Background: The purpose of our study was to review a series of patients with scaphoid fractures to determine whether there was an association between lunate morphology and progression to delayed union or nonunion when treated operatively or nonoperatively. Secondary aims included evaluation of the relationship between lunate morphology and scaphoid fracture location. Methods: A retrospective review of all patients with a diagnosis of scaphoid fracture was performed at our institution between 2014 and 2017. Medical records and radiographs were evaluated to determine lunate morphology, scaphoid

Transstyloid, transscaphoid, transcapitate fractures are uncommon. We report the case of a 28-year-old man who sustained this fracture following direct trauma. The patient was successfully treated by open reduction internal fixation of the scaphoid and proximal capitate fragment, with a good clinical outcome at 1-year follow-up. This pattern is a new variant of scaphocapitate fracture as involves a fracture of the radial styloid as well.

Volar perilunar dislocations of the carpus are rare. They occur in young patients following a violent injury. Open reduction is often needed because of instability of the lesion. The authors report a case of volar transscaphoid perilunate fracture-dislocation treated surgically, with a satisfactory functional result after a follow-up of two and a half years.

Recent advances in techniques and implants have led to renewed interest in percutaneous screw fixation of acute scaphoid fractures. The closed (cast) treatment of acute scaphoid fractures generally has good outcome, with bony union resulting; however, closed treatment can result in delayed union, nonunion, malunion, cast- induced joint stiffness, and lost time from employment and avocations. Acute percutaneous fixation of scaphoid fracture has been proposed as a means to minimize some of the complications of closed (cast) treatment. Percutaneous treatment of both nondisplaced and displaced sca

Percutaneous cannulated screw fixation (PCSF) of acute scaphoid fractures has been shown to consistently produce good results. It is less invasive and avoids damage to the radiocarpal ligaments. The scar is good. The procedure could be performed under Bier's block anaesthesia. No cast is required after the operation and the range of wrist motion is regained early. Most recently reported series achieved more than 95% primary radiological fracture union. The functional results were good. The surgical technique of PCSF is described in detail and special tricks for difficult problems are highlight

UNLABELLED: The purpose of this study was to determine the functional, radiographic, and subjective outcome of the authors' technique of four-corner arthrodesis using the en bloc excised scaphoid as the principal donor bone graft coupled with Kirschner wire fixation. The study comprised 40 consecutive patients with progressive Stage II and III scapholunate advanced collapse or scaphoid nonunion advanced collapse deformities. Preoperative and postoperative range of motion, grip strength, carpal height, and Michigan Hand Outcomes Questionnaire responses were assessed with a mean follow-up of 4.4

We reexamined 17 patients at 16 to 57 months (average, 23.4 months) after scaphocapitate arthrodesis. Surgery was performed for treatment of rotary scaphoid instability, isolated arthrosis, resistant scaphoid nonunion, and prevention of carpal collapse in Kienböck's disease. Scaphocapitate fusion with autogenous bone grafting was used to bridge carpal spaces. Two patients had nonunion of the scaphocapitate arthrodesis, which required reoperation. Seven patients experience persistent pain with heavy use; of these, two changed occupations and one remains disabled. Compared with the nonoperated s

BACKGROUND: The scaphoid is the most frequently fractured carpal bone, yet its diagnosis remains a significant clinical challenge. A substantial percentage of non-displaced fractures are missed on initial radiographs, leading to delays in treatment and an increased risk of serious long-term complications such as non-union and avascular necrosis. While advanced imaging like CT and MRI are highly accurate, they are associated with higher costs, radiation exposure (CT), and limited immediate availability. High-resolution musculoskeletal ultrasound has emerged as a rapid, non-invasive, and cost-ef

Fractures of the carpal bones, excluding the scaphoid, are less common and are often missed on standard, plain radiographs. The diagnosis requires knowledge of the anatomy and common fracture patterns of the bones and the specialized radiographic views necessary to image them. Although the hamate hook, trapezial ridge, and pisiform often fracture from direct trauma, other fractures of the carpus may indicate more widespread injury and require a detailed evaluation to rule out associated perilunate or carpometacarpal fracture-subluxations. Fortunately, if treated appropriately, the vast majorit

CASE: A 12-year-old boy with osteopetrosis sustained a scaphoid waist fracture, which has not previously been reported with this condition. Healing was successfully achieved with percutaneous cannulated compression screw fixation, albeit with prolonged healing as is typical for this condition.
CONCLUSION: A lack of osteoclastic remodeling predisposes patients with osteopetrosis to fractures, commonly in long bones. Increased risks of infection, anesthesia, and broken hardware along with limited ability to compress bone may discourage operative management. Internal fixation should still be con

A technique for using an x-ray detectable radiopaque element from a surgical sponge to assess the adequacy of debridement of carpal bone cysts, Kienböck disease (lunate avascular necrosis), enchondroma of the digits, and scaphoid nonunions is described. The technique is simple to perform, inexpensive, and presents minimal additional risk to the patient. The technique has been used over the past 16 years and has enabled adequate debridement of these hand and wrist maladies by radiographically demonstrating the adequacy of the debridement to be bone grafted. It avoids the problems associated wit

Recombinant human bone morphogenetic proteins (rhBMPs) have past a long journey in human orthopaedic surgery during the last 15 years. From the first reports of the use of rhBMPs in hostile environments such as critically-sized bone defects, avascular femoral head necrosis, unstable thoracolumbar vertebral fractures, instability between the atlas and axis due to rheumatoid arthritis; over the use for nonunions of long bones and the scaphoid, reconstructive and revision surgeries of the hip, acute fractures, allograft nonunions, congenital pseudarthrosis, and various approaches of lumbar and ce

Scaphoid fracture nonunion can often lead to pain, arthrosis, and disability. While typically the result of delayed diagnosis or inadequate treatment, it can sometimes occur even if the initial care was timely and appropriate. Whereas early recognition of acute fractures allows for nonoperative management, nonunions frequently require surgical treatment. Traditionally, this has involved open debridement and bone grafting. However, some publications suggest that certain stable nonunions may be amenable to percutaneous debridement and fixation without formal bone grafting. Although certain chara

Advances in MRI technology have greatly improved visualization of the radio-carpal region. With todays high-resolution imaging, even very small anatomic structures like the triangular fibrocartilage may be seen. Established indications include the osteonecroses (scaphoid and lunate); however, MR images always should be evaluated in conjunction with plain radiographs. By delineating bone marrow edema, MRI is well suited for the detection of microfractures. Visualization of small soft tissue tumors, soft tissue extension of bone tumors and for staging of tumors in general may also be regarded as

OBJECTIVE: To systematically review the literature on the performance of ultrasound in diagnosing radiographically occult scaphoid fracture.
METHODS: A systematic search was performed in the MEDLINE and Embase databases. Original studies investigating the performance of ultrasound in diagnosing radiographically occult scaphoid fracture in more than 10 patients were eligible for inclusion. Studies that included both radiographically apparent and occult scaphoid fractures (at initial radiography) were only included if independent data on radiographically occult fractures were reported. Congenital bipartite carpal scaphoid has been reported as an extremely rare congenital anomaly. We report a case of a 47-year-old man who was found with bilateral bipartite carpal scaphoids with no history of injury. Radiographs demonstrated symmetric findings of the distinct bipartition of the scaphoid without obvious degenerative change in both wrists. The patient complained of no other symptoms. The diagnostic differentiation of scaphoid bipartition from the pseudarthrosis of the scaphoid could be important in avoiding unnecessary surgery and inappropriate assessment in compensation cases.
