Screening of 1-Month-Old Infants With Prolonged QT Interval and Its Cutoff Value.

BACKGROUND: The prevalence of congenital long QT syndrome (LQTS) (1 : 2,000) is based on genetic testing and ECG data, but the prevalence of electrocardiographically determined prolonged corrected QT interval (pQTc) in infants is unclear. METHODS AND RESULTS: Subjects were 10,282 1-month-old infants who participated in 2 prospective ECG screening studies performed in 2010-2011 and 2014-2016. Infants with a QTc ≥0.45 using Bazett's formula [QTc(B)] at 1-month medical checks were re-examined. pQTc was defined as QTc ≥0.46 on 2 different ECGs in early infancy. Infants with QTc ≥0.50 or progressi

Preterm Infants May Better Tolerate Feeds at Temperatures Closer to Freshly Expressed Breast Milk: A Randomized Controlled Trial.

PURPOSE: Feeding intolerance is one of the most frequent problems among preterm infants. These infants are fed with expressed breast milk or preterm formulas of which the temperature is not routinely measured. In this study, we aimed to examine the effects of feeds with warm milk versus room temperature milk in preterm infants. MATERIALS AND METHODS: Infants with a birth weight ≤1,500 g or gestational age ≤34 weeks were included in the study and assigned to two different feeding temperature groups (22-24°C and 32-34°C). Some infants in both groups were exclusively breast milk-fed, and some re

Response of protracted neonatal hypocalcaemia to 1alpha,25-dihydroxyvitamin D3.

Six infants with protracted hypocalcaemia (5 classed as tetany of the newborn) had a mean plasma-calcium of less than 6-4 mg/dl and raised plasma-inorganic-phosphate. Four had been hypocalcaemic for longer than 2 weeks despite treatment with low phosphate formulae and extra calcium. All the infants were treated with 1alpha,25-dihydroxyvitamin D3 (1alpha,25-(OH)2D3), usually in a dose of 0-05 mug/kg body-weight/day, administered intravenously for 5--12 days. The plasma-calcium levels began to rise within 24 h after starting 1alpha,25-(OH)2D3 treatment, increased above 7-5 mg/dl within 3 days, a

Facial skeleton dysmorphology in syndromic craniosynostosis: differences between FGFR2 and no-FGFR2-related syndromes and relationship with skull base and facial sutural patterns.

PURPOSE: To assess the role of FGFR2 mutations and sutural synostotic patterns on facial skeleton dysmorphology in children with syndromic craniosynostosis. METHODS: Preoperative high-resolution CT images in 39 infants with syndromic craniosynostosis were evaluated. Patients were divided into infants with and without FGFR2 mutations; each group was split according to synostotic involvement of minor sutures/synchondroses: isolated or combined involvement of middle (MCF) and posterior cranial fossae (PCF). Quantitative analysis of the midface and mandible measures was performed. Each subgroup w

Congenital chylothorax: associations and neonatal outcomes.

AIM: Congenital chylothorax is a rare but significant neonatal entity with major morbidity and mortality. The study aims to describe the related associations, management and outcomes of this condition in neonates. METHODS: This is a retrospective case series of all cases of congenital chylothorax admitted to a tertiary neonatal centre in the last 15 years. RESULTS: Ten cases of congenital chylothorax were identified. Eight infants were diagnosed antenatally and three infants had antenatal pleural drainage. Most infants were ventilated at birth and required immediate pleurocentesis. Post-nata

Breastfeeding infants who were extremely low birth weight.

OBJECTIVE: To compare the clinical effect of breastfeedings and bottle feedings in extremely low birth weight (ELBW) infants (birth weight </=800 g). METHODS: A total of 12 ELBW infants (mean birth weight, 672 +/- 95 g; mean gestation, 26 +/- 2 weeks) served as their own controls in this prospective study comparing physiologic parameters during bottle and breastfeeding. The infants were put to breast the same week they began on bottle feedings of human milk or premature infant formula (mean gestation, 35 weeks). One breastfeeding and one bottle feeding were observed each day for 10 days. Pre-…

Real-world data-enabled, 2-randomisation, adaptive trial to determine the clinical efficacy and safety of widely used enteral feeds in preterm babies: the COLLABORATE trial - protocol.

INTRODUCTION: Enteral feed type is widely believed to affect brain growth and development directly as well as influencing the risk of necrotising enterocolitis (NEC), a leading cause of death and neuro-impairment in extremely preterm babies. Own mother's milk (OMM) enhances neurodevelopment and reduces NEC. However, most babies born below 29 weeks gestation require some supplementary feeds during their neonatal unit stay because their mothers are unable to express sufficient milk. The available options, pasteurised human donor milk (pHDM) and preterm formula (PTF), differ markedly in nutrient

Infant hyperglycemic excursions by saliva 1,5-anhydroglucitol varied by weight status, rapid weight gain, and mode of feeding: an exploratory secondary analysis.

