Review on mallet finger treatment.

Mallet finger is a common injury involving either an extensor tendon rupture at its insertion or an avulsion fracture involving the insertion of the terminal extensor tendon. It is usually caused by a forceful blow to the tip of the finger causing sudden flexion or a hyperextension injury. Fracture at the dorsal aspect of the base of the distal phalanx is commonly associated with palmar subluxation of the distal phalanx. Most mallet finger injuries are recommended to be treated with immobilisation of the distal interphalangeal joint in extension by splints. There is no consensus on the type of

Non-uniformity in the healthy patellar tendon is greater in males and similar in different age groups.

There is increasing evidence that tendons are heterogeneous and take advantage of structural mechanisms to enhance performance and reduce injury. Fascicle-sliding, for example, is used by energy-storing tendons to enable them to undergo large extensions while protecting the fascicles from damage. Reductions in fascicle-sliding capacity may thus predispose certain populations to tendinopathy. Evidence from the Achilles tendon of significant superficial-to-deep non-uniformity that is reduced with age supports this theory. Similar patellar tendon non-uniformity has been observed, but the effects

One Step Double Augmentation with Human Dermis Allograft and Homologous PRP in Misdiagnosed and or Chronic Achilles Tendon Ruptures.

OBJECTIVE: Misdiagnosed/chronic Achilles tendon injuries are rare and disabling for patients. The surgical treatment of these rare injuries aims to ensure the tendon heals mechanically and biologically. This is the prerequisite for a good clinical and functional outcome and reduces recurrences. The main aim of the study is to present a surgical technique that has proven to be original, reproducible, and capable of guaranteeing solid tendon repair and optimal tissue regeneration. METHODS: We treated five patients, four males and one female, with the one-step double augmentation technique. All …

Upper Extremity Reconstruction in Patients With Cervical Spinal Cord Injury.

BACKGROUND: Regaining hand and upper extremity function is a functional priority for patients with cervical spinal cord injuries (SCI). Despite the high demand for upper extremity reconstruction (UER) to restore function, the utilization of these procedures remains low. This study evaluates the incidence and timing of UER in patients with cervical SCI. METHODS: The TriNetX Research Network was queried to identify patients with cervical SCI. Then, we identified patients who underwent procedures to restore motor function (nerve transfer, tendon transfer, tenodesis, or arthrodesis) and procedure

Comparison of Roll Stitch Technique and Core Suture Technique for Extensor Tendon Repair at the Metacarpophalangeal Joint level.

BACKGROUND: Proper suturing technique is needed to ensure good outcome in extensor tendon surgery. Different techniques have been reported for the repair of extensor tendon injuries at the level of the metacarpophalangeal joint (MCPJ). These reports were in vitro studies on cadaver models. Repair techniques must be clinically tested, to determine results. OBJECTIVES: The purpose of this in vivo study was to compare results of extensor tendon repair, using roll stitch and core suture techniques. PATIENTS AND METHODS: Forty two fingers, in 38 patients (aged 15- 45 years), with simple complete …

The anatomy and morphometry of the proximal semimembranosus muscle tendon - a cadaveric study with clinical implications.

INTRODUCTION: The semimembranosus muscle (SM) is a key component of the posterior thigh and an integral part of the hamstring group. It plays a crucial role in lower limb movement, particularly in knee flexion, hip extension, and medial rotation of the tibia (with the knee flexed), in cooperation with other muscles of the hamstring complex. The aim of this study was to assess the anatomy and morphometry of the SM proximal tendon. MATERIALS AND METHODS: The proximal tendon of the semimembranosus muscle was carefully dissected and examined in 60 cadaveric specimens (32 left and 28 right) obtain

Distinct risk profiles for short-term surgical complications and venous thromboembolism exist among extensor mechanism repair procedures.

PURPOSE: Extensor mechanism injuries, which comprise patella fractures, patella tendon tears and quadriceps tendon tears, are severely debilitating injuries and a common cause of traumatic knee pathology that requires surgical intervention. Risk factors for short-term surgical complications and venous thromboembolism (VTE) in this population have not been well characterised. The aim of this study was to identify perioperative risk factors associated with these short-term complications. METHODS: The National Surgical Quality Improvement Program database was used to identify patients who underw

Patellar Tendon Ruptures.

