Pulmonary Lobar Torsion following Bronchoscopic Lung Volume Reduction with Endobronchial Valves: A Case Report.

INTRODUCTION: Bronchoscopic lung volume reduction (BLVR) with endobronchial valves (EBVs) is an effective and minimally invasive alternative to lung volume reduction surgery for some patients with severe emphysema. While generally well tolerated, it may lead to rare but serious complications. Pulmonary lobar torsion and secondary infarction are known postoperative complications after thoracic surgery but have not been previously reported following BLVR. CASE PRESENTATION: A 70-year-old woman with chronic obstructive pulmonary disease underwent EBV placement in the right upper lobe, with a goo

Use of Autotransfusion following Percutaneous Thrombectomy for Cardiogenic Shock Due to Pulmonary Embolism in a Single Session-A Case Report.

A 64-year-old male patient was admitted to the catheterization laboratory with a suspected myocardial infarction and in cardiogenic shock. Upon further investigation, a massive bilateral pulmonary embolism with signs of right heart dysfunction was discovered, leading to a decision to perform a direct interventional treatment with a thrombectomy device for thrombus aspiration. The procedure was successful in removing almost the entirety of the thrombotic material from the pulmonary arteries. The patient's hemodynamics stabilized and oxygenation improved instantly. The procedure required a total

Diagnostic Bias in the COVID-19 Pandemic: A Series of Short Cases.

UNLABELLED: During the COVID-19 pandemic, healthcare systems have faced unprecedented pressures. One challenge has been to promptly recognise non-COVID-19 conditions. Cognitive bias due to the availability heuristic may cause difficulties in reaching the correct diagnosis. Confirmation bias may also affect imaging interpretation. We report three cases with an alternative final diagnosis in whom COVID-19 was initially suspected: (a) Pneumocystis jirovecii pneumonia with unrecognised HIV infection; (b) pulmonary lymphangitis carcinomatosis; and (c) ST elevation myocardial infarction causing acut

Unique ECG Findings in Acute Pulmonary Embolism: STE with Reciprocal Changes and Pathologic Q Wave.

A 68-year-old male presented to the emergency department with retrosternal chest pain, presyncope, and then a pulseless electrical activity cardiac arrest. An ECG prior to his arrest revealed ST elevations in leads V1-V3, Q waves in lead V2, and reciprocal ST depressions in the lateral and inferior leads. He received thrombolytic therapy for a presumptive diagnosis of ST elevation myocardial infarction. Return of spontaneous circulation was achieved and he underwent a coronary angiogram. No critical disease was found and his left ventriculogram showed normal contraction. His ongoing metabolic

Individual anatomy demands various techniques in correction of an anomalous origin of the left coronary artery in the pulmonary artery.

Anomalous origin of the left coronary artery (ALCA) from the pulmonary artery is a rare cardiac anomaly. It can result in decreased myocardial perfusion and impaired left-ventricular function, which can lead to congestive heart failure or even myocardial infarction. Reconstructive surgery of the left coronary artery is the desirable measure in the management of this anomaly. Since July 1992, five patients with ALCA from the pulmonary artery were treated surgically. Age at operation ranged from 3 months to 11 years. Three patients had congestive heart failure and one was in cardiogenic shock. O

Cardiovascular death among women under 40 years of age using low-estrogen oral contraceptives and intrauterine devices in Finland from 1975 to 1984.

From 1975 to 1984, there were 1,585,000 women-years of oral contraceptive use and 1,975,000 women-years of copper-bearing intrauterine device use in Finland. During this 10-year period, 20 women between 15 and 39 years of age died of pulmonary embolism, 41 of coronary heart disease, and 311 of intracranial hemorrhage. The diagnoses were confirmed in 89% at necropsy examination. The contraceptive method used by patients was determined in 84% by means of inquiry sent to family planning clinics. Among the 20 patients who died of pulmonary embolism, four died while taking the pill, which gives a r

Acute Pulmonary Embolism With Multiple ST-Segment Elevations in Anterior and Inferior Leads: A Case Report.

