When Should ED Physicians Use an HIE? Predicting Presence of Patient Data in an HIE.

OBJECTIVES: Health information exchanges (HIEs) make possible the construction of databases to characterize patients as multisystem users (MSUs), those visiting emergency departments (EDs) of more than one hospital system within a region during a 1-year period. HIE data can inform an algorithm highlighting patients for whom information is more likely to be present in the HIE, leading to a higher yield HIE experience for ED clinicians and incentivizing their adoption of HIE. Our objective was to describe patient characteristics that determine which ED patients are likely to be MSUs and therefor

The electronic health record Risk of Alzheimer's and Dementia Assessment Rule (eRADAR) Brain Health Trial: Protocol for an embedded, pragmatic clinical trial of a low-cost dementia detection algorithm.

BACKGROUND: About half of people living with dementia have not received a diagnosis, delaying access to treatment, education, and support. We previously developed a tool, eRADAR, which uses information in the electronic health record (EHR) to identify patients who may have undiagnosed dementia. This paper provides the protocol for an embedded, pragmatic clinical trial (ePCT) implementing eRADAR in two healthcare systems to determine whether an intervention using eRADAR increases dementia diagnosis rates and to examine the benefits and harms experienced by patients and other stakeholders. Effects of sub-chronic oral exposure to pyrogenic synthetic amorphous silica (NM-203) in male and female Sprague-Dawley rats: focus on reproductive systems.

Synthetic amorphous silica (SAS) consists of agglomerates and aggregates of primary particles in the nanorange (<100 nm) and it is the E551 authorized food additive. The potential risks for human health associated to dietary exposure to SAS are not completely assessed; in particular, data on male and female reproductive systems are lacking. A 90-day oral toxicity study with pyrogenic SAS nanomaterial NM-203 was carried out on the basis of the OECD test guideline 408 in the frame of the NANoREG project. Adult Sprague-Dawley rats of both sexes were orally treated for 90 days with 0, 2, 5, 10, 20

Evaluating Patient Representation Strategies for Early Prediction of Alzheimer's Disease and Related Dementias from Longitudinal EHRs.

The neurodegenerative processes underlying Alzheimer's disease and related dementias (ADRD) begin years before clinical diagnosis, motivating scalable approaches for earlier identification. Electronic health records (EHRs) provide longitudinal clinical information for early ADRD prediction but are sparse, temporal, and hierarchical, and alternative EHR representations encode these characteristics differently. We systematically evaluated how EHR representation and temporal distance from diagnosis influence ADRD prediction and whether representation-specific performance generalizes across health

Using Human Factors and Systems Simulation to Optimize the Usability of a Code Documentation Tool.

OBJECTIVES: Implementing a new electronic health record (EHR) system in high-pressure, high-stakes environments, such as documenting during pediatric and neonatal cardiac and respiratory arrest scenarios, can be challenging for health care staff. Poorly designed tools or technology that don't align with existing workflows may lead to incomplete or inaccurate documentation. Lower adoption rates and continued reliance on paper forms for code documentation can result in gaps in patient data within the EHR, requiring the transcription of critical information after the event. METHODS: This project

Large language models for clinical decision support in gastroenterology and hepatology.

Clinical decision making in gastroenterology and hepatology has become increasingly complex and challenging for physicians. This growing complexity can be addressed by computational tools that support clinical decisions. Although numerous clinical decision support systems (CDSS) have emerged, they have faced difficulties with real-world performance and generalizability, resulting in limited clinical adoption. Generative artificial intelligence (AI), particularly large language models (LLMs), are introducing new possibilities for CDSS by offering more flexible and adaptable support that better

Global ageing: successes, challenges and opportunities.

The world's population is ageing rapidly, with significant increases in the numbers of the oldest old. This places great pressure on societies to adapt to this changing demography. Pertinent issues include provision of education and resource for long-term conditions. The priorities older people hold need to be fully understood and their contributions to society, often diverse and far-reaching, recognised with sincerity. Currently, health systems for older people can often feel reactive, fragmented and disjointed. These systems can harbour inequity and ageism, and leave both patients and health

Costs per discharge and hospital ownership under prospective payment and cost-based reimbursement systems in Taiwan.

This study in Taiwan examined the relationships between health care costs and hospital ownership under two financing systems with diametrically opposite incentives, case-payment (a form of prospective payment) and cost-based reimbursement. The universal sample of patients treated in 2000, for three standard care groups under each payment method, was included. The case payment diagnoses were uncomplicated cases of caesarean section, femoral/inguinal hernia operation and thyroidectomy, and the cost-based reimbursement diagnoses were uncomplicated cases of benign breast neoplasm, pneumococcal pne

Parenting and proximity to social services: Lessons from Los Angeles County in the community context of child neglect.

Using a sample of 438 parents in Los Angeles County, CA, this study examines the role of proximity to social services in child neglect. In an extension of social disorganization theory, it seeks to understand the potential sources of support in neighborhoods for families. It uses ordinary least squares regression to examine driving distance from parents' residences to four types of services (child care, domestic violence, mental health/substance abuse, and poverty). The results show an association between proximity to mental health and substance abuse services and parents' self-reported neglec
