Frequency, diagnosis and clinical characteristics of patients with multiple accessory atrioventricular pathways.

Multiple accessory atrioventricular (AV) pathways were documented in 52 of 388 patients (13%) who underwent detailed electrophysiologic evaluation. Multiple AV pathways were identified during intraoperative mapping or electrophysiologic study by different patterns of ventricular preexcitation during atrial fibrillation, flutter or atrial pacing with different delta-wave morphologic and ventricular activation patterns; different sites of atrial activation during right ventricular pacing or orthodromic reciprocating tachycardia; or preexcited reciprocating tachycardia using a second pathway as t

[Pre-excitation ventricular syndrome. Syndrome of the additional latent retrograde-conducting ventriculoatrial connection].

A new syndrome of ventricular pre-excitation syndrome is differentiated--of additional obscure retrograde conducting ventricular-atrial connection, different from the so far known syndrome of Wolff-Parkinson-White, Lown-Ganong-Levine and syndrome of Mahaim. That is determined by the anatomically confirmed additional connection, conducting only retrogradely from ventricles to auricle. Diagnostic criteria have been developed from the most significant clinical symptoms and electrocardiographic constellations. With the syndrome of additional obscure retrogradely conducting ventricular-atrial conne

Occult arrhythmias as the etiology of unexplained syncope in children with structurally normal hearts.

We evaluated six children for syncope of unknown etiology between March, 1983, and March, 1984. All had undergone previous neurologic evaluation which was normal. Cardiac examination, chest roentgenograms, and two-dimensional echocardiograms were also normal in all of the patients. Abnormal noninvasive findings in five patients included Mobitz type II atrioventricular (AV) block (one patient), sinus bradycardia (three patients), and supraventricular tachycardia (one patient). Four patients had one or more abnormal findings at invasive electrophysiologic study including evidence of sinus node d

[Electrophysiologic study of paroxysmal supraventricular tachycardia].

Seventy patients (pts) with paroxysmal supraventricular tachycardia (PSVT) were examined by using electrophysiological techniques (EPS). At rest, 23 pts had the ECG evidence of W-P-W syndrome (group I) and 47 pts did not have (group II). In 20 pts of group I, PSVT could be induced by EPS. In 19 pts of them, the mechanism of PSVT was reentry using the accessory pathway (AP) retrogradely and the normal pathway (NP) antegradely, but only one showed the antidromic type (the former in reverse) of PSVT. Whereas in 45 pts of group II, PSVT could be induced reproduceably by EPS. In 31 pts of them, the

[Paroxysmal AV tachycardia in triplets].

It is reported on triplet sisters with paroxysmal tachycardias. Two of them were enzygotic. For the two an AV-node longitudinal dissociation could be proved as cause of the tachycardia. This observation is regarded as proof for the existence of congenital anomalies of the AV-node as a possible cause of its longitudinal dissociation.

[Electrophysiologic demonstration of dual atrioventricular nodal pathways and final common pathway in a case of A-V nodal paroxysmal tachycardia. Considerations on the functional complexity of the A-V node (author's transl)].

The AA. studied the A-V nodal conduction using the technique of induced PAB in a patient with A-V reentrant paroxysmal tachycardia. They observed that the conduction through the A-V node failed when coupling intervals A1-A2 were between 280 and 260 msec and, after, recovered, with consistent slackening, when A1-A2 intervals were shortened, until the atrial ERP was reached. This uncommon response indicates the functional complexity of the A-V node and, particularly, suggests the presence of a final common pathway distal to the fast and slow A-V pathways, that are the anatomic-functional basis o

Demonstration of multiple antegrade and retrograde atrioventricular pathways.