BACKGROUND: Acute pulmonary embolism (APE) is a common and potentially life-threatening cardiovascular emergency. Although APE typically presents with nonspecific electrocardiogram (ECG) abnormalities, a rare but critical presentation may mimic ST-segment elevation myocardial infarction. We report a case of APE with multiple ST-segment elevations in the anterior and inferior leads. CASE PRESENTATION: A 74-year-old man presented with chest tightness and dyspnea. His initial ECG revealed sinus tachycardia with ST-segment elevation in Leads III, aVF, and V1-V4, along with T-wave inversion and pa

An autopsy case of pulmonary arterial hypertension in an elderly patient with multimorbidity: a case report.

BACKGROUND: There is an increasing number of elderly patients with pulmonary arterial hypertension (PAH), and their characteristics differ from those of young or middle-aged patients with this condition. CASE SUMMARY: A 73-year-old woman with a history of myocardial infarction and cardiovascular risk factors was admitted to the hospital with 2-week exertional dyspnoea. Her initial diagnosis was heart failure with preserved ejection fraction, but the symptoms persisted despite receiving treatment with diuretics. Additional tests showed a significant decrease in diffusing capacity of carbon mon

Predictive Value of Echocardiographic Abnormalities and the Impact of Diastolic Dysfunction on In-hospital Major Cardiovascular Complications after Living Donor Kidney Transplantation.

Patients with end-stage renal disease (ESRD) show characteristic abnormalities in cardiac structure and function. We evaluated the influence of these abnormalities on adverse cardiopulmonary outcomes after living donor kidney transplantation in patients with valid preoperative transthoracic echocardiographic evaluation. We then observed any development of major postoperative cardiovascular complications and pulmonary edema until hospital discharge. In-hospital major cardiovascular complications were defined as acute myocardial infarction, ventricular fibrillation/tachycardia, cardiogenic shock

Volume overload and pressure overload due to left-to-right shunt-induced myocardial injury. - Evaluation using a highly sensitive cardiac Troponin-I assay in children with congenital heart disease-.

BACKGROUND: Cardiac troponin I (cTnI) is currently considered to be the most sensitive and specific biochemical marker of acute coronary syndrome and acute myocardial infarction. However, few reports have described the use of cTnI assays for evaluating abnormal hemodynamic load in children with congenital heart disease (CHD). It was hypothesized that significant hemodynamic overload due to a left-to-right shunt induces myocardial injury. METHODS AND RESULTS: A highly sensitive cTnI assay was used to measure the serum cTnI levels in 30 children with atrial septal defect (ASD), 32 children with

Diagnostic Value of Systemic Inflammatory Indices in Myocardial İnfarction With Non-Obstructive Coronary Arteries: Comparison With Obstructive Myocardial Infarction and Non-Critical Coronary Artery Disease.

BACKGROUND: Myocardial infarction with non-obstructive coronary arteries (MINOCA) represents a heterogeneous clinical entity with diverse underlying mechanisms, and its diagnostic evaluation remains challenging in routine clinical practice despite advances in multimodality imaging. Systemic inflammatory indices derived from routine laboratory parameters have recently gained attention as potential markers reflecting inflammatory burden in cardiovascular diseases. This study aimed to evaluate the potential diagnostic utility of systemic inflammatory indices in patients with MINOCA and to compare

A Stepwise Multimodality Imaging Approach to Out-of-Hospital Cardiac Arrest in Cystic Fibrosis.

Out-of-hospital cardiac arrest (OHCA) without ST-segment elevation represents a challenging diagnostic scenario, particularly in young patients with low pretest probability of atherosclerotic disease. We report the case of a 25-year-old man with cystic fibrosis who presented with ventricular fibrillation during a pulmonary exacerbation complicated by hemoptysis and hypoxemia. After successful resuscitation, electrocardiography showed anterolateral T-wave inversions and high-sensitivity troponin demonstrated a marked rise and fall. Transthoracic echocardiography revealed regional wall motion ab

Fatal gelsemium alkaloid poisoning versus cardiovascular cause in a sudden death: a forensic case report.

