Evaluation of the capability of a new water lift system to reduce the risk of Schneiderian membrane perforation during sinus elevation.

The purpose of this study was to evaluate the effectiveness of a new water lift system used as a surgical instrument in the crestal approach of the sinus membrane lifting operation and the capability of this technique to reduce the risk of Schneiderian membrane perforation in comparison with a lateral approach using piezoelectric surgery. 50 sinus membrane-lifting operations were performed. Patients were randomized in 2 groups to receive either lateral sinus elevation with piezosurgery or crestal sinus elevation using the new surgical device. Schneiderian membrane perforation was noted in 6 pa

Evaluation of the Stress Levels of Different Implant Treatment Options in Extremely Atrophic Maxillae by Finite Element Analysis Method.

BACKGROUND: Bone resorption progresses in the alveolar ridge after tooth loss. In extremely athropic maxillae, treatment options include quad-zygoma implants, iliac bone augmentation with intraosseous implants, (the All-on-4 concept), Group-SUB periosteal implant. In this study, extremely atrophic maxilla was defined as an edentulous ridge with 3 mm crest thickness, 6 mm nasal base-crest distance, and 5 mm sinus floor-crest distance. AIM: This study aimed to compare stress distribution of these protocols using finite element analysis. METHODS: In this study, analyses were performed using the

A new HA/TTCP material for bone augmentation: an in vivo histological pilot study in primates sinus grafting.

OBJECTIVE: Synthetic calcium phosphate bone substitutes are widely used in sinus graft procedures due to their osteoconductive and biocompatible properties. Hydroxyapatite (HA), beta-tricalcium phosphate (β-TCP), and HA/β-TCP composite are the most applied materials. The aim of this study was to propose a new mineralogical formulation, HA/tetracalcium phosphate (TTCP), as biomaterial for bone regeneration in the maxillary sinus. METHODS: Sinus grafts were performed by using granules of a HA/TTCP blend and a collagen membrane. Bone response at time points of 14 and 17 weeks was histologically

Immediate rehabilitation of the posterior maxilla with extensive sinus pneumatization with one axial and one trans-sinus tilted implant: a 3-year clinical report and a classification.

Implant rehabilitation of the posterior maxilla can entail difficulties due to reduced bone quantity and poor bone quality, especially after long-term edentulism. In some patients, multiple surgeries are necessary, which may lead to higher patient morbidity and longer treatment times before a prosthetic rehabilitation can be achieved. This article presents an immediate fixed prosthesis in a posterior atrophic maxilla supported by 1 anterior axial implant and 1 posterior tilted fixture placed with an intrasinus insertion. Additionally, a classification scheme for immediate treatment for the pos

Histologic and histomorphometric evaluation of an implant retrieved 8 years after insertion in a sinus augmented with anorganic bovine bone and anorganic bovine matrix associated with a cell-binding peptide: a case report.

Few histologic and histomorphometric reports are present in the literature regarding the peri-implant bone response around implants inserted in sinuses grafted with different biomaterials. Anorganic bovine bone (ABB) and anorganic bovine matrix with the addition of an active cell-binding peptide (PepGen P-15) are xenogenic materials that have been reported to present biocompatibility and osteoconductivity. A monolateral sinus augmentation procedure with ABB (50%) and PepGen P-15 (50%) was performed in a 54-year-old man. Two titanium implants with a sandblasted and acid-etched surface were inse

Clinical evaluation of demineralized bone allograft for sinus lifts in humans: a clinical and histologic study.

PURPOSE: Severe alveolar bony resorption in the edentulous posterior maxilla and pneumatization of the maxillary sinus wall make traditional implant placement impossible in the posterior maxilla. To reconstruct the severely resorbed maxilla for dental implant placement, 1 successful technique is to elevate the maxillary sinus floor using demineralized bone matrix (DBM) grafts. The purpose of this study was to evaluate a histologic and histomorphometric evaluation of DBM grafts in the human maxilla. PATIENTS AND METHODS: Nine months after grafting, at the time of dental implantation, biopsy sa

Recombinant human bone morphogenetic protein 2 in augmentation procedures: case reports.

To successfully rehabilitate edentulous patients using endosseous implants, there must be enough available bone. Several techniques have been proposed for augmentation of sites with insufficient bone volume. Although autogenous bone has long been considered the gold standard for such procedures, the limited availability of graft material and a high morbidity rate are potential disadvantages of this type of graft. An alternative is to use recombinant human bone morphogenetic protein 2 (rhBMP-2), which is able to support bone regeneration in the oral environment. These cases demonstrate the appl

Are gingival phenotype, residual ridge height, and membrane thickness critical for the perforation of maxillary sinus?

BACKGROUND: Sinus membrane perforation is the most common complication of the sinus-augmentation procedure. Anatomic factors have been implicated in the risk of sinus membrane perforation (SMP) during sinus-lift surgery. However, there are no reports on the relationships among gingival phenotype (GP), residual ridge height (RRH), membrane thickness (MT), and perforation rate of the maxillary sinus. Therefore, the aim of this clinical study is to determine the associations among GP, RRH, MT, and SMP rate. METHODS: Forty-four patients (64 sinus-lift procedures) were included in the study. Preop

Clinical Outcome and 8-Year Follow-Up of Alveolar Bone Tissue Engineering for Severely Atrophic Alveolar Bone Using Autologous Bone Marrow Stromal Cells with Platelet-Rich Plasma and β-Tricalcium Phosphate Granules.

BACKGROUND: Although bone tissue engineering for dentistry has been studied for many years, the clinical outcome for severe cases has not been established. Furthermore, there are limited numbers of studies that include long-term follow-up. In this study, the safety and efficacy of bone tissue engineering for patients with a severely atrophic alveolar bone were examined using autogenous bone marrow stromal cells (BMSCs), and the long-term stability was also evaluated. METHODS: BMSCs from iliac bone marrow aspirate were cultured and expanded. Then, induced osteogenic cells were transplanted wit
