Multidisciplinary management of video-assisted thoracoscopic left cardiac sympathetic denervation in a young child with Jervell and Lange-Nielsen syndrome.

Left cardiac sympathetic denervation (LCSD) via video-assisted thoracoscopy (VATS) is an effective therapy for drug-refractory malignant arrhythmias in congenital long QT syndromes and requires meticulous perioperative planning in children with automatic implantable cardioverter-defibrillators (AICDs). We describe what is likely the first paediatric VATS-LCSD performed in Pakistan. A boy in early childhood with Jervell and Lange-Nielsen syndrome, severe QT prolongation and recurrent ventricular arrhythmias despite beta-blockade and mexiletine had received multiple AICD shocks. Intraoperative m

Renal Denervation Effects on Blood Pressure in Resistant and Uncontrolled Hypertension: A Meta-Analysis of Sham-Controlled Randomized Clinical Trials.

BACKGROUND: Although some guidelines recommend Renal denervation (RDN) as an alternative to anti-HTN medications, there are concerns about its efficacy and safety. We aimed to evaluate the benefits and harms of RDN in a systematic review and meta-analysis of sham-controlled randomized clinical trials (RCT). METHODS: Databases were searched until September 10th, 2024, to identify RCTs evaluating RDN for treating URH versus sham control. The primary outcomes were the change in office and ambulatory 24-h systolic (SBP) and diastolic blood pressure (DBP). Secondary outcomes were changes in daytim

Renal nerves in physiology, pathophysiology and interoception.

Sympathetic efferent renal nerves have key roles in the regulation of kidney function and blood pressure. Increased renal sympathetic nerve activity is thought to contribute to hypertension by promoting renal sodium retention, renin release and renal vasoconstriction. This hypothesis led to the development of catheter-based renal denervation (RDN) for the treatment of hypertension. Two RDN devices that ablate both efferent and afferent renal nerves received FDA approval for this indication in 2023. However, in animal models, selective ablation of afferent renal nerves resulted in comparable an

Renal denervation in the management of hypertension in adults. A clinical consensus statement of the ESC Council on Hypertension and the European Association of Percutaneous Cardiovascular Interventions (EAPCI).

Since the publication of the 2018 European Society of Cardiology/European Society of Hypertension (ESC/ESH) Guidelines for the Management of Arterial Hypertension, several high-quality studies, including randomised, sham-controlled trials on catheter-based renal denervation (RDN) were published, confirming both the blood pressure (BP)-lowering efficacy and safety of radiofrequency and ultrasound RDN in a broad range of patients with hypertension, including resistant hypertension. A clinical consensus document by the ESC Council on Hypertension and the European Association of Percutaneous Cardi

Bilateral cardiac sympathetic denervation on an ECMO patient for refractory ventricular arrhythmia: A case report.

Genetic syndrome such as Brugada syndrome can lead to lethal ventricular arrhythmias. Cardiac sympathetic denervation has been shown to be effective in ameliorating refractory ventricular arrhythmias. We present a 33-year-old black female with a medical history of Brugada syndrome with an implantable cardiac defibrillator who presented with refractory ventricular tachycardia leading to cardiogenic shock, requiring extracorporeal membrane oxygenation. The patient subsequently underwent bilateral stellate ganglion sympathetic denervation in the setting of refractory ventricular arrhythmias. We p

Tolvaptan-induced remission of primary palmar hyperhidrosis in a patient with ADPKD: a serendipitous finding.

A 37-year old man had suffered palmar hyperhidrosis since he was fifteen years old. In the last year, he has been treated with tolvaptan for autosomic polycystic kidney disease (ADPKD). The start of tolvaptan therapy was associated with a complete resolution of palmar hyperhidrosis and a sensation of relaxation. During the year on which the patient took tolvaptan, he had to suspend the drug twice. The suspension of tolvaptan was associated with the reappearance of palmar hyperhidrosis followed by sudden remission after the drug reintroduction. Palmar sweating also known as 'emotional sweating'

Thoracoscopic sympathicotomy for the treatment of intolerable palmar and axillary hyperhidrosis in children is associated with high recurrence rates.

BACKGROUND: Treatment of palmar and axillary primary focal hyperhidrosis (PFH) in children up to 16 years using thoracoscopic sympathicotomy is supported by scarce evidence. Therefore, this study aimed to summarize the results of our standardized bilateral, one-stage, single-port sympathicotomy (BOSS) in children up to 16 years of age. METHODS: Consecutive children (n = 14) up to 16 years of age undergoing BOSS between October 2011 and June 2015 in our institution were included in this retrospective study. RESULTS: Recurrence of primary hyperhidrosis was noted in seven patients (50.0%), of w

Recent trends in renal denervation devices for resistant hypertension treatment.

Hypertension is one of the most important risk factors for cardiovascular disease, which is the leading cause of mortality. The World Health Organization (WHO) estimated that in 2019 more than 1.13 billion people worldwide were suffering from hypertension. In spite of the advances in new medical therapies, control of hypertension remains suboptimal. Renal denervation (RDN) neuromodulation treatment was primarily developed to treat resistant hypertension and is potentially a new method for treating congestive heart failure, diabetes, and chronic renal failure. RDN consists of passing a catheter

Deep and future insights into neuromodulation therapies for heart failure.

Major pathophysiology of heart failure is an autonomic nervous system dysfunction as a result of excess sympathoexcitation and/or withdrawal of vagal nerve activity. Although we already have various pharmacological and non-pharmacological therapies for heart failure, survival of heart failure patients remains around 50%. To achieve further reductions in morbidity and mortality of heart failure, neuromodulations with devices, such as baroreflex activating therapy, vagal nerve stimulation, renal sympathetic denervation, spinal cord stimulation, and left cardiac sympathetic denervation, have been