A patient with paroxysmal supraventricular tachycardia had discontinuous antegrade (1a-A2, H1-H2) and retrograde (V1-V2, A1-A2) conduction curves suggesting dual A-V nodal pathways in both directions. Atrial echoes occurred with premature atrial pacing only at short A1-A2 coupling intervals after long antegrade (A2-H2) and retrograde (H2-A3) conduction intervals. Premature ventricular stimulation revealed ventricular echoes simultaneously with a sudden increase in the V2-A2 interval. The echo zone coincided with the slow pathway curve. Following atropine the echo zone was extended over the slo

["Slowing" right bundle-branch block in reciprocating tachycardia from a latent right lateral Kent bundle].

The authors report the case of a 56 year old man with paroxysmal reciprocating tachycardia. The participation of a right lateral Kent bundle, latent in sinus rhythm and with retrograde atrioventricular conduction during tachycardia was proved by : 1) the slowing of the tachycardia rhythm and lengthening of the ventriculo-atrial conduction time by 50 ms during right bundle branch block ; 2) atrial mapping during tachycardia showing right lateral atrial pre excitation ; 3) the spontaneous termination of some attacks after a blocked Hisian depolarisation. Analysis of the mechanisms of spontaneous

Triple AV nodal pathways in man?

A 41 year old woman underwent electrophysiological studies for the evaluation of paroxysmal supraventricular tachycardia. AV nodal conduction time and refractory period plots (atrial extra-stimulus technique) suggested the presence of triple AV nodal pathways.

Electrotonic interaction during canine ventricular repolarization.

Canine ventricular refractory periods were measured during test site drive, during drive of single ectopic sites, during fusion drive from two ectopic sites, and during fusion drive from an ectopic site and the test site. Refractory period duration was dependent on the driving modes employed. Refractory periods were 2.63 +/- 0.73%, 3.42 +/- 0.87%, 3.54 +/- 100%, and 4.68 +/- 1.36% (mean +/- SD) shorter during drive of single ectopic sites 2, 4, 6, and 40-60 mm, respectively, from the test site than during test site drive. During fusion drive from two ectopic sites, refractory periods were an a

[Heart involvement in Anderson-Fabry disease: Italian recommendations for diagnostic, follow-up and therapeutic management].

Anderson-Fabry disease is a rare X-linked lysosomal storage disorder caused by mutations of the GLA gene that encodes alpha-galactosidase A. It is characterized by a multisystemic involvement: the renal, neurological, heart, cochleovestibular and cutaneous systems are the most damaged. Morbidity and mortality of Anderson-Fabry disease depend on renal insufficiency, heart failure and nervous system involvement. Left ventricular hypertrophy is the most common cardiac manifestation followed by conduction system disease, valve dysfunction, and arrhythmias. Mild to moderate left ventricular hypertr

Case Report: Fabry disease presenting with electrocardiographic findings mimicking acute myocardial infarction: a diagnostic challenge.

UNLABELLED: Fabry disease (FD) is a rare X-linked lysosomal storage disorder characterized by heterogeneous clinical manifestations. Cardiac involvement arises from progressive glycosphingolipid accumulation within cardiomyocytes, vascular endothelium, and the conduction system, resulting in left ventricular hypertrophy, conduction abnormalities, and arrhythmias. A subset of patients exhibits electrocardiographic findings that mimic acute myocardial infarction (AMI), often leading to misdiagnosis as ischemic heart disease. CASE PRESENTATION: We describe a 62-year-old man presenting with recur

Non-Coding RNAs and Gut Microbiota in the Pathogenesis of Cardiac Arrhythmias: The Latest Update.

Non-coding RNAs (ncRNAs) are indispensable for adjusting gene expression and genetic programming throughout development and for health as well as cardiovascular diseases. Cardiac arrhythmia is a frequent cardiovascular disease that has a complex pathology. Recent studies have shown that ncRNAs are also associated with cardiac arrhythmias. Many non-coding RNAs and/or genomes have been reported as genetic background for cardiac arrhythmias. In general, arrhythmias may be affected by several functional and structural changes in the myocardium of the heart. Therefore, ncRNAs might be indispensable