BACKGROUND: Early infant feeding and weight gain can impact the development of chronic diseases. 1,5 anhydroglucitol (1,5-AG) is a measure of hyperglycemic excursions (HEs) and decreases during HEs due to increased urinary excretion. OBJECTIVES: We hypothesized that HEs as measured by 1,5-AG are associated with infant rapid weight gain (RWG). METHODS: Infants from the Maternal and Infant Nutrition Trial, a randomized controlled feeding trial, participated in this exploratory secondary analysis. Saliva from 127 infants aged 5 mo was collected before the introduction of complementary foods. Sa

Randomized, controlled trial of slow versus rapid feeding volume advancement in preterm infants.

OBJECTIVES: To determine whether infants who are fed initially and advanced at 30 mL/kg per day (intervention) take fewer days to get to full feedings than those who are fed initially and advanced at 20 mL/kg per day (control), without increasing their incidence of feeding complications and necrotizing enterocolitis (NEC). We also examined whether these infants regain birth weight earlier, have fewer days of intravenous fluids, and a have shorter hospital stay. METHODS: A randomized, controlled, single-center trial was conducted in a Neonatal Intensive Care Unit of a community-based county ho

Observable essential fatty acid deficiency markers and autism spectrum disorder.

Autism Spectrum Disorder (ASD) has been associated with essential fatty acid (EFA) deficiencies, with some researchers theorising that dysregulation of phospholipid metabolism may form part of the biological basis for ASD. This pilot study compared observable signs of fatty acid status of 19 children with an ASD diagnosis to 23 of their typically developing siblings. A pregnancy, birth and breastfeeding history was also obtained from their parents, which included a measure of infant intake of fatty acid rich colostrum immediately post-partum. When considered within their family group, those in

Birth weight, slowness in eating and feeding practices as independent determinants of rapid weight gain.

AIM: Our aim was to identify independent determinants of rapid weight gain in infants at 3-4, 6, and 12 months of age. METHODS: A cohort study was conducted on Mexican term infants in public and private settings between March 2021 and May 2023. Rapid weight gain was defined as a ≥0.67 SD change in weight-for-age-Z-score from birth to 3-4, 6, and 12 months of age. Maternal and infant characteristics were described, and infant feeding practices, appetitive traits, weight, and length were analysed at 3-4, 6, and 12 months of age. Rapid weight gain predictors were determined using generalised lin

Surfactant therapy for maternal blood aspiration: an unusual cause of neonatal respiratory distress syndrome.

Surfactant replacement therapy is the main treatment of neonatal respiratory distress syndrome. However, surfactant therapy has been shown to be effective in the treatment of other diseases causing neonatal respiratory diseases such as pulmonary hemorrhage, meconium aspiration syndrome, pneumonia/sepsis, pulmonary edema or acute lung injury resulting a secondary surfactant deficiency (SSD). Rarely, as like as in the present patient, exogenous blood aspiration such as breast milk or formula aspiration may lead to SSD. Blood in alveolus leads to a significant biochemical and functional disturban

A case of protein-losing enteropathy caused by intestinal lymphangiectasia in a preterm infant.

Intestinal lymphangiectasia is characterized by obstruction of lymph drainage from the small intestine and lacteal dilation that distorts the villus architecture. Lymphatic vessel obstruction and elevated intestinal lymphatic pressure in turn cause lymphatic leakage into the intestinal lumen, thus resulting in malabsorption and protein-losing enteropathy. Intestinal lymphangiectasia can be congenital or secondary to a disease that blocks intestinal lymph drainage. We describe the first case of intestinal lymphangiectasia in a premature infant. The infant presented with peripheral edema and low

The viral dsRNA analogue poly (I:C) induces necrotizing enterocolitis in neonatal mice.

BACKGROUND: Necrotizing enterocolitis (NEC) is a life-threatening gastrointestinal disease in premature infants with high mortality and morbidity with uncertain pathogenesis. Recent research focused on the role of intraluminal bacteria and lipopolysaccharide (LPS). However, an additional role of viral agents in the pathogenesis of NEC has recently been postulated. We assessed the role of polyinosinic:polycytidylic acid (pIC) mimicking viral dsRNA in contributing to the development of NEC in neonatal mice. METHODS: Four-d-old C57BL/6J pups were stressed by asphyxia and hypothermia twice daily.

Safety of Breastfeeding During Favipiravir Therapy: A Case Study.

INTRODUCTION: Favipiravir is a known anti-influenza agent that has been used globally, especially during the coronavirus disease 2019 (COVID-19) pandemic. This drug has been important for treating influenza and other emerging infectious diseases, although it was ultimately proven to be ineffective for COVID-19. Currently, no studies have reported the human-milk transfer of favipiravir at doses of 1,600-3,600 mg/day in the treatment of diseases, including COVID-19. MAIN ISSUE: A 38-year-old mother gave birth at 38 gestational weeks because of severe respiratory distress caused by COVID-19. Con
