Acute disseminated encephalomyelitis following Plasmodium falciparum malaria caused by varicella zoster virus reactivation.

Neurologic complications in the course of Plasmodium falciparum infections are commonly diagnosed as cerebral malaria, but bacterial or vira … elpful for diagnostic decisions. Varicella zoster virus was identified with a reverse transcriptase multiplex polymerase chain reaction in the initially obtained and frozen CSF. This case and findings from the literature indicate that P. falciparum-associated ADEM might not be immune mediated, but of infectious origin. With unclear cerebral complications during or after P. falciparu …

Presence of human herpesviruses in young children with acute otitis media.

Some herpesviruses have been detected in middle ear fluid (MEF) of patients with acute otitis media (AOM), but their role in middle ear disease is unknown. We examined 73 middle ear fluid samples from 73 children with acute otitis media for the presence of four major herpesviral DNA, respiratory viral genomes, and bacterial DNA by multiplex polymerase chain reaction (PCR). Herpesviruses were detected in 16 specimens (22%), with 18 viral infections were identified overall. Respiratory viruses were detected in 35 specimens (48%), 39 viral infections overall. Bacterial DNA was detected in 51 spec…

Serologic and molecular diagnosis of Colorado tick fever viral infections.

Molecular and serologic methods usable for the biological diagnosis of Coltivirus infection are reported. We designed a multiplex reverse transcription-polymerase chain reaction system that allowed the simultaneous and specific amplification of three genomic segments from as little as 0.01 plaque-forming units. Another system in the S2 viral segment permitted the differential diagnosis of American and European viral isolates. We also discuss some improvements of previous ELISAs, and the results obtained with paired sera from Colorado tick fever (CTF) virus-infected individuals. Western blot an

DQB1-0602 (DQw1) is not present in most nonDR2 Caucasian narcoleptics.

Human narcolepsy is a genetically determined disorder of sleep strongly associated with the human leucocyte antigens (HLA) DR2 and DQw1. In black narcoleptic patients, susceptibility for narcolepsy is more closely related to a specific gene subtype of DQw1, DQB1-0602, than to DR2. About 30% of black narcoleptic patients are nonDR2, but all carry the HLA DQB1-0602 gene. In the present study, we have tested caucasian nonDR2 cataplectic patients (6 sporadic cases and 7 familial cases from 3 multiplex families) for the presence of the HLA DQB1-0602 and DQA1-0102 (DQw1) using a specific polymerase

A novel real-time PCR for New World screwworm fly (Cochliomyia hominivorax) and its application in a non-destructive multiplex for efficient detection of screwworm flies.

Surveillance and diagnostics are critical for the early detection, containment and eradication of exotic pests. For the screwworm fly, this is usually via targeted surveillance and exclusion testing of trap-caught flies, as well as the identification of larvae associated with myiasis wounds. We present a specific and sensitive real-time polymerase chain reaction (PCR) assay for the detection of the New World screwworm fly, Cochliomyia hominivorax Coquerel (Diptera: Calliphoridae). The assay targets the cytochrome oxidase subunit 1 (cox1) gene from adult flies or larvae and retains high analyti

Pathogen-targeted quick multiplex PCR identification of Chlamydia psittaci and Haemophilus influenzae coinfection in pneumonia: a case report.

This study describes a rare case of pneumonia induced by coinfection with Chlamydia psittaci (C. psittaci) and Haemophilus influenzae (H. influenzae). The patient had a history of chronic nephritis and long-term pigeon breeding. The initial manifestation was slurred speech, followed by rapid progression to high fever and respiratory failure. Chest computed tomography revealed bilateral interstitial pneumonia, predominantly in the right lung. Pathogen-targeted quick multiplex polymerase chain reaction (pt-qPCR) confirmed the pathogens as C. psittaci and H. influenzae, and metagenomic next-gener…

First Identification of an Autochthonous Case of Chikungunya in Guinea.

The first confirmation of chikungunya virus infection in Guinea is reported in the present study. The patient, a 68-year-old woman presumptively diagnosed with malaria and with no history of travel, was identified through a research project focused on the etiologies of febrile illnesses in Guinea. A whole-blood sample from the patient was analyzed using multiplex reverse-transcription polymerase chain reaction, yielding a positive result for chikungunya virus, and was subsequently sequenced. The near-complete genome sequence was most closely related to strains circulating that year in Senegal.

Extrapulmonary Paragonimiasis Diagnosed by Molecular Tools.

Paragonimiasis is a parasitic disease that involves the lungs but can manifest in extrapulmonary sites, including the peritoneum or subcutaneous tissue. Diagnosis is limited by nonspecific clinical manifestations and overlapping features with other diseases, such as tuberculosis (TB). A 55-year-old Korean male presented with acute abdominal pain and a recurrent subcutaneous mass in the left thigh. Although TB peritonitis was initially suspected, further diagnostic tests were performed. To confirm the diagnosis, multiplex real-time polymerase chain reaction (PCR) targeting the 28S rRNA gene and

Quantitative real-time PCR in the diagnosis of opportunistic infections in immunocompromised patients: Diagnostic performance, clinical utility, and future directions.

Opportunistic infections (OIs) remain a major cause of morbidity and mortality among immunocompromised individuals, and delayed or inaccurate diagnosis can rapidly lead to adverse clinical outcomes. This narrative review critically evaluates the diagnostic performance, clinical utility, and evolving applications of quantitative real-time polymerase chain reaction (qPCR) for detecting opportunistic pathogens in vulnerable patient populations. Evidence across bacterial, fungal, viral, and parasitic infections demonstrates that qPCR consistently surpasses conventional culture- and serology-based
