Impact of angina burden and other factors on treatment satisfaction after acute coronary syndromes.

PURPOSE: Although of great importance to clinicians, hospitals, and health care systems, little is known about factors that influence treatment satisfaction after acute coronary syndromes (ACS). The objective of this study was to identify potentially modifiable factors associated with treatment satisfaction 7 months after ACS. METHODS: The study population included 1957 patients with ACS who were enrolled in the multicenter, prospective Veterans' Health Administration Access to Cardiology Study. The primary outcome was treatment satisfaction 7 months after ACS as measured by the Seattle Angin

Infected calyceal diverticulum in pregnancy: A rare case.

A calyceal diverticulum is a transitional epithelium-lined outpouching of a renal calyx which communicates with the main collecting system through a narrow infundibulum. There are two types of calyceal diverticula: type I, the most common, communicates with the minor calyx, and type II communicates with the major calyx or renal pelvis. Calyceal diverticula are rare and mostly found incidentally; however, they can cause urinary tract infection symptoms (e.g., hematuria, pain, and fever). Diagnosing an infected calyceal diverticulum during pregnancy is particularly challenging due to overlapping

A qualitative study of factors resulting in care delays for adults with meningitis in Zambia.

BACKGROUND: Meningitis causes significant mortality in regions with high comorbid HIV and TB. Improved outcomes are hindered by limited understanding of factors that delay adequate care. METHODS: In-depth interviews of patients admitted to the University Teaching Hospital with suspected meningitis, their caregivers, doctors and nurses were conducted. Patient/caregiver interviews explored meningitis understanding, treatment prior to admission and experiences since admission. Provider interviews addressed current and prior experiences with meningitis patients and hospital barriers to care. A co

Application of a Systems Theory-Based Accident Analysis Technique to Perioperative Safety Reports From the COVID-19 Pandemic.

OBJECTIVES: Nonlinear retrospective analytic techniques can allow for in-depth understanding of accidents and their causes, yet they are infrequently used in health care. The purpose of this study was to provide an example, using Causal Analysis based on Systems Theory (CAST) together with an inductive thematic analysis to understand the contextual factors contributing to one hospital's perioperative safety events. METHODS: We created a hierarchical control structure of the hospital's perioperative system with input from a multidisciplinary group. We then analyzed safety events that were self

Risk of prevalence of latent tuberculosis infection in health care workers-an idiographic meta-analysis from a Chinese perspective.

BACKGROUND: China is one of the countries sharing the major burden of tuberculosis (TB) in the world. Health care workers (HCWs) are subject to a high risk of occupational latent tuberculosis infection (LTBI)-an asymptomatic state of TB disease. However, the heterogenic composition of healthcare professionals in terms of nature of their work leads to the inconsistency in predicting the prevalence of LTBI amongst them. Furthermore, the global statistics do not account for the analysis conducted within the Chinese population. Our study reflects a systemic and epidemiological meta-analysis to inv

Association of Hospital Telestroke Adoption With Changes in Initial Hospital Presentation and Transfers Among Patients With Stroke and Transient Ischemic Attacks.

IMPORTANCE: It has been proposed that the implementation of telestroke services (a web-based approach to using video telecommunication to treat patients with stroke before hospital admission) changes where patients with stroke symptoms receive care, but this proposal has not been rigorously assessed. OBJECTIVE: To assess whether the implementation of telestroke services is associated with changes in where and how patients initially present with stroke symptoms, in their decision to be transferred to another hospital, and which hospitals they are transferred to. Assessment of the use of electronic medical records system and barriers in Rwanda.

BACKGROUND: Currently the use of electronic medical records (EMR) systems emerged globally especially OpenMRS to significantly improve healthcare delivery. However, challenges persist in achieving full EMR adoption, particularly in developing countries. In Rwanda, OpenMRS system was introduced in 2013 as a strategy to align digital health to the country’s information and communication technologies (ICT)-for-development agenda. Reports from healthcare providers indicate varying EMR adoption levels in health facilities in Rwanda. This study aims to assess the current usage of EMR and identify im

Healthcare Continuity in Crisis: Addressing Gaps in Longitudinal Care Through Inferential Statistics.

Continuity of care is vital to improving outcomes for an aging US population increasingly burdened by chronic conditions. However, systemic fragmentation remains pervasive, exacerbated by disjointed care transitions, misaligned incentives, and inadequate communication across providers and care settings. This study investigates the role of transitional care management (TCM) services, Medicare-reimbursed follow-up visits after hospital discharge, as a potential proxy for continuity of care. Using 2022 publicly available Centers for Medicare and Medicaid Services (CMS) data, the study evaluates w

[Expectations and service performance of social psychiatry services in free Saxony. An analysis of viewpoints of psychiatric practice and clinical specialists and coworkers of social psychiatric services].

After the "Polyclinic system" that had predominated in the GDR had been dismantled, a far-reaching restructuring of the complementary psychiatric care sector became necessary. In the State of Saxony, comprehensive establishment of a homogeneous home-visit outpatient service is the first building block in an interlinked system of regionalized community psychiatry. Based on about 20 years of experience in the Federal States of the FRG, this function is fulfilled by social psychiatry services (SPS). The present study investigates the expectations of free-practising psychiatrists (n = 165), doctor

[Acute infections, venous thrombosis, and recommended thromboprophylaxis].

One of the most serious complications of acute infection is venous thromboembolism (VTE). The mechanism of host procoagulant responses in bacterial and viral infections is complex, and various factors are known to trigger the proinflammatory responses and activate systemic coagulation. Host defense mechanisms include proinflammatory cytokines such as interleukins, tumor necrosis factor-α, and complement system proteins, all of which can induce coagulopathy. Tissue factor (TF) expression on monocytes/macrophages, neutrophil and platelet activation, produce activation of thrombosis. This th

Efficacy of Facilitated Capacity Building in Providing Cleft Lip and Palate Care in Low- and Middle-Income Countries.

Providing surgical repair for congenital anomalies such as cleft lip and palate (CLP) can be challenging in low- and middle-income countries. One nonprofit organization seeks to address this need through a partnership model. This model provides long-term aid on multiple levels: surgeon and healthcare provider education, community outreach, and funding. The authors examined the effectiveness of this partnership model in providing CLP care and increasing cleft care capacity over time. This organization maintains data on each partner and procedure and collected data on hospital and patient charac