Gelsemium alkaloids, such as gelsemine and koumine, are highly toxic compounds that can induce rapid respiratory and cardiovascular failure. Their clinical manifestations-dizziness, ptosis, and sudden collapse-closely resemble those of acute myocardial infarction, presenting diagnostic challenges in forensic practice, especially in distinguishing between poisoning and cardiovascular events. We describe a sudden death in a middle-aged male following a restaurant meal. The decedent developed acute dizziness and eyelid drooping shortly after ingestion and died within hours despite resuscitation.

A case report of a giant coronary artery aneurysm masquerading as a ventricular mass.

BACKGROUND: A coronary artery aneurysm is a dilation exceeding 1.5 times the diameter of the patient's largest coronary vessel. They are rare, varying in prevalence between 1.4 and 4.9%. Additionally, they carry a high risk of potential complications, including thrombosis and myocardial infarction, with a risk of rupture. We present an interesting case of a patient with initial imaging suggesting a mass in the right ventricle. CASE SUMMARY: This patient initially presented with acute hypoxic respiratory failure related to pulmonary oedema. His course was complicated by symptomatic ventricular

Acute Coronary Syndrome in Patients with SARS-CoV-2 Infection: Pathophysiology and Translational Perspectives.

Acute coronary syndromes (ACS) may complicate the clinical course of patients with Coronavirus Disease 2019 (COVID-19). It is still unclear whether this condition is a direct consequence of the primary disease. However, several mechanisms including direct cellular damage, endothelial dysfunction, in-situ thrombosis, systemic inflammatory response, and oxygen supply-demand imbalance have been described in patients with COVID-19. The onset of a prothrombotic state may also be facilitated by the endothelial dysfunction secondary to the systemic inflammatory response and to the direct viral cell d

A Rare Complication of Myocardial Infarction: Ventricular Septal Defect.

Ventricular septal defect (VSD) is a rare but lethal complication of myocardial infarction. We present a case of a 65-year-old male who presented with a history of progressive shortness of breath associated with productive cough. Physical examination was significant for crepitation in both lower lung fields and bilateral lower extremity edema. Chest X-ray revealed bilateral reticular opacities with small bilateral pleural effusions. Polymerase chain reaction (PCR) for COVID was positive. Echo showed a left ventricular ejection fraction (LVEF) of 30-35%, ischemic cardiomyopathy, and muscular ve

A critical increase in right-to-left shunt after acute myocardial infarction in a 68-year-old male with tetralogy of Fallot.

A male patient with tetralogy of Fallot accompanied by aortic regurgitation had maintained sufficient exercise capacity for a number of decades with the status of acyanotic tetralogy of Fallot. When he was 67 years old, he suffered a posterior wall acute myocardial infarction and direct percutaneous coronary angioplasty successfully revascularised the target lesion in the left circumflex artery. However, a few months after the onset of the myocardial infarction, his shortness of breath became clinically significant and was associated with increased right-to-left shunt and increased right ventr

A quantitative RT-PCR assay of surfactant-associated protein A1 and A2 mRNA transcripts as a diagnostic tool for acute asphyxial death.

Although an immunohistochemical investigation of pulmonary surfactant-associated protein A (SP-A) suggested a characteristic increase in fatal asphyxiation, no particular change was observed in the total amount of SP-A mRNA. SP-A is encoded by two highly similar genes, SP-A1 and SP-A2, which are differentially regulated in the expression. In the present study, to investigate the molecular pathology of SP-A, we established a method for quantitative RT-PCR assay of SP-A1 and SP-A2 mRNA transcripts. Using this method, fatalities from acute mechanical asphyxia (n=12) and drowning (n=9) were examin

Thrombosis in the young: epidemiology and risk factors. A focus on venous thrombosis.

Thrombosis occurs most often as myocardial infarction, cerebral infarction or venous thromboembolism, ie, deep-vein thrombosis and pulmonary embolism. The incidence of all types of thrombosis is strongly dependent on age. Among young individuals, up to age 40, venous thrombosis is the most common form of thrombosis. The risk factors for arterial and venous thrombosis differ, and among the latter disorders of hemostasis appear to be more prominent. In children venous thrombosis appears almost exclusively in association with venous catheters, with an exception of the renal vein thrombosis of the
