Chronic suppression of a multidrug-resistant Pseudomonas aeruginosa in prosthetic joint infection using personalized bacteriophage treatment.

Multidrug resistant (MDR) bacterial infections without antibiotic options are a public health emergency. Infections associated with medical implants serve as an example. Conventional antibiotics have limited ability to eradicate these infections as they are associated with antibiotic-tolerant biofilms. Here, we report the use of bacteriophage therapy for the treatment of a MDR, non-operable Pseudomonas aeruginosa periprosthetic joint infection that had failed multiple antibiotic and surgical interventions. Treatment with intermittent bacteriophage therapy alone without antibiotics over a 2 yea

Effect of Caerin 1.9 Antimicrobial Peptide Gel on Early Bacterial Infections in Oral Titanium Implants: An Animal Study.

OBJECTIVE: The current study aims to investigate the antibacterial potential of caerin 1.9 in the form of a thermo-sensitive gel for prevention and management of early postoperative infection of oral titanium metallic implants. METHODS: Twenty-two New Zealand rabbits were randomly assigned to three groups: the F3 (caerin 1.9) group (n = 8), the control group (n = 8), and the blank group (n = 6). The early infection model of implant was established by implanting the micro-titanium nail after oral mixed bacteria drip into the mandible. The F3 group was given early intervention with caerin 1.9 a

Simultaneous Bilateral Methicillin-Resistant Staphylococcus aureus Infection After Total Knee Arthroplasty Successfully Treated with Continuous Local Antibiotic Perfusion: A Case Report.

BACKGROUND Continuous local antibiotic perfusion (CLAP) delivers gentamicin continuously via a bone marrow needle and double-lumen tube in the infected area, taking advantage of the concentration-dependent bactericidal activity of gentamicin, and allowing even resistant bacteria to be inhibited at high local concentrations. CLAP has shown good outcomes in bone and soft-tissue infections. However, reports on its use for periprosthetic joint infection (PJI) after total knee arthroplasty are limited. Moreover, bilateral simultaneous methicillin-resistant Staphylococcus aureus (MRSA) infection fol

Prosthetic Joint Infection Caused by Streptococcus pneumoniae Serotype 15F: A Rare Manifestation of an Emerging Non-vaccine Serotype.

Prosthetic joint infection (PJI) caused by Streptococcus pneumoniae is rare, particularly in the absence of bacteremia. This case report describes a 69-year-old Japanese woman with a history of follicular lymphoma and bilateral total hip arthroplasty who developed a right periprosthetic abscess caused by S. pneumoniae. This was an unusual presentation as the patient lacked typical respiratory or neurological symptoms or bacteremia. The causative strain was identified as serotype 15F, a non-vaccine serotype (NVT) not covered by current pneumococcal vaccines. This case highlights serotype replac

An overview of risk factors, management and prevention of cochlear implant infections.

PURPOSE OF REVIEW: With cochlear implantation becoming increasingly performed worldwide, an understanding of the risk factors, preventive measures, and management of cochlear implant (CI) infection remains important given the significant morbidity and cost it conveys. RECENT FINDINGS: At the turn of the 21st century there was a decrease in rates of CI infection, particularly meningitis, following the discontinuation of positioner use for CI. However, in more recent years rates of CI infection have remained largely static. Recently, studies evaluating preventive measures such as pneumococcal v

Successful Treatment of Staphylococcus aureus Prosthetic Joint Infection with Bacteriophage Therapy.

Successful joint replacement is a life-enhancing procedure with significant growth in the past decade. Prosthetic joint infection occurs rarely; it is a biofilm-based infection that is poorly responsive to antibiotic alone. Recent interest in bacteriophage therapy has made it possible to treat some biofilm-based infections, as well as those caused by multidrug-resistant pathogens, successfully when conventional antibiotic therapy has failed. Here, we describe the case of a 61-year-old woman who was successfully treated after a second cycle of bacteriophage therapy administered at the time of a

High re-revision rate following one-stage exchange for streptococcal periprosthetic joint infection of the hip.

INTRODUCTION: This study was undertaken to analyse the outcome of 1-stage exchange in the management of streptococcal periprosthetic joint infection (PJI) of the hip at a single hospital. METHODS: We identified 30 patients with a streptococcal PJI of the hip who had been treated by 1-stage exchange at our hospital between 2002 and 2017. Postoperative complications and the need for any subsequent re-revision for infection or other reasons were analysed. The Harris Hip Score (HHS) was evaluated at final follow-up. RESULTS: The mean follow-up was 8.2 years (SD 4.1). The overall re-revision rate

Periprosthetic joint infection with streptococcus dysgalactiae subspecies equisimilis: Case report.

