Somatostatin receptor expression in a parathyroid hormone-related peptide-secreting pancreatic neuroendocrine tumour causing severe hypercalcaemia.

BACKGROUND: Humoral hypercalcaemia is a common complication of malignancy with parathyroid hormone-related protein (PTHrP) as a major cause. … and sst5 were not detected. We demonstrate the transient improvement of hypercalcaemia after adding octreotide to the treatment mainstays in hypercalcaemia of malignancy (fluid repletion, administration of bisphosphonates, loop diuretics, and glucocorticoids). CONCLUSION: To the best of our knowledge, this is the first report showing somatostatin receptor e …

Cardiac compromise due to a pancreatic mediastinal pseudocyst.

Cardiac complications from a pancreatic mediastinal pseudocyst are rare. Pericardial effusions associated with pancreatitis have been report … ricardial catheter was placed producing rapid symptomatic relief. Surgery was avoided by the use of octreotide as an adjuvant to ultrasound guided catheter drainage of the pseudocyst and it resolved completely within 4 weeks of admission to hospital. The importance of rapid and accurate diagnosis of this life-threatening complication is reiterated and the ma …

Endoscopic treatments for portal hypertension.

Endoscopic treatments for bleeding gastroesophageal varices include injection sclerotherapy, variceal obturation with tissue adhesives, and variceal rubber band ligation. Acute injection sclerotherapy remains a quick and simple technique for the control of active bleeding from esophageal varices. Although few trials have been published so far, some evidence suggests that the early administration of vasoactive drugs (somatostatin, octreotide, or terlipressin) is safe and may increase the efficacy of endoscopic treatments. Banding ligation is the optimal endoscopic treatment for the prevention o…

Continuous intravenous Octreotide treatment was introduced on day 72. Due to the recurrence of hypoglycaemia episodes despite reaching maximum doses of Octreotide, from day 92 the treatment was switched to Pasireotide. Genetic tests revealed the same genotypes for both infants: the …

Somatostatin receptors over-expression in castration resistant prostate cancer detected by PET/CT: preliminary report of in six patients.

Prostate cancer (PC) is usually characterized by an excellent prognosis, largely due to little biological aggressiveness and the power of ho … eceptors have been synthesized during the past two decades for diagnostic and therapeutic purposes. Octreotide, the most popular of these, is widely used to treat patients affected by neuroendocrine tumors. A number of researches carried out in the past evaluated the possible neuroendocrine differentiation (NED) of PC cells in the castration resistant phase. …

Successful Management of Pancreatic Ascites with both Conservative Management and Pancreatic Duct Stenting.

Pancreatic ascites is a rare complication and should be suspected in patients with chronic alcoholism and pancreatitis presenting with ascites. The etiology is likely from a pancreatic pseudocyst leakage or due to ductal disruption. Treatment is controversial but includes conservative medical therapy or endoscopic transpapillary pancreatic duct stenting or surgery. We present a case of pancreatic ascites in a patient with alcohol use and chronic pancreatitis. Patient received conservative therapy including octreotide. An endoscopic retrograde cholangiopancreatography was performed, which confi…

Advanced pediatric inoperable thymus carcinoma (type C thymoma): case report on a novel therapeutic approach.

Although there has been considerable advancement in treatment techniques but still there are some illnesses that continue to exhibit a rather poor curability, such as thymoma. This report highlights the benefit of octreotide and prednisolone therapy in a 15-year-old girl, who was diagnosed with inoperable thymus carcinoma, with chemotherapy and radiotherapy being the last resort. The detection of type 2 somatostatin receptors on the surface of the tumor justified the introduction of treatment with somatostatin analog and prednisolone. Fortunately, after 6 months of this treatment, the tumor sh…

Treatments include pharmacologic (propanolol, somatostatin, octreotide, vasopressin, and nitroglycerin), endoscopic (injection sclerotherapy and banding), vascular (transjugular intrahepatic portosystemic shunt), surgical (portovariceal disconnection and portosystemic shunts), and tamponade (via use of the Minnesota tube). This article will focus on use and care of the Minnesota tube, including numerous suggestions from the literature. Tamponade for treatment of esophageal varices may be accompanied by numerous complication…

[Surgical treatment of portal hypertension in schistosomiasis mansoni].

Portal hypertension is defined by an hepatic venous pressure gradient greater than 5mmHg. It is usually caused by an increase in resistance in the portal-hepatic vascular bed due to obstruction to flow. The severe forms of schistosomiasis usually evolve with portal hypertension, esophageal, intraabdominal, retroperitoneal and abdominal wall varices. Massive bleeding due to esophageal or gastric variceal rupture is the major complication of portal hypertension in schistosomiasis. In regard to treatment, clinical (propranolol, somatostain, octreotide), endoscopic (sclerotherapy, clips and ligatu…

Systemic treatment of hepatocellular carcinoma.

Systemic treatment for hepatocellular carcinoma is indicated in locally advanced or metastatic disease. Monochemotherapies have yielded unsatisfactory results with response rates of around 20% but survival is often not improved. Polychemotherapies may induce complete responses but have substantial toxicity and are limited to selected patients with preserved liver function. Hormonal treatment with tamoxifen is ineffective while megestrol has shown an improvement in quality of life. Octreotide can be given even in cases of impaired liver function, has also a favorable side effect profile and can…

[Carcinoid syndrome: advances in diagnosis and treatment].

Carcinoid is the most common endocrine digestive tumor. The carcinoid syndrome resulting from the variety of amines and peptides produced by this tumor is usually apparent once there are metastases to the liver. Tumors with direct systemic venous drainage seldom produce a carcinoid syndrome without the presence of liver metastasis. This may occur because the hormone escapes the normal metabolic pathway (monoamine oxidase) in the liver. The most significant and important advance in diagnosis for tumor localization has been the introduction of scintigraphy using 111In-labeled octreotide. Current…

Characterization of the somatostatin system in tilapia: implications for growth and reproduction.

18 F-OC PET/CT Localization of a Phosphaturic Mesenchymal Tumor Presenting with Tumor-Induced Osteomalacia.

Phosphaturic mesenchymal tumor (PMT) is an uncommon neoplasm and the leading cause of tumor-induced osteomalacia (TIO). We report a 55-year-old woman with a 2-year history of progressive hip pain and hypophosphatemia. Laboratory findings demonstrated renal phosphate wasting. Conventional imaging revealed a small soft-tissue nodule in the right thigh, and 18 F-AlF-NOTA-octreotide (18F-OC) PET/CT (positron emission tomography/computed tomography) showed intense radiotracer uptake, accurately localizing the culprit lesion. Surgical excision confirmed a PMT, and serum phosphate levels normalized p…
