BACKGROUND: A 46-year-old healthy male Air Force pilot presented to the emergency department (ED) experiencing symptoms of exercise-induced anaphylaxis (EIAn), during a vigorous outdoor run. The patient recovered in the ED and was seen, subsequently, by a civilian allergist; eventually a diagnosis consistent with EIAn was made. Juvenile sterile granulomatous dermatitis and lymphadenitis is a rare immune-mediated skin disease in young dogs. History, signalment, diagnostics, treatment, and outcome in 10 dogs are described. The age ranged from 8 - 36 weeks. The lymph nodes were enlarged in all dogs, especially the mandibular and prescapular lymph nodes. Systemic signs including fever were present in 8 dogs. Seven dogs suffered from blepharitis and painful edema of the muzzle with hemorrhagic discharge, pustules and papules. The pharmacological profile of a new histamine H2-receptor antagonist, 2-(3-(3-(1-piperidinylmethyl)phenoxy)propylamino)-4 (3H)-quinazolinone (NO-794), was studied. NO-794 was a potent and selective histamine H2-receptor antagonist in the guinea-pig atria and gastric mucosal cells. NO-794 (1 X 10(-5) M) did not interact with H1-, muscarinic and beta 1-receptors. Helicobacter pylori is known to be responsible for most cases of chronic gastritis, but its role in the outcome of gastric ulcer is unknown. The purpose of this study was to determine the prevalence of H. pylori infection before and after treatment of gastric ulcer, the micro-organism being untreated. Of 55 horses treated with histamine type 2 (H2) receptor antagonists for gastric ulcers, 32 were examined by gastroendoscopy before and after treatment. Distribution and severity of lesions in the squamous fundus (sf), margo plicatus (mp), glandular fundus (gf), lesser curvature (lc) and cardia (car) were recorded. Retrospectively, samples from 337 newly diagnosed gastric ulcers were studied for the prevalence of fungus using a periodic-acid Schiff-diastase stain. In 64 (19%) gastric ulcers, fungal material was found in the ulcer base. There was no difference in prevalence of fungi between benign gastric ulcers and ulcerating gastric carcinomas. The "fungal" ulcers tended to be larger in diameter and are more often suspected to be malignant than "normal" gastric ulcers. The aim of the study was to identify the clinical markers useful in characterising slow healing and relapsing gastric ulcer patients. Ninety nine subjects entered the short term and 79 the long term study (12 months).
