Fiberoptic tracheal intubation through a classicial laryngeal mask airway under spontaneous ventilation in a child with Treacher Collins syndrome.

This report cites the usefulness of fiberoptic scope-guided endotracheal tube intubation through a classical laryngeal mask airway (LMA) during spontaneous breathing. Treacher Collins Syndrome (TCS) is a condition where airway management is stressful to anesthesiologists. We report a pediatric patient with TCS undergoing cleft palate repair. The patient had a history of sleep apnea syndrome, chronic lung disease, and congenital heart disease. Intubation by rigid laryngoscopy was unsuccessful at the first attempt. One month later, under spontaneous ventilation, tracheal intubation was smoothly

Pediatric Preassessment Service Evaluation Across the South Thames Pediatric Network.

BACKGROUND: Pediatric preassessment services across the United Kingdom remain variably developed and inconsistently funded when compared to adult care. There has been little systematic evaluation of pediatric preassessment models across regions or networks. The aim is to evaluate the quality of pediatric preassessment within the South Thames Pediatric Network (STPN). By engaging families across a large, multi-organizational network, this questionnaire collated evidence to inform local and national service improvement, promote equitable access and enhance patient experience for children. Use of paralytic agents during the operative treatment of type III supracondylar humerus fractures in children: A single-center investigation and survey of Pediatric Orthopaedic Society of North America members.

BACKGROUND: Paralytic agents are occasionally used during the surgical treatment of type III (OTA 13A2) supracondylar humerus fractures (SCHFx) in children depending on surgeon preference. Paralytic agents create a neuromuscular blockade and therefore may potentially help with ease of fracture reduction. Controversy regarding the use of a paralytic agent as an adjunct to anesthesia exists due to potential associated adverse drug reactions, including prolonged paralysis, cardiovascular effects, or electrolyte abnormalities. The purpose of this study was to investigate intraoperative paralytic us

Analgesic and Sedative Effects of Virtual Reality on Children with Acute Pulpitis: A Study Protocol for a Randomised Controlled Trial.

BACKGROUND: Root canal treatment is common in dentistry. Pulp exposure and drainage during the procedure cause significant discomfort in children, triggering fear and anxiety that reduce their physical comfort and treatment compliance. Virtual reality (VR) glasses divert patients' attention from pain, alleviate anxiety, and feature simple operation and high safety. This study aims to investigate the safety and efficacy of virtual reality technology during pulp chamber open for drainage, providing empirical evidence for alleviating pain and reducing anxiety in pediatric patients undergoing the

Transient Facial Nerve Palsy as a Complication of Local Anesthesia After Tonsillectomy.

Chronic tonsillitis is a common otolaryngological disease worldwide. The treatment of choice is tonsillectomy, which is one of the most frequently performed procedures in Ear, Nose and Throat Departments. As with any surgical procedure, there are possible complications. Among them, rarely encountered and reported mainly in the pediatric population, is a short-term, transient facial nerve palsy resulting from local anesthetic administration. Here, we present a rare case of a 20-year-old woman with recurrent angina, who developed short-term, fully transient peripheral facial nerve palsy immediat

Severe intraoperative bronchospasm treated with a vibrating-mesh nebulizer.

Bronchospasm and status asthmaticus are two of the most dreaded complications that a pediatric anesthesiologist may face. With the occurrence of severe bronchospasm and the inability to ventilate, children are particularly vulnerable to apnea and ensuing hypoxia because of their smaller airway size, smaller lung functional residual capacity, and higher oxygen consumption rates than adults. Nebulized medication delivery in intubated children is also more difficult because of smaller endotracheal tube internal diameters. This case demonstrates the potentially lifesaving use of a vibrating-mesh m

Minor increase of endtidal CO2 during sevoflurane-induced malignant hyperthermia.

Malignant hyperthermia (MH) in a pediatric patient during sevoflurane anesthesia with only a minor rise of endtidal CO(2) is described. MH was considered because of increased rectal temperature. The outcome was favorable after the initiation of a few simple measures (increased minute ventilation, cessation of sevoflurane). As recommended by the regional MH Reference Center, dantrolene was not used. In vitro contracture tests were performed on muscle biopsies from both parents. A strong contracture in response to halothane confirmed the father's MH susceptibility, thereby according a high proba

Effective doses of esketamine oral or esketamine intranasal for the prevention of preoperative anxiety in pediatric patients: A randomized double-blind dose-finding trial.

