign bone tumor whose histologic and radiologic patterns are well known. In this article, we describe a 16-year-old boy with such a tumor in his left tibia. Scintigraphically, this tumor is represented by a "doughnut sign". This aspect, though not pathognomonic, has been reported sporadically in a few diseases: osteoporosis circumscripta cranii, angioblastic meningioma, cranial coccidioidomycosis, and aseptic necrosis of frontal bone. Because of their sites, it is easy to reject these diagnoses in our patient. The most pertinent differential diagnos…

Cerebral air embolism from oesophageal-pulmonary vein fistula induced by fish bone ingestion.

Fish bone ingestion is common, but cerebral air embolism as a secondary complication is exceedingly rare. A man in his 80s presented to our tertiary emergency centre after sudden collapse following oesophageal discomfort from fish bone ingestion the previous day. Head CT revealed punctate hypodense foci in the right frontal white matter, consistent with cerebral air, while CT perfusion showed decreased cerebral blood flow with delayed transit time. Chest CT demonstrated an impacted fish bone in the distal oesophagus and air bubbles in the pulmonary vein. Urgent endoscopy confirmed the fish bon

Development of the frontal sinus following bilateral fronto-orbital osteotomies.

The presence of frontal sinuses following bilateral fronto-orbital advancement is discussed controversially in the literature. In a retrospective study, 33 patients (18 male and 15 female) were operated on between 1982 and 1993, with at least one year postoperative follow-up and with a minimum age of 6 years at the end of the follow-up period following bilateral fronto-orbital remodelling, were included. The average age at which the procedure was performed was 29 months with a minimum of 3 months and a maximum of 7.8 years. The study presented shows a pneumatization of the frontal sinus in 72.

Computed Tomography, Magnetic Resonance, Cross-Sectional Anatomy, and 3D Volume Rendering of the Nasal Cavity and Paranasal Sinuses of the Alpaca (Vicugna pacos).

Understanding the anatomy of the paranasal sinuses and nasal cavity in alpacas (Vicugna pacos) is limited by scarce published data. This prospective cadaveric study aimed to enhance the interpretation of clinical cross-sectional imaging and expand knowledge of normal anatomy using computed tomography (CT), magnetic resonance imaging (MRI), macroscopic cross-sections, and 3D reconstructions (Amira v5.3.3; Thermo Fisher Scientific, Waltham, MA, USA). Seven alpacas (median age 3.9 years; five males, two females) underwent CT and MRI. Frozen anatomical slices were obtained from three heads, and si
