BACKGROUND: A 74-year-old Caucasian female presented with jaundice, clay-colored stools, diarrhea, and fatigue of 3 months' duration, accompanied by a weight loss of 6.8 kg. Using 13 patients undergoing operation of the bile duct, cefmenoxime (CMX) transport into the bile and the gallbladder tissue was investigated. One gram of CMX was administered through drip infusion over 30 minutes every 12 hours. INTRODUCTION AND IMPORTANCE: Obstructive jaundice following iatrogenic biliary injury is a rare but serious complication, often resulting from procedures such as laparoscopic cholecystectomy. It poses significant diagnostic and therapeutic challenges due to its complexity and potential for life-altering sequelae. Obstructive jaundice due to hilar cholangiocarcinoma is difficult to decompress because of the location of the tumor. We used external radiation alone for biliary decompression and reviewed its efficacy in this study. Subjects comprised 14 patients diagnosed as having inoperable hilar cholangiocarcinoma by ultrasonography, percutaneous transhepatic cholangiography, and CT scanning. In the cases of protracted obstructive jaundice, it is reported that there was a positive correlation between the hepatic copper concentration and the severity of destruction of the bile ducts. The author produced the model of obstructive jaundice in rats by common bile duct ligation and investigated whether additional copper overload can cause injury on the liver. The simple ligating procedure induced a significantly increased level of the copper concentration (p less than 0.001).
