Radiation-induced petrous internal carotid artery aneurysm.

Iatrogenic internal carotid artery aneurysm is a rare complication of irradiation. There are few reported cases in the literature. A case of radiation-induced petrous internal carotid artery aneurysm in a patient with nasopharyngeal cancer treated with radiotherapy is reported. The approach to managing such an aneurysm is discussed.

Relationship between UVB and erythemally weighted radiation.

We discuss the move from reporting damaging UV radiation in terms of UVB to the now widely accepted erythemally weighted UV radiation (UV(Ery)) and the UV Index (UVI). The relationship between these quantities is given: to a good approximation, it is found that UVB(280-315 nm)= 7.55 [times] UV(Ery). In terms of the UV Index, the estimated UVB(280-315 nm) in units of W m(-2) is 18.9 times the UVI. These approximations generally hold to within approximately 10% for all solar zenith angles (sza) less than 70 degrees. For most practical purposes, this is a sufficient range, since for larger sza, t…

Cataracts in retired actinide-exposed radiation workers.

Radiation-induced cataracts are predominantly of the posterior sub-capsular (PSC) type, whereas about 90% of age-related cataracts are of other types. Retired workers, likely to have transuranic body burdens, from three DOE-supported installations were questioned regarding their eye-care history and asked for permission to contact their eye-care providers regarding any cataracts. In 97 cases with lifetime exposure records 20 cases (20.6%) were reported to have PSC cataracts. However, of 24 individuals with recorded lifetime doses of 200-600 mSv, nine (37.5%) had PSC cataracts, compared with 15…

UV radiation-induced immunosuppression and skin cancer.

UV radiation (UVR)-induced skin cancers in mice are highly antigenic and rejected by the host immune system when transplanted to syngeneic recipients. Exposure to UVB radiation causes immunologic changes that inhibit the host immune system from recognizing the tumor and leads to immunologic tolerance. This tolerance involves the appearance of regulatory T cells within the tumor-bearing hosts, which inhibit immunologic recognition of the tumor. Experiments performed with chemical haptens as surrogates for tumor antigens indicate that this comes about because of abnormal antigen presentation. An…

Analyzing cell cycle checkpoints after ionizing radiation.

Several methods to measure cell cycle perturbation after exposure to ionizing radiation are presented in this chapter. These methods include the G1 checkpoint assay by 5' bromode-oxyuridine (BrdUrd) labeling followed by flow cytometric analysis, the S-phase checkpoint assay by measuring DNA synthesis in response to ionizing radiation, and the G2/M checkpoint assay by quantitating histone H3 phosphorylation after ionizing radiation.

Universal thermal radiation drag on neutral objects.

We compute the force on a small neutral polarizable object moving at velocity v--> relative to a photon gas equilibrated at a temperature T. We find a drag force linear in v-->. Its physical basis is related to that in recent formulations of the dissipative component of the Casimir force, i.e., the change in photon momentum in emission and absorption between the moving body and the stationary thermal bath. We estimate the strength of this universal drag force for different dielectric response functions and comment on its relevance in various contexts, especially to radiation-matter coupling in…

[Radiation-induced skin reactions (except malignant tumors)].

The aim of this work, synthesized from personal case reports and a review of literature is to describe cutaneous complications of radiation therapy (except radiation-induced cancers): known and frequent such as radiation dermatitis or less frequent, beginning or strictly limited on irradiated skin areas: acne, infectious diseases, dyskeratosis, Grover's disease, sub-cutaneous pustulosis, cutaneous lichen, morphea, autoimmune bullous dermatosis, subacute cutaneous lupus erythematosus. Furthermore, we try to precise the physiopathogenic mechanisms of these dermatosis and we want to draw the atte…

Low-dose radiation for posttransplant lymphoproliferative disorder.

Initial treatment for posttransplant lymphoproliferative disorder (PTLD) usually involves discontinuation of immunosuppressants. Anti-CD20 monoclonal antibody and/or antivirals are typically employed for persistent disease. Chemotherapy is generally reserved as a final option. The role of radiation therapy, and doses required, have not been well established. Low-dose involved field radiation therapy was used in three pediatric bone marrow transplant (BMT) patients with biopsy-proven PTLD. One patient received a matched T-cell-depleted BMT for dyskeratosis congenita. Two patients with acute mye…

Intensity-modulated radiation therapy (IMRT) for meningioma.

