Anterior exposure of the thoracic spine.

Spinal operation via an anterior thoracic approach is becoming increasingly common, and the thoracic surgeon is now being called upon to provide exposure for orthopedic and neurosurgical colleagues. We report experience with 126 such patients from 1982 through 1993. There were 61 male and 65 female patients (mean age, 39.0 years; range, 14 to 77 years). Indications were trauma in 45 patients (36%), spinal deformity in 42 (33%), cancer in 15 (12%), disc disease in 12 (10%), and infection in 12 (10%). Operative incisions included 22 (17%) right and 14 (11%) left thoracotomies, 33 (26%) right and

Comparative analysis of proximal femoral nail and dynamic hip screw devices for the treatment of unstable trochanteric fractures AO31A2.

PURPOSE: We hypnoses that the DHS with adequate reduction is comparable to PFN as regard functional outcome and offers low cost especially in developing and low-income countries. However PFN has a faster radiological union. Does the DHS and PFN provide comparable clinical and functional outcome for unstable trochanteric fractures? This is the research question. METHODS: This was a retrospective study of unstable trochanteric fractures (AO-classification: 31-A2) that were managed surgically at a university hospital between 2020 and 2023. All patients were scheduled for a follow-up review for a

Small and large titanium plates are equally effective for treating mandible fractures.

PURPOSE: The use of small titanium plates for the management of mandibular fractures continues to be a source of controversy because of their load-sharing properties. The purpose of the present study was to determine whether the use of small plates for mandibular fractures is as efficacious as large plates in a large level I trauma center. MATERIALS AND METHODS: Consecutive subjects presenting with mandibular fractures were randomly allocated to the use of either small plates (group 1) or large plates (group 2). The primary predictor variable was the plate size. The primary outcome variable w

Transmural migration of gastrostomy tube retention discs.

BACKGROUND: Accidental dislodgment is one of the most frequent causes of avoidable cost and consternation related to gastrostomy tubes. The Sacks-Vine gastrostomy tube, inserted in an antegrade fashion by a percutaneous technique, is extremely stable due to the construction of its disc retention device. However, transmural migration of the retention disc is a known severe delayed complication associated with this tube. OBJECTIVE: To review the presentation, diagnosis, and treatment of transmural migration of gastrostomy retention discs, to propose a mechanism for the progressive development o

Tibiotalocalcaneal Arthrodesis: Comparison of Ipsilateral Distal Fibular Autograft as an Intramedullary Strut and Lateral Tibia Plating Versus Intramedullary Nail.

BackgroundVarious techniques using different implants with or without bone grafts have been described in tibiotalocalcaneal arthrodesis (TTCA). However, which reconstructive method and fixation device results in better outcomes remains unclear. The purpose of this study was to compare the surgical outcomes of TTCA performed with ipsilateral distal fibular autograft as intramedullary strut and lateral plating (FAISP) or retrograde intramedullary nail (RIMN).MethodsWe retrospectively reviewed the records of patients who underwent TTCA between 2010 and 2021 performed by a single surgeon. The Functional Outcome of Proximal Femoral Nail Antirotation-2 (PFNA-2) in Peritrochanteric Fractures of the Femur.

Introduction Peritrochanteric fractures are defined as extra-articular fractures involving the trochanter and frequent extension into the subtrochanteric region. These fractures exhibit a bimodal distribution in terms of age. These fractures commonly happen in young individuals who experience high-energy trauma, often in combination with other injuries. In contrast, elderly individuals with osteopenia are more prone to fractures caused by low-energy trauma. Methods This study is a prospective investigation that was carried out over 30 months. The study focused on peritrochanteric fractures tha

Reverse LISS plates for unstable proximal femoral fractures.

The best treatment for unstable proximal femoral fractures is controversial. In this prospective study, we assessed the outcomes of reverse less invasive stabilisation system (LISS) plates for treatment of unstable proximal femoral fractures that are expected to be difficult to nail. From April 2004 to January 2007, 20 patients with unstable proximal femoral fractures that were assessed to be difficult to nail were managed with reverse less invasive stabilisation system-distal femur (LISS-DF) plates, which included (1) subtrochanteric fractures with extension into the piriform fossa, (2) short

Three-dimensional corrective osteotomy of malunited fractures of the upper extremity with use of a computer simulation system.

