Trigeminal neuralgia and multiple sclerosis. A complex diagnosis.

Trigeminal neuralgia is a well-recognized complication in patients with multiple sclerosis. A case report is presented that describes multiple sclerosis in a middle-aged man with otherwise classical unilateral trigeminal neuralgia who demonstrated a variable response to pharmacologic and surgical intervention. The case highlights the difficulties of diagnosis when trigeminal neuralgia occurs concurrently with multiple sclerosis.

Multiple sclerosis in children: report of two cases.

Multiple sclerosis is a disease of white matter in the central nervous system characterized by clinically relapsing and remitting course. It is rare in children and the mean age of onset is about 30 years. To establish diagnosis, one must note that white matter lesions are disseminated in time and place. We report two female patients with recurrent episodes of headaches, seizures, hemiparesis, and optic neuritis since childhood. Four of the five CT scans done during the relapse attacks in these two patients showed negative findings. The only one abnormal finding was decreased density in the wh…

Defecography in multiple sclerosis patients with severe constipation.

PURPOSE: To evaluate defecography in assessment of anorectal function in patients with multiple sclerosis (MS) who have intractable constipation. MATERIALS AND METHODS: Eleven patients with MS and constipation (10 women, one man) underwent defecography. A total of 130 mL of barium, liquid (20 mL) and paste, was introduced into the rectum. A dab of barium marked the external anal orifice, and, in the women, a tampon soaked with contrast medium marked the vagina. Video radiographic images and supplemental 100-mm static camera images were obtained. RESULTS: During defecation, six patients had n…

[A case of multiple sclerosis associated with granulomatous panuveitis].

A case of multiple sclerosis (MS) associated with granulomatous panuveitis was reported. A 45-year-old woman developed diplopia in 1972, at age 24, optic neuritis in 1974 and acute transverse myelopathy in 1981. Subsequently, while being under our care with the clinical diagnosis of MS, right abducens palsy in 1989, deterioration of paraparesis with remission in 1991, and weakness of left arm in 1992 occurred. CSF study revealed high IgG index and brain MRI showed multiple abnormal intensity areas in the deep white matter and periventricular areas bilaterally. On the beginning in March, 1993, …

The role of trauma in the pathogenesis of multiple sclerosis: a review.

The suggestion that an alteration of the blood-brain barrier (BBB) is an obligatory step in the pathogenesis of the multiple sclerosis (MS) lesion has been amply confirmed by innumerable magnetic resonance scans. There also exists a large body of clinical, neuropathologic, neuropsychologic, radiologic and experimental evidence that shows that trauma, in particular mild concussive injury to the head, neck or upper back, thus impinging on the brain and spinal cord, may result in an increase in BBB permeability. It is only logical therefore to infer that when such mild trauma to those parts of th…

Multiple sclerosis lesions of the auditory pons are not silent.

To understand the relationship between brainstem lesions and auditory neurology in patients with multiple sclerosis, we compared behavioural, electrophysiological and imaging data in 38 patients with probable or definite multiple sclerosis and normal or near normal hearing. Behavioural measures included (i) general hearing tests (audiogram, speech discrimination) and (ii) hearing tests likely to be critically dependent upon brainstem processing (masking level difference, interaural time and level discrimination). Brainstem auditory evoked potentials provided the electrophysiological data. Mult…

A patient with primary biliary cirrhosis and multiple sclerosis.

A 48-year-old woman with primary biliary cirrhosis (PBC) and multiple sclerosis (MS) is reported. She presented with a visual deficit in 1975. In the course of a diagnostic evaluation, she was found to have PBC. In 1986, the diagnosis of MS was made. The patient's liver function deteriorated and liver transplantation was performed in May 1988. This is the third known case of this association.

AIDS and multiple sclerosis: neural and mental features.

The presence of mental disorder and cognitive functioning were examined in groups of 20 multiple sclerosis (MS) and homosexual acquired immunodeficiency syndrome (AIDS) ambulatory male outpatients matched for disability and demographic features. Patients who were somatically ill, had past central nervous system infection or tumours or abused intravenous drugs or alcohol were excluded. The groups significantly differed in mental symptoms and mental disorders (DSM-III classification) seen currently and after the diagnosis of MS or human immunodeficiency virus-1 infection. AIDS patients had pre-e…

The pineal gland and the clinical course of multiple sclerosis.