BACKGROUND: Preoperative anxiety is a prevalent concern among preschool pediatric patients. Esketamine, an S-enantiomer of ketamine, has emerged as a potential option for pediatric premedication. This study aimed to determine the 95% effective dose (ED95) of oral or intranasal esketamine for alleviating preoperative anxiety in this age group. METHODS: Children patients aged between 1 to 6 years old who were undergoing elective surgery under general anesthesia were recruited in the study. The initial patient received a single dose of esketamine orally (2.5 mg/kg) or intranasally (1 mg/kg), whi

Early Institutional Experience With the Dynamic Femur Fracture (DF2) Brace for Pediatric Femoral Fractures: An Institutional Case Series.

Traditional spica casting is the standard treatment for pediatric femoral fractures in children under five years of age, but it carries well-documented limitations, including the need for general anesthesia, increased caregiver burden, and high rates of skin complications. Prefabricated functional bracing, such as the Dynamic Femur Fracture (DF2) brace (OrthoPediatrics, Warsaw, IN), has emerged as an alternative that eliminates anesthesia exposure and improves patient mobility. However, clinical data regarding the practical limitations, complication profiles, and patient selection boundaries d

Noninferiority of Ultrasound-Guided Modified Intercostal Block to Traditional Approach for Analgesia After Minimally Invasive Repair of Pectus Excavatum in Children: A Randomized Trial.

BACKGROUND: For pediatric pectus excavatum, the standard treatment is minimally invasive repair of pectus excavatum (MIRPE). A major challenge, however, is the severe postoperative pain. Although ultrasound-guided intercostal nerve block (UINB) offers effective analgesia, the technique's complexity and associated safety concerns are significant barriers, deterring its routine use. Modified intercostal nerve block (MINB) is effective in adult thoracic surgery but unvalidated in pediatric MIRPE. OBJECTIVE: To evaluate MINB's noninferiority to UINB for postoperative analgesia and safety in child

Magnitude and associated factors of intraoperative hypothermia among pediatric patients undergoing elective surgery at Ayder Comprehensive Specialized Hospital 2023: prospective observational cross-sectional study Tigray, Ethiopia.

BACKGROUND: Hypothermia is characterized by a drop in core body temperature of less than 36°C. It occurs frequently throughout the operating period and affects surgical patient outcomes differently in terms of morbidity and mortality. Because of coagulopathy, metabolic acidosis, multiple organ failure, hemodynamic instability, and infections, a core temperature below 34°C is strongly associated with mortality. OBJECTIVE: This study aimed to assess the magnitude and associated factors of intraoperative hypothermia in pediatric patients undergoing elective surgery at the Ayder Comprehensive Spe

Factors associated with intravenous lidocaine in pediatric patients undergoing laparoscopic appendectomy - a retrospective, single-centre experience.

BACKGROUND: Due to its potential beneficial effects, intra- and postoperative application of intravenous lidocaine has become increasingly accepted over the last couple of years, e.g. in patients undergoing laparoscopic surgical procedures. Based on its beneficial properties, lidocaine was introduced to the standard of care for all pediatric laparoscopic procedures in our institution in mid-2016. In contrast to adult care, scarce data is available regarding the use of perioperative intravenous lidocaine administration in children undergoing laparoscopic procedures, such as an appendectomy. [Anesthetic Management of a Pediatric Case of Blue Rubber Bleb Nevi Syndrome Combined with Small-intestinal Intussusception].

We report the anesthetic management of a pediatric case of blue rubber bleb nevi syndrome combined with small-intestinal intussusception. A 2-year-old girl was transferred to our hospital for small-intestinal intussusception. Emergent ablation of the upper gastrointestinal tract nevus under general anesthesia was planned. Given the presence of several nevi in the oral and pharyngeal space, we utilized the McGRATH MAC (McGRATH; Aircraft Medical Ltd, United Kingdom) laryngoscope for gentle and stress-free tracheal intubation. The venous line was kept patent preoperatively, and rapid-sequence int

Cardiomyopathy in childhood.

PURPOSE OF REVIEW: Cardiomyopathy is an important cause of heart failure and a major indication for heart transplantation in children. Unfortunately, there is a paucity of literature to guide the anesthesiologist who cares for these high-risk children. This review describes the cardiomyopathy phenotypes that occur in children and the factors that are associated with clinical outcomes and perioperative complications. Anesthesia considerations will be reviewed. RECENT FINDINGS: During the past decade, there has been a dramatic increase in knowledge related to cardiomyopathy. New genotypes and p

[Pharyngeal foreign bodies in infants persisting for two months: two case reports].

In general, patients who suffer from pharyngeal foreign bodies can explain the time and nature of the object which had been swallowed, resulting in easy diagnosis. However, in infants, difficulty in communication makes diagnosis troublesome. Two cases of long-standing pharyngeal foreign bodies were treated at our hospital. Case 1 was a 16-month-old girl complaining of stridor and fever. She had fallen with a toothbrush in her mouth two months before, and was hospitalized in the pediatrics department for one week with upper airway inflammation and dehydration. A toothbrush head was embedded in