PURPOSE: To assess the safety and efficacy of intensity-modulated radiation therapy (IMRT) in the treatment of intracranial meningioma. METHODS AND MATERIALS: Forty patients with intracranial meningioma (excluding optic nerve sheath meningiomas) were treated using IMRT with the NOMOS Peacock system between 1994 and 1999. Twenty-five patients received IMRT after surgery either as adjuvant therapy for incomplete resection or for recurrence, and 15 patients received definitive IMRT after presumptive diagnosis based on imaging. Thirty-two patients had skull base lesions, and 8 had nonskull base l…

Predictors of weight loss during radiation therapy.

OBJECTIVE: To define risk factors for weight loss or dehydration during radiation therapy (RT). STUDY DESIGN AND SETTING: Retrospective chart review, academic tertiary care center. RESULTS: The incidence of severe weight loss during RT was 32.7%, the incidence of dehydration was 10.9%, and the rate of prophylactic feeding gastrostomy tube placement was 32%. The patients most likely to suffer severe weight loss included patients with tumor sites of nasopharynx and base of tongue, those treated with chemoradiation, and patients with severe pretreatment weight loss. Prophylactic feeding gastros…

Radiation induced valvulitis with late leaflet rupture.

Various cardiac sequelae of mediastinal irradiation have been reported, from pericarditis to conduction defects. Despite the potentially fatal nature of some of these abnormalities, many may present with few or no symptoms. In this case report, the patient, who had received 4000 rads to the mediastinum 24 years previously, presented with worsening shortness of breath and two episodes of lightheadedness. Subsequently, he was found to have aortic valve rupture associated with fibrosis. A review of the literature indicates that valve rupture is a novel consequence of mediastinal radiation.

Gemcitabine-induced radiation recall dermatitis: case report.

A 65-year-old male with lung adenocarcinoma received radiotherapy to the mediastinum and right upper lobe, followed by chemotherapy with gemcitabine. Radiation recall dermatitis developed in the area corresponding to the radiotherapy portal. This is one of just a few cases reported recently concerning radiation recall dermatitis stemming from gemcitabine.

Radiation doses to patients undergoing scoliosis radiography.

In this study we computed the radiation doses associated with scoliosis radiography and investigated how these radiation doses are influenced by the weight of the patient. We recorded the radiographic technique factors of 61 consecutive patients (46 females and 15 males) undergoing scoliosis radiography. A wedge-shaped aluminium filter attenuated the X-ray beam in the "chest region" relative to the "abdomen region". X-ray tube air kerma output factors (microGy mAs-1) and half value layers (HVLs) were determined experimentally for the "chest region" and "abdomen region". The energy imparted to …

Molecular genetic alterations in radiation-induced astrocytomas.

Astrocytic tumors occasionally arise in the central nervous system following radiotherapy. It is not clear if these gliomas represent a unique molecular genetic subset. We identified nine cases in which an astrocytoma arose within ports of previous radiation therapy, with total doses ranging from 2400 to 5500 cGy. Irradiated primary lesions included craniopharyngioma, pituitary adenoma, Hodgkin's lymphoma, ependymoma, pineal neoplasm, rhabdomyosarcoma, and three cases of lymphoblastic malignancies. Patients ranged from 9 to 60 years of age and developed secondary tumors 5 to 23 years after rad…

Radiation-associated thyroid cancer--facts and fiction.

Thyroid cancer is a rare and complex disease and the thyroid contains various cell types from which specific diseases arise. The malignancies range from indolent to extremely aggressive. Several risk factors for thyroid cancers have been suggested, but only ionizing radiation has been found to have a causative effect. With the exception of tobacco, ionizing radiation is probably the most studied carcinogen there is. It is not yet possible to determine whether or not a specific cancer was induced by radiation and epidemiological studies therefore provide the primary data on cancer risk in man a…

Surgical management of radiation-induced heart disease.

BACKGROUND: With the increasing population of patients with prior mediastinal irradiation, cardiac surgeons will encounter patients with radiation-induced damage to the heart and the great vessels. Awareness of the pathology and the surgical management is essential to provide optimal care for these patients. METHODS: Eight patients with radiation-induced heart disease were encountered in the last 10 years. After a brief clinical presentation, the surgical management of radiation-induced heart disease is reviewed. RESULTS: Radiation can affect all the structures in the heart, including the co…