Clinical, epidemiological, biochemical, immunological, and radiological studies suggest that the pineal gland may be implicated in the pathophysiology of multiple sclerosis (MS). The following communication is concerned with the association among MS, pregnancy, the postpartum period, and melatonin secretion and illustrates, based on a clinical case report, the influence of the pineal gland on the clinical course of MS. This association is noteworthy since MS may worsen during the postpartum period and melatonin secretion is reported to be altered most dramatically by pregnancy and delivery. Si…

Primary Sjögren's syndrome in patients with multiple sclerosis.

The relationship between multiple sclerosis (MS) and primary Sjögren's syndrome (PSS) is ambiguous; it was suggested that some patients diagnosed with MS may instead have PSS. In a recent epidemiologic study, the prevalence of PSS was 2.7% in southern Sweden. We randomly selected 30 patients with definite MS from our patient population and investigated them for evidence of PSS according to the Copenhagen criteria. One patient had clinical symptoms compatible with PSS and also fulfilled the Copenhagen criteria. In addition, five patients had keratoconjunctivitis sicca, one had xerostomia, and t…

[Diagnosis and etiopathogenesis of retinitis in patients with multiple sclerosis].

Presented are: the incidence, symptomatology and diagnostics together with electroretinography of retinal periphlebitis as well as retinitis and maculopathy in patients with multiple sclerosis. Particular attention is called to the histopathology of the retina in this disease. The etiopathology of retinitis and maculopathy in multiple sclerosis is still unknown. Four etiopathological hypotheses are shortly discussed.

Localization of visual field defects in multiple sclerosis.

Demyelinating plaques can occur throughout the central nervous system in multiple sclerosis, possibly involving the visual pathways at any location. The proper evaluation of visual field defects in multiple sclerosis patients enables the optometrist to become a part of the health care team in monitoring the severity and progression of the disease. Three case reports are presented which illustrate the variable presentations and the proper interpretation of findings to localize demyelinating plaques in the visual pathways.

Temporary chromatopsia and alternate umbropsia in multiple sclerosis.

Uncommon visual symptoms were described in two women with clinically definite, remittent multiple sclerosis. In the first case with right optic atrophy temporary chromatopsia (vision of only blue colour) and left-sided, paroxysmal and alternate "fading out" followed by "brightening up" vision (umbropsia) occurred spontaneously. In the second case with bilateral optic atrophy transient chromatopsia (vision of only green colour) and short-lived "fading out" vision when fixating on a target were noted. Chromatopsia might be associated with impaired function of selected optic nerve fibers conducti…

[Somatosensory evoked potentials in the diagnosis of multiple sclerosis].

The authors examined SEP in a group of 54 patients with the clinical diagnosis of SM. The sensitivity of the test in SEP of the median nerve was 68%, in SEP of the tibial nerve 78%. Concurrent use of both techniques revealed an abnormality in 83%. Subclinical abnormalities were present in 28% (median nerve) and 24% resp. (tibial nerve) of the patients. In case of a clinical disturbance of sensitivity the SEP finding was normal in 10% of the patients. A typical abnormality in SM was the absence of spinal responses and prolongation of the central conduction time. In 41% of the patients with an a

Psychiatric disorders in the encephalitic form of multiple sclerosis.

Multiple sclerosis lesions may occur predominantly in the hemispheric white matter and cause various psychiatric disorders such as remitting-relapsing endoform or exogenous psychosis, organic personality alterations and dementia. Nineteen patients suffering from this encephalitic form of multiple sclerosis as diagnosed by characteristic CSF immunoglobulin findings are analysed according to established psychopathological criteria. All cases began with psychiatric symptoms and neurological signs were either absent or overlooked. Several patients developed typical encephalomyelitic symptoms in su…

The electroretinogram in multiple sclerosis and demyelinating optic neuritis.

The electroretinogram (ERG) to flashes of white light presented under photopic conditions and the pattern reversal visual evoked potentials (PR-VEPs) from both eyes were recorded from 14 patients with multiple sclerosis (MS) with monocular demyelinating optic neuritis (DON) and from 11 patients soon after presenting with monocular demyelinating optic neuritis alone. Fifteen and 10 normal subjects, matched for age and sex, were used as controls for each group of patients respectively. In the DON group of patients and controls the flicker following ERG (FF-ERG) to white flashes of light at 40 Hz…
