Periodic fevers and autoinflammatory syndromes in childhood.

Recurrent fever is a common presentation in paediatric practice and can be caused by a wide variety of diseases including autoinflammatory conditions. The innate immune system plays an essential role in the 'first line' response to infection through mediation of inflammatory responses. Inflammasomes are part of the regulatory process for this system and result in the production of the powerful pro-inflammatory cytokine interleukin-1B. Dysregulation of inflammasomes, and Interleukin 1 production, contributes to the pathogenesis of autoinflammatory diseases. This review focuses on described peri

Optic neuritis in a patient with Miller-Fisher syndrome.

Miller-Fisher syndrome (MFS) is considered a variant of Guillain-Barrd syndrome (GBS). The syndrome is characterized by acute onset of gait ataxia, ophthalmoplegia, and areflexia. Conventionally, MFS has been considered exclusively a disease of the peripheral nervous system. However there are occasional reports of central nervous system involvement. Here, a 62 year-old woman who presented with classical features of MFS and progressive bilateral dyschromatopsia and reported visual impairment. Normal MRI of the brain and CSF albumino-cytologic dissociation were observed. CSF oligoclonal IgG patt

Scorpion envenomation and inflammation: Beyond neurotoxic effects.

Scorpion envenomation results in a wide range of clinical manifestations that are mostly attributed to the activation of the autonomic nervous system by venom toxins. In fact, sympathetic and parasympathetic disturbances play important roles during poisoning. However, scorpion venom also induces a complex hyperinflammatory state that occurs parallel to systemic inflammatory response syndrome and acute sepsis. After a scorpion sting, innate immune cells are exposed to the venom molecules, which bind to pattern recognition receptors and activate pro-inflammatory pathways that contribute toward t…

Acute Respiratory Distress Syndrome After Uterine Artery Embolization.

INTRODUCTION: Although uterine artery embolization has been established as a less invasive alternative to hysterectomy or myomectomy for the treatment of uterine leiomyomas, it is not devoid of life-threatening complications. CASE: We present the case of a 44-year-old patient who developed a severe systemic inflammatory response and acute respiratory distress syndrome 5 weeks after uterine artery embolization of a large multileiomyomatous uterus. Deterioration of her clinical status prompted an emergency hysterectomy that led to rapid improvement. CONCLUSION: Although fatalities from septice

Management of Hidradenitis Suppurativa in Special Populations: A Narrative Review.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease that can be treated with a variety of medical and surgical therapies. However, these therapies carry inherent risks that might be heightened in medically complex patients. This narrative review examines literature for the nuances of standard HS management in immunosuppressed individuals, patients with cancer, and those with gastrointestinal or cardiovascular comorbidities, Down syndrome, and PASH syndrome, a rare condition that is characterized by the presence of pyoderma gangrenosum (PG), acne, and HS. The treatment of HS of

Leukocytic ADAM10 and ADAM17 modulate disease severity and systemic outcome in bacterial and viral pneumonia.

BACKGROUND: Pneumonia caused by viral or bacterial pathogens such as severe acute respiratory syndrome coronavirus 2 or Pseudomonas aeruginosa may result in life-threatening disease with a strong contribution of protease dysregulation. The present study aimed to systematically characterise the contribution of ADAM10 and ADAM17 on leukocytes and circulating exosomes to viral and bacterial pneumonia. METHODS: The analysis of coronavirus disease 2019 (COVID-19) and bacterial pneumonia patient samples was combined with in vivo experiments in conditional knockout animals lacking either ADAM10 or A

Finally, β-GlcCer accumulated in the plasma of patients with systemic inflammatory response syndrome, and its levels correlated with the extent of systemic NET formation in these patients. Overall, our results posit β-GlcCer as a potent NET-inducing ligand of Mincle with diagnostic and therapeutic potential in inflammatory disease settings.

Risk Factors for Invasive Pulmonary Aspergillosis in Critically Ill Patients With Coronavirus Disease 2019-Induced Acute Respiratory Distress Syndrome.

Severe coronavirus disease 2019 pneumonia can lead to acute respiratory distress syndrome. Recently, several publications reported on coronavirus disease 2019-associated pulmonary aspergillosis. However, risk factors remain unclear. We retrospectively collected all the cases of coronavirus disease 2019 acute respiratory distress syndrome patients (n = 46) admitted to our 34-bed ICU between March 24, 2020, and May 25, 2020, and identified six patients that met the diagnosis of invasive pulmonary aspergillosis according to previously established definitions. This population exhibited higher seve

Acute-on-chronic liver failure: a new syndrome in cirrhosis.

Patients with cirrhosis who are hospitalized for an acute decompensation (AD) and also have organ failure(s) are at high risk of short-term death. These patients have a syndrome called Acute-on-Chronic Liver Failure (ACLF). ACLF is now considered as a new syndrome that it is distinct from "mere" AD not only because of the presence of organ failure(s) and high short-term mortality but also because of younger age, higher prevalence of alcoholic etiology of cirrhosis, higher prevalence of some precipitants (such as bacterial infections, active alcoholism), and more intense systemic inflammatory r

Acute Endotoxemia-Induced Respiratory and Intestinal Dysbiosis.