Streptococcus dysgalactiae (SD) is a common pathogen among elderly population. However, to our knowledge, there is no periprosthetic joint infection case reported that is infected with Streptococcus dysgalactiae subspecies equisimilis (SDSE) in the English literature. In this article, we report a 77-year-old male patient who had undergone total knee arthroplasty three years ago and had the diagnosis of cellulitis at his leg followed by swelling, pain and hyperemia localized at his knee. Three knee aspirations were performed and the SDSE was identified. There was no direct contact of patient to

Biofilm formation and migration on ventricular assist device drivelines.

OBJECTIVES: Driveline infections remain an important complication of ventricular assist device therapy, with biofilm formation being a major contributor. This study aimed to elucidate factors that govern biofilm formation and migration on clinically relevant ventricular assist device drivelines. METHODS: Experimental analyses were performed on HeartWare HVAD (HeartWare International Inc, Framingham, Mass) drivelines to assess surface chemistry and biofilm formation. To mimic the driveline exit site, a drip-flow biofilm reactor assay was used. To mimic a subcutaneous tissue environment, a tunn

In vitro activity of tedizolid and linezolid against Staphylococcus epidermidis isolated from prosthetic joint infections.

Prosthetic joint infections (PJIs) are rare but long-lasting and are serious complications without any spontaneous resolution, requiring additional surgery and long-term treatment with antibiotics. Staphylococci are the most important aetiological agents of PJIs, and among the coagulase-negative staphylococci Staphylococcus epidermidis is the most common. However, S. epidermidis often displays multidrug resistance (MDR), demanding additional treatment options. The objective was to examine the effectiveness of tedizolid and linezolid against S. epidermidis isolated from PJIs. The standard antib

Effects of Implant-Associated Osteomyelitis on Cefuroxime Bone Pharmacokinetics: Assessment in a Porcine Model.

BACKGROUND: The prolonged antibiotic therapy that is often needed for successful management of osteomyelitis may be related to incomplete penetration of antibiotics into the target site. The objective of this study was to assess the effects of implant-associated osteomyelitis on cefuroxime penetration into bone. METHODS: Implant-associated osteomyelitis using a Staphylococcus aureus strain was induced in the right tibia in ten pigs. After five days and following administration of 1500 mg of cefuroxime, measurements of cefuroxime were obtained using microdialysis for eight hours in the implant

Antifouling foldable acrylic IOLs loaded with norfloxacin by aqueous soaking and by supercritical carbon dioxide technology.

Cataracts treatment usually involves the extraction of the opaque crystalline lens and its replacement by an intraocular lens (IOL). A serious complication is the occurrence of endophthalmitis, a post-surgery infection mainly caused by Staphylococcus epidermidis, Staphylococcus aureus, and Pseudomonas aeruginosa. IOLs having the ability to load and to release norfloxacin in a controlled way and at efficient therapeutic levels may help to overcome these issues. In this work, acrylic hydrogels combining 2-hydroxyethyl methacrylate (HEMA) and 2-butoxyethyl methacrylate (BEM) at various ratios wer

Coagulase-positive Staphylococcus pseudintermedius from animals causing human endocarditis.

We report a case of infection with coagulase-positive Staphylococcus pseudintermedius related to the implantation of a cardioverter-defribrillator device. This species is usually isolated from infected animals, and contact with a dog was the probable source of infection in this patient. This isolate produced a leukotoxin effective against human polymorphonuclear leukocytes.

Dacron patch infection after carotid endarterectomy: case report and review of the literature.

Patch angioplasty of the carotid artery after endarterectomy is a widely utilized technique with potential benefits of perioperative and long-term stroke risk reduction as well as a decreased incidence of recurrent stenosis. One of the complications of using a synthetic material as a patch is the development of prosthetic patch infection. We present a case report of a patient who was treated at our institution with such a complication and review the literature on this rare occurrence. The risk factors, workup, diagnosis, and treatment of prosthetic patch infection are reviewed. Despite the ser

Endocarditis after closure of ventricular septal defect by transcatheter device.

Advances in interventional cardiology have enabled the treatment of severe congenital heart defects without the need for surgery. The percutaneous closure of atrial septal defects and, more recently, ventricular septal defects is considered a safe procedure with fewer complications and less morbidity compared with surgery. We report on a 2-year-old child who developed endocarditis after ventricular septal defect closure with an Amplatzer device. The patient recovered after intravenous antibiotics and anticoagulation. To the best of our knowledge, this is the first report of endocarditis associ
