Management of refractory cardiogenic shock.

Cardiogenic shock is a life-threatening condition that occurs in response to reduced cardiac output in the presence of adequate intravascular volume and results in tissue hypoxia. Cardiogenic shock has several underlying aetiologies, with the most common being acute myocardial infarction (AMI). Refractory cardiogenic shock presents as persistent tissue hypoperfusion despite administration of adequate doses of two vasoactive medications and treatment of the underlying aetiology. Investigators of the SHOCK trial reported a long-term mortality benefit of emergency revascularization for shock comp

Bedside chest radiography.

The bedside chest x-ray (CXR) is an indispensible diagnostic tool for monitoring seriously ill patients in the intensive care unit. The CXR often reveals abnormalities that may not be detected clinically. In addition, bedside CXRs are an irreplaceable tool with which to detect the malposition of tubes and lines and to identify associated complications. Although the image quality is often limited, bedside CXRs still provide valuable diagnostic information. The interpretation of the bedside CXRs is often challenging, and requires extensive radiologic experience to avoid misinterpretation of the

Expanded indications for ultrasound-guided thrombin injection of pseudoaneurysms.

PURPOSE: We previously reported preliminary data on a new procedure that we developed for the treatment of femoral pseudoaneurysms after catheterization. This study presents our current results of percutaneous ultrasound-guided thrombin injection for treating pseudoaneurysms that arise from various locations and causes. METHODS: Between February 1996 and May 1999, we performed thrombin injection of 83 pseudoaneurysms in 82 patients. There were 74 femoral pseudoaneurysms: 60 from cardiac catheterization (36 interventional), seven from peripheral arteriography (four interventional), five from i

Intra-aortic balloon counterpulsation in blunt cardiac injury.

Intra-aortic balloon counterpulsation (IABC) is a widely used form of mechanical circulatory assistance. We have successfully employed IABC in three patients with refractory cardiogenic shock secondary to cardiac contusion. All patients had multiple blunt injuries with cardiac contusion documented electrocardiographically, by CPK isoenzyme determinations, and clinically. IABC was instituted for hypotension refractory to vasoactive drugs and optimization of intravascular volume status. Cardiac output and blood pressure rose in all cases and fewer ventricular arrhythmias were noted. Discontinuat

Intra-aortic balloon counterpulsation and delayed revascularization for myocardial infarction and shock in the absence of primary angioplasty: a treatment strategy with or without thrombolysis?

OBJECTIVE: When revascularization facilities are not available, thrombolytic therapy (TT) added to intra-aortic balloon counterpulsation (IABC) has been proposed as initial therapy for the management of patients presenting with postmyocardial infarction (MI) cardiogenic shock, followed by prompt transfer to another institution for revascularization. The use of TT in this setting, however, remains controversial. METHODS: We reviewed the records of 81 consecutive patients admitted with cardiogenic shock after acute MI and compared the outcomes of patients initially stabilized, including IABC as

Percutaneous extracorporeal ventricular assist device as a successful bridge strategy to durable left ventricular assist device in refractory cardiogenic shock: a case report.

INTRODUCTION: Cardiogenic shock (CS) is a life-threatening condition with high mortality. Temporary mechanical circulatory support (tMCS) is important for stabilizing patients with refractory CS, acting as a bridge to recovery, decision-making for durable left ventricular assist device (LVAD) implantation or transplantation. In China, commonly used devices like intra-aortic balloon pumps (IABP) and extracorporeal membrane oxygenation (ECMO) have limitations such as inadequate ventricular unloading and high complication rates, and advanced options like Impella, TandemHeart and CentriMag are not

Early Clinical Outcomes of Patients With Stress-Induced Cardiomyopathy Receiving Acute Mechanical Support in the US.

BACKGROUND: The role of acute mechanical circulatory support (aMCS) in patients with stress-induced cardiomyopathy (SIC) complicated by cardiogenic shock (CS) is not well studied. Here, we describe the incidence and outcomes of aMCS use in SIC-CS using a large national database. METHODS: Using the Nationwide Readmissions Database from January 2016 to November 2019, we identified patients hospitalized with SIC who received isolated intra-aortic balloon pump (IABP), microaxial flow pump (Impella, Abiomed), or extracorporeal membrane oxygenation (ECMO) during the index hospitalization. RESULTS:

The COVID-19 Pandemic and Coronary Angiography for ST-Elevation Myocardial Infarction, Use of Mechanical Support, and Mechanical Complications in Canada: A Canadian Association of Interventional Cardiology National Survey.

BACKGROUND: As a result of the COVID-19 pandemic first wave, reductions in ST-elevation myocardial infarction (STEMI) invasive care, ranging from 23% to 76%, have been reported from various countries. Whether this change had any impact on coronary angiography (CA) volume or on mechanical support device use for STEMI and post-STEMI mechanical complications in Canada is unknown. METHODS: We administered a Canada-wide survey to all cardiac catheterization laboratory directors, seeking the volume of CA use for STEMI performed during the period from March 1 2020 to May 31, 2020 (pandemic period),

Rapid Progression of Cardiac Allograft Vasculopathy With Sudden Cardiac Death Within the First Year of Heart Transplantation: A Case Report.

Cardiac allograft vasculopathy (CAV) is a major cause of mortality and morbidity within the first 3 years of heart transplantation (HT). CAV results in progressive coronary artery stenosis and may lead to arrhythmias and sudden cardiac death. In some cases, sudden cardiac death represents the initial presentation of CAV, as cardiac denervation blunts anginal symptoms that otherwise could have preceded sudden cardiac death (SCD). Here, we describe the case of a 56-year-old man with non-ischemic cardiomyopathy. He underwent left ventricular assist device implantation followed by orthotopic HT. F

Eosinophilic myocarditis in dilated cardiomyopathy: a case report.

BACKGROUND: Eosinophilic myocarditis (EM) is a rare cardiac condition that is often difficult to diagnose. Although endocardial myocardial biopsy is considered the gold standard for diagnosis, sampling errors can lead to false-negative results. This case report discusses the diagnosis and treatment of dilated cardiomyopathy in a patient with EM. CASE PRESENTATION: In this article, we report a case of a 32-year-old female patient diagnosed with dilated cardiomyopathy. EM was strongly suspected based on a progressive increase in eosinophil count, the absence of known allergens or common etiolog

Cardiogenic Shock Management in the Modern Era: A Narrative Review of Percutaneous Mechanical Circulatory Support Devices.

Cardiogenic shock (CS) remains a significant clinical challenge with persistently high mortality rates. Defined by impaired cardiac output resulting in end-organ hypoperfusion, CS commonly arises from acute myocardial infarction (AMI-CS) or acute exacerbations of heart failure (HF-CS). The severity of CS is classified by the Society for Cardiovascular Angiography and Interventions (SCAI) into stages A (at risk) through E (extremis), which informs treatment strategies, including pharmacotherapy and mechanical circulatory support (MCS). Recent advancements in percutaneous mechanical circulatory