BACKGROUND: Three-dimensional anatomical correction is desirable for the treatment of a long-bone deformity of the upper extremity. We developed an original system, including a three-dimensional computer simulation program and a custom-made surgical device designed on the basis of simulation, to achieve accurate results. In this study, we investigated the clinical application of this system using a corrective osteotomy of malunited fractures of the upper extremity. METHODS: Twenty-two patients with a long-bone deformity of the upper extremity (four with a cubitus varus deformity, ten with a m

Development of a nanocement platform for site-specific antibiotic delivery in skeletal tuberculosis.

Osteoarticular tuberculosis infection in spine, hip, and knee is the most common bone and joint extrapulmonary tuberculosis (EPTB), accounting for 10-15% of all EPTB cases. The urgent need for new treatment strategies arises from the severe clinical consequences of delayed or misdiagnosed osteoarticular TB infections, compounded by the inadequacies of current antibiotic therapies. Local antibiotic delivery approaches using advanced biomimetic biomaterials as a carrier system provide an efficient alternative. Herein, we develop an injectable, biocompatible nanohydroxyapatite-based carrier syste

Antibiotic-coated intramedullary rod.

Surgical reconstruction for limb salvage is an acceptable and often preferred treatment for patients who are facing potential amputation. Because of the invasive nature of these surgeries, complications such as infection are not uncommon. Osseous infection at the site of implantable hardware may result in a tremendous consequence to the patient. In an attempt to minimize infection with implantable hardware, a local delivery system involving the use of antimicrobial coating on medical devices has been developed. This system optimizes local administration of antibiotics to the sources of infecti

Titanium mesh cages (TMC) in spine surgery.

The introduction of the titanium mesh cage (TMC) in spinal surgery has opened up a variety of applications that are realizable as a result of the versatility of the implant. Differing applications of TMCs in the whole spine are described in a series of 150 patients. Replacement and reinforcement of the anterior column represent the classic use of cylindrical TMCs. The TMC as a multisegmental concave support in kyphotic deformities and as a posterior interlaminar spacer or lamina replacement after wide laminectomy are additional applications. Implant subsidence, pseudarthrosis and implant loose

Anatomic Multiligament Knee Reconstruction With Biceps Femoris Tendon and Medial Meniscal Repair.

BACKGROUND: Multiligament knee injuries are defined as tears of ≥2 of the following ligament structures: anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), posteromedial corner, or posterolateral corner (PLC). These injuries occur in various settings and may be associated with knee dislocations. INDICATIONS: The literature has reported that anatomic technique and a single-;stage approach for treating multiligament knee injuries are optimal because they may lead to improved outcomes. An anatomic double-;bundle PCL reconstruction is suggested to improve knee kinematics compare

Innovative approach to distraction osteogenesis using a novel external fixator design. In vivo experimental rabbit model.

Distraction osteogenesis is an established surgical technique for bone lengthening. The procedure involves making osteotomies, in the bone and using a device to gradually separate, or "distract," the bone fragments. New Zealand white male rabbits underwent distraction osteogenesis on the right tibia and lengthening using external fixators at two different time points: a speed of 1.4 mm/day in 15 days and 2.1 mm/day in 10 days. Radiological analysis was used to determine the distraction osteogenesis rates during tibia lengthening in rabbits. The regenerated callus was carefully removed and subj

Fundoplication with 180-Degree Wrap During Esophagogastrostomy After Robotic Proximal Gastrectomy for Early Gastric Cancer.

BACKGROUND: Compared with total gastrectomy, proximal gastrectomy (PG) has potential advantages from a nutritional perspective, such as anemia and postoperative loss of body weight. However, PG is associated with some postoperative functional disorders, such as reflux esophagus (13-31%) and anastomotic stenosis (3-29%).1 We therefore developed a new procedure for fundoplication during esophago-gastrostomy after robotic PG (RPG). METHODS: We performed RPG for early gastric cancer localized in the upper third of the stomach using the da Vinci Surgical System (Intuitive, Sunnyvale, CA). After RP

Comparing the costs and outcomes of an integrated twin compression screw (ITCS) nail with standard of care using a single lag screw or a single helical blade cephalomedullary nail in patients with intertrochanteric hip fractures.

BACKGROUND: Surgical treatment is the optimal strategy for managing intertrochanteric fractures as it allows for early rehabilitation and functional recovery. The purpose of the study was to assess the cost-effectiveness of commonly used cephalomedullary nails for the treatment of unstable intertrochanteric hip fractures. METHODS: A decision analytic model was developed from a US payer's perspective using clinical data from a pairwise meta-analysis of randomised controlled trials (RCTs) and comparative observational studies comparing the integrated twin compression screw (ITCS) nail versus tw
