Carcinoma of the base of the tongue: results of radical irradiation with surgery reserved for irradiation failure.

Between 1964 and 1977, 95 previously untreated patients with squamous cell carcinoma of the base of the tongue received treatment with curative intent at the University of Florida. Eighty-six of the 95 patients (91%) had Stage III or IV disease at presentation. Eighty-nine patients received radical courses of irradiation to the primary with or without neck dissection(s), with surgery reserved for salvage of irradiation failure. Six patients underwent planned combined treatment of the primary lesion. Of the 89 patients whose primary lesions were radically irradiated, failure at the primary site…

High-located aberrant innominate artery: an unusual cause of serious hemorrhage of percutaneous tracheotomy.

An aberrant innominate artery located high was observed during a cadaveric neck dissection. To our knowledge, an innominate artery crossing the 4th and 5th tracheal rings has rarely been reported in available literature. Knowledge of this anomaly is important for those who are involved with percutaneous procedures in the neck to avoid major complications.

Surgical strategy for airway management and bleeding control in penetrating neck injury in zone II: A case report.

We report the case of a 33-year-old man with a self-inflicted neck wound with severe hypopharynx injury, and hemorrhagic shock, which was well managed by a trauma surgeon trained in esophageal surgery. Training in cervical lymph node dissection for esophageal cancer could be useful in the management of penetrating neck injuries.

Dissecting aneurysms of the vertebral arteries following cervical manipulation: a case report.

Neck manipulation may uncommonly be associated with serious and even fatal vascular complications. Although well recognised, the nature of the vascular injury has only rarely been directly established by pathological examination. The case is reported of a 43-year-old man who died following neck manipulation, and in whom multiple dissecting aneurysms within both vertebral arteries were demonstrated radiologically and found at necropsy. Bilateral dissecting aneurysms were found both at the level of atlanto-axial articulation and close to the origins of the vertebral arteries. No predisposition w

Delayed fatal hemothorax due to traumatic carotid dissection: a case report of a previously unreported cause of death.

A unique case of delayed fatal hemothorax in a 12-year-old girl resulting from atypical dissection of a traumatic carotid aneurysm is reported, due to occult neck trauma received in a sledding accident which occurred one week prior to death. Aspects of traumatic carotid dissection and forensic implications of delayed presentation of such findings in the setting of occult trauma are discussed.

Radio-anatomopathological discrepancies in lymphonodal staging of squamous cell carcinoma of the upper aerodigestive tract.

OBJECTIVES: The therapeutic strategy for squamous cell carcinomas of the upper aerodigestive tract (SCC-UADT) relies, in part, on the histopathological study of the neck dissection. Currently, several studies aim to limit the use of lymph node neck dissection based on a radioclinical cTNM evaluation. The objective of this study was therefore to assess the accuracy of clinical and radiological examination in nodal analysis of SCC-UADT compared with the histopathological assessment of neck dissection, in order to evaluate the risk of inadequate oncological management in the absence of decision-m…

Prevalence of submandibular gland involvement in oral carcinoma. A retrospective cohort study.

BACKGROUND: Refining the overall survival of oral cancer patients has gained traction in the treatment of oral cancer by offering a good quality of life. Restoring the anatomy and function of the glands in the oral cavity has also been an issue of attention. During every neck dissection, the submandibular gland (SMG) is electively removed as a prophylactic measure. The aim of this study was to evaluate the prevalence of SMG involvement in oral cancer patients and to determine the oncological safety of preserving the SMG during neck dissection in patients with oral cancer. RESEARCH DESIGN AND …

[Sentinel lymph node in oral and oropharyngeal epithelial tumors].

Carcinomatous metastases in regional lymph nodes worsen substantially the prognosis of patients with oral cavity and oropharyngeal cancer. Due to the high probability of occult metastasis (about 30%), during surgical resection of the primary tumor usually also elective dissection of lymph nodes is performed. Opinions on the extent of the elective neck dissection still differ, whereas selective dissection increasingly gains in importance. The aim of selective dissections, based on the predictability of formation of metastases, is the identification and exstirpation of the sentinel lymph node. I…

Treatment of infrarenal aortic aneurysm by minimally invasive retroperitoneal approach: use of a video-assisted technique.

We propose a technique for treating abdominal aortic aneurysm in which first a minilaparotomy (retroperitoneal) is performed to make a conventional arterial dissection; then, video assistance is used to control the left lumbar arteries and to make the aortic anastomosis. This technique can be performed with no significant laparoscopic capability. The patient was installed in a 20-degree right lateral decubitus. A transverse left 8- to 10-cm abdominal incision was made, followed by separation of the muscular structures. Retroperitoneal control of the proximal aortic neck and of both common iliac

PURPOSE: To report long-term results using radiotherapy with or without a planned neck dissection for T1-T2 carcinoma of the pyriform sinus. METHODS: An analysis of 101 patients treated at the University of Florida with RT with or without a planned neck dissection for organ preservation. RESULTS: The 5-year local control rates after RT were 90% for T1 cancers and 80% for T2 lesions. The only parameter that significantly influenced local control in univariate analyses was apex involvement for T1 tumors. Multivariate analysis revealed no parameter that significantly affected local control. Cau…

A Case of Dissection and Rupture of the Innominate Artery in Acute Type A Aortic Dissection.

We report the case of a 59-year-old male presenting with dissection and rupture of the innominate artery with acute type A aortic dissection. We performed total arch replacement via median sternotomy with extension into the right side of the neck. The innominate artery was reconstructed just proximal to the bifurcation of the right subclavian artery and common carotid artery using the 1st limb of a 4-branched graft. This case illustrates an unusual aspect of an isolated innominate artery rupture in the absence of aortic rupture in acute type A aortic dissection.

Severe Orthostatic Hypotension due to Unilateral Carotid Artery Dissection.

A 46-year-old woman was admitted to our hospital with mild right-sided hemiparesis and orthostatic hypotension. Magnetic resonance angiography of the neck showed stenosis of the left distal carotid sinus surrounded by intraluminal hyperintensities on both T1-weighted and T2-weighted images, representing a periluminal hematoma secondary to carotid artery dissection. The dissection hyper-extended the carotid artery wall and stimulated baroreceptors in the carotid sinus. The stimulated baroreceptors induced carotid sinus hypersensitivity, which may have been related to her orthostatic hypotension

Raeder's paratrigeminal syndrome due to spontaneous dissection of the cervical and petrous internal carotid artery.

The combination of pain, ipsilateral oculosympathetic defect (ptosis and miosis), and ipsilateral trigeminal dysfunction constitutes Raeder's syndrome. We describe a patient with an acute presentation of Raeder's syndrome due to spontaneous internal carotid artery dissection. True trigeminal dysfunction due to carotid dissection is rare, and the potential mechanisms for its involvement are reviewed in this paper. Finally, we remind clinicians to consider dissection in the differential diagnosis of Raeder's syndrome because of its potential for ischemic cerebral neurologic sequelae and suggest