Systemic inflammatory response syndrome (SIRS) is a severe condition characterized by systemic inflammation, which may lead to multiple organ failure, shock and death. SIRS is common in burn patients, pancreatitis and sepsis. SIRS is often accompanied by intestinal dysbiosis. However, the mechanism, role and details of microbiome alterations during the early phase of acute SIRS are not completely understood. The current study aimed to characterize the dynamic alterations of both the intestinal and respiratory microbiome at two timepoints during the early phase of acute SIRS (4 and 8 h after LP…

Leptospira interrogans HtrA protease is a potent inducer of multifaceted cellular responses.

Leptospirosis, a bacterial zoonosis caused by select pathogenic spirochetes belonging to the genus Leptospira, can trigger a systemic inflammatory response syndrome due to a cytokine storm that can occur during infection. However, the molecular mechanisms underlying the extensive inflammatory responses during leptospiral infection remain poorly understood. In this study, we characterized a high temperature requirement protein A (HtrA) homologue encoded by the L. interrogans gene locus LIC12812, referred herein as LepHtrA. We used AlphaFold 3 to predict the structure of this protein. And we sho…

Stress, visceral obesity, and metabolic complications.

Stress is a state of threatened homeostasis or disharmony caused by intrinsic or extrinsic adverse forces and is counteracted by an intricate repertoire of physiologic and behavioral responses that aim to reestablish the challenged body equilibrium. The adaptive stress response depends upon an elaborate neuroendocrine, cellular, and molecular infrastructure, the stress system. Crucial functions of the stress system response are mediated by the hypothalamic-pituitary-adrenal (HPA) axis and the central and peripheral components of the autonomic nervous system (ANS). The integrity of the HPA axis

Compartment syndrome-induced muscle injury is diminished by the neutralization of pro-inflammatory cytokines.

OBJECTIVES: Compartment syndrome (CS) is one of the most devastating consequences of musculoskeletal trauma. The pathophysiology of CS includes elevation of intracompartmental pressure (ICP), causing damage to the microcirculation, decreased oxygen delivery, tissue anoxia, and cell death. CS is a combined ischemic and inflammatory condition that induces the systemic inflammatory cascade. In complete ischemia, within the first hour of reperfusion, a peak in the pro-inflammatory cytokine, tumor necrosis factor alpha (TNF-α) has been previously reported. The purpose of this study was to examine t

Demons syndrome with pericardial effusion followed by intensive care unit-acquired weakness: A case report and literature review.

Demons syndrome is defined by hydrothorax and ascites associated with a benign genital tumor that resolves after resection of the tumor. However, Demons syndrome with pericardial effusion has never been reported. Intensive care unit-acquired weakness is a neurological sequela to sepsis/systemic inflammatory response syndrome, or multi-organ failure. A 47-year-old, nulligravid, Japanese woman, was transferred to our hospital for refractory heart failure and a ruptured ovarian tumor. She had an 11-cm left ovarian tumor with ascites, hydrothorax, and pericardial effusion; she was intubated for pu…

A Challenging Case of Recurrent Fever.

Syndrome of undifferentiated recurrent fever (SURF) includes heterogeneous episodes of systemic inflammation without documented infection, without response to antibiotherapy, and characterized by a paucity of specific clinical or molecular criteria. Colchicine is an effective treatment with an impact on morbimortality. We describe a case of a previously healthy one-year-old male, with consanguineous ancestry, admitted four times due to recurrent fever, associated with nonspecific symptoms and an increase of inflammatory markers in a sepsis-like pattern. No consistent infection was documented,

Corticosteroids for the syndrome of hemolysis, elevated liver enzymes, and low platelets (HELLP): what evidence?

The fundamental premise that has governed the proposal relative to the use of corticosteroids for the purpose of disease modification in Hemolysis, Elevated Liver Enzymes, and Low Platelets (HELLP) syndrome was that pre-eclampsia is a condition characterized by an inappropriate maternal systemic inflammatory response and possibly immune-mediated impairment in maternal-fetal communication, while corticosteroids have the capacity to exercise anti-inflammatory and immunosuppressive effects. The present article reviews the evidence behind this proposal, concluding that corticosteroids administrati

Incidence of Post-Implantation Syndrome with Different Endovascular Aortic Aneurysm Repair Modalities and Devices and Related Etiopathogenetic Implications.

BACKGROUND: Postimplantation syndrome (PIS) is a systemic inflammatory response occurring in an early phase after abdominal aortic aneurysm (AAA) endovascular aneurysm repair (EVAR). The pathophysiology underlying PIS is still not well understood. It is speculated that the type of the stent graft or the mural thrombus within the AAA may play a role in determining this inflammatory response. At present, there is no consensus about the influence of PIS on clinical outcomes during follow-up. The endovascular aneurysm sealing (EVAS) with the Nellix sac-anchoring endoprosthesis (Nellix Endovascular

Diagnostic accuracy of NLR, MLR, PLR, SII, and SIRI for predicting major adverse maternal outcomes in preeclampsia: A systematic review.

ObjectivePreeclampsia is a major contributor to maternal morbidity worldwide, and early identification of women at risk of major adverse maternal outcomes is essential. However, commonly used biomarkers are costly and poorly accessible. Complete blood count-derived inflammatory ratios are inexpensive and widely available, but their prognostic value remains unclear. Therefore, this study aimed to evaluate whether complete blood count-derived inflammatory ratios are associated with major adverse maternal outcomes (a diagnostic-prognostic question) and whether they can differentiate women with pr
