Thirty-five consecutive patients treated with the radial forearm flap were reviewed. This flap was used in head and neck reconstruction in 25 patients, soft-tissue cover of an extremity in 9 patients, and as a new technique for penile reconstruction in 1 patient. Osteocutaneous flaps were used for mandibular reconstruction in 13 patients. In 6 patients innervated flaps were used to provide sensation on the dorsum of the hand or on the weight-bearing surface of the foot. There was only one total flap failure and no partial failures. Recipient-site complications were few, with prompt healing and…

Bone defects in the maxillofacial region severely impair patient function and esthetics. Free autologous bone grafting remains the gold-standard treatment; however, surgical intervention at donor sites limits clinical applicability. Treatment using artificial materials also presents challenges, including insufficient bone regeneration and poor biocompatibility. Bio three-dimensional (3D) printing, which enables the fabrication of scaffold-free 3D constructs from cellular spheroids has emerged as a promising regenerative approach. This study investigated the osteogenic potential of scaffold-fre

Temporomandibular joint (TMJ) ankylosis is a refractory disease that is difficult to predictably treat. This study evaluated the prognosis of using standard alloplastic TMJ prostheses for the treatment of TMJ ankylosis in Chinese patients with severe mandibular deficiency. Patients treated from 2013 to 2015 were reviewed. The computer-aided design and manufacture (CAD/CAM) technique was used to guide bony mass removal and locate the TMJ prosthesis (Biomet, USA). Eleven patients were included in this study. All prostheses were successfully installed and stabilized intraoperatively. In 4 patient

BACKGROUND: The choice of tissue type for free flap reconstruction of posterolateral mandible resections is dependent on patient and defect characteristics. We compared clinical and patient-reported outcomes following reconstruction of these defects with a soft tissue or bony free flap.
METHODS: A retrospective review was performed on patients who underwent posterolateral segmental mandibulectomy with immediate free flap reconstruction at MSKCC from 2006 to 2021. Outcomes of interest were patient-reported outcome measures (PROMs) assessed by FACE-Q surveys and complications at the flap recipi

Additively manufactured (3D-printed) surgical reconstruction implants are increasingly encountered in head and neck oncology and may introduce density assignment challenges during radiotherapy treatment planning. This case report describes a clinically significant dosimetric discrepancy arising from the application of nominal bulk material density to an additively manufactured surgical reconstruction implant and to highlight workflow considerations for density assignment. A patient treated with helical tomotherapy following mandibular reconstruction with an additively manufactured titanium imp…

AIMS: Implant-supported rehabilitation following maxillary or mandibular reconstruction with vascularized osseous free flaps is frequently compromised by inadequate peri‑implant soft tissues, vestibular obliteration, and scar contracture. This study describes a fully digital workflow for peri‑implant soft tissue management based on a patient-specific CAD/CAM vestibuloplasty stent (PSVS) and reports its preliminary clinical use.
MATERIALS AND METHODS: Five consecutive patients who had previously undergone jaw reconstruction with vascularized osseous free flaps underwent subperiosteal vestibulo…

Macrostomia is arare congenital craniofacial deformity that influences the appearance and function of patients. In most cases, it coexists with craniomaxillofacial deformities such as craniofacial microsomia (CFM). This study aimed to analyze the relationship between macrostomia and mandibular hypoplasia so as to facilitate the early detection and diagnosis of children with CFM. It included 236 patients diagnosed with CFM. All underwent facial expression analysis, multi-angle photography, computed tomography, and three-dimensional reconstruction of soft and hard tissues. The clinical classific

BACKGROUND: Planning of surgical procedures in patients suffering from oropharyngeal cancer requires appropriate imaging, particularly in consideration of the spatial relationship to the mandible. Resection of portions of the mandible (box, marginal, or segmental resection) is often necessary, while simultaneously avoiding overtreatment. Typically, a computed tomography (CT) scan is initially performed. However, the question arises of whether CT alone is adequate for reliable assessment of mandibular involvement.
MATERIALS AND METHODS: A patient cohort of 25 individuals aged 44-79 years (mean

BACKGROUND: Temporo-Mandibular Joint (TMJ) replacement has been used clinically for years. The objective of this study was to evaluate outco … mplications. As such, TMJR represents a viable technique and a stable long-term solution for cranio-mandibular reconstruction in patients with irreversible end-stage TMJ disease. Comparing stock and custom-made groups, no statistically significant differences were detected with respect to pain intensity reduction and maximum mouth opening improvement.

Rehabilitation of an edentulous posterior mandible to restore function and arch stability can be accomplished with a removable partial denture or an implant supported fixed partial denture. If the alveolus is severely resorbed, implant placement becomes challenging due to inadequate bone and the position of the inferior alveolar nerve. This report details a situation where a mandibular fracture occurred soon after inferior alveolar nerve (IAN) transposition and simultaneous implant placement. The prosthodontic reconstruction was completed using a fixed-dental prosthesis.

OBJECTIVE: Mandibular reconstruction using patient-specific cage implants is a promising alternative to the vascularized free flap reconstruction for nonirradiated patients with adequate soft tissues, or for patients whose clinical condition is not conducive to microsurgical reconstruction. This study aimed to assess the biomechanical performance of 3D printed patient-specific cage implants designed with a semi-automated workflow in a combined cadaveric and retrospective case series study.
METHODS: We designed cage implants for two human cadaveric mandibles using our previously developed desi…

INTRODUCTION: Deepbites can be corrected by intrusion of mandibular anterior teeth. Direct anchorage with miniscrews simplifies complex tooth movements; however, few studies have reported their use for mandibular anterior intrusion. The purpose of this study was to evaluate, by means of the finite element method, initial tooth displacement and periodontal stress distribution using various mandibular anterior intrusion mechanics. Miniscrews were used as skeletal anchorage devices.
METHODS: Cone-beam computed tomography scans were used for 3-dimensional reconstruction of the mandible and the ma