Rupture of the patellar tendon is a relatively infrequent, yet disabling, injury, which is most commonly seen in patients less than 40 years of age. It tends to occur during athletic activities when a violent contraction of the quadriceps muscle group is resisted by the flexed knee. Rupture usually represents the final stage of a degenerative tendinopathy resulting from repetitive microtrauma to the patellar tendon. This injury may also occur during less strenuous activity in patients whose tendons are weakened by systemic illness or the administration of local or systemic corticosteroid medic

Conservative management of zone II partial flexor tendon lacerations greater than half the width of the tendon.

Over a 5-year-period 15 patients with zone II partial flexor tendon lacerations that were larger than half the width of the tendon were treated conservatively without tendon suturing. Surgical exploration was done with a digital block and the flexor tendons were observed as the patient fully extended and flexed the finger. If present, the cause of triggering was determined and eliminated by trimming any beveled tendon edge, resection of the involved pulleys, and repair of the flexor sheath. Early protected mobilization was started the first day after injury using a dorsal splint. At 4 weeks af

The management of partial extensor tendon lacerations of the hand and forearm: A systematic review.

Injuries to the extensor mechanism of the hand and forearm are common and cause significant functional disability. Complete tendon lacerations are managed with surgical repair, whereas selected partial tendon injuries may be managed without repair but with splinting and physiotherapy alone. There is limited evidence to support the management of partial lacerations, in particular the decision of whether to repair or not. We aimed to systematically review the literature to determine the optimal management of partial extensor tendon lacerations in the hand and forearm. A protocol for the systemat…

Functional Chimeric Double-barrel Fibula and Reinnervated Peroneus Brevis Osteomyocutaneous Flap for One-stage Forearm Reconstruction.

Reconstructing a mangled limb is complex and requires expertise in both bone and soft-tissue reconstruction, particularly when there is significant muscle loss. Typically, multistage surgery is necessary, starting with soft-tissue coverage, followed by bone grafting and tendon transfers. Sometimes, microsurgical techniques such as vascularized bone grafts and free functional muscle transfers are necessary, especially when there is a bone defect of over 6 cm; the soft-tissue environment is infected, scarred, or poorly vascularized; or there are extensive musculotendinous injuries. We treated a

A Novel "Shark-Fin" Pyramidal Achilles Bone-Tendon Allograft Surgical Technique for Delayed Reconstruction of Triceps Tendon Rupture and Review of the Literature.

PURPOSE: Triceps tendon injuries are rare and often caused by direct trauma to the arm. There are no clear guidelines on the management of these and typically partial tears are treated conservatively whilst full thickness tears are treated with primary surgical repair. We aim to review the literature on the methods for triceps repair and propose a novel surgical technique. METHODS: A "Medline" and "Embase" literature search of titles and abstracts combining "triceps brachii muscle," "reconstruct/ed" or "reconstruction" alongside "reconstructive surgical procedures," and further cross referenc…

Characterization of Hand Tendons Through High-Frequency Ultrasound Elastography.

Tendon stiffness plays an important role in the tendon healing process, and many studies have indicated that measuring the shear wave velocity (SWV) on tendons relates to their stiffness. Because the thickness of hand tendons is a few millimeters, high-resolution imaging is required for visualizing hand tissues. However, the resolution of current ultrasound elastography systems is insufficient. In this study, a high-frequency (HF) ultrasound elastography system is proposed for measuring the SWVs of hand tendons. The HF ultrasound elastography system uses an external vibrator to create shear wa

Clinical outcomes after treatment of quadriceps tendon ruptures show equal results independent of suture anchor or transosseus repair technique used - A pilot study.

Biomechanical studies have shown the use of suture anchors (SA) to be superior to the traditional transosseous sutures (TS) in the repair of quadriceps tendon rupture (QTR). This study aimed to analyze and compare the functional outcomes of patients treated for quadriceps tendon ruptures using suture anchors or transosseous sutures. Patients having undergone suture anchor repair or transosseous suture repair for quadriceps tendon rupture between 2010 and 2015 at one of the two participating hospitals were included. Patients from site A underwent TS repair (TS group) while patients from site B

Single-Stage Repair and Reconstruction of Multiligament Injury With Bicondylar Tibial Plateau Fracture Following Traumatic Posterolateral Knee Dislocation: A Case Report.

Knee dislocations (KDs) accompanied by bicondylar tibial plateau fractures and multiligament injuries are uncommon and present significant technical challenges, particularly in patients at high thrombotic risk where tourniquet use is contraindicated. We present the case of a 30-year-old male who sustained a traumatic posterolateral KD with a bicondylar tibial plateau fracture, meniscal and capsular disruption, and patellar tendon and multiligament injury following a high-velocity motor vehicle accident. Due to bilateral lower-extremity deep venous thromboses, surgery was performed without a to
