A 54-year-old woman presented with painless subcutaneous masses on her right hand and spontaneous loss of extension of her ring finger. Surgical exploration of the wrist showed hypertrophic white-colored crystal deposits that both surrounded and invaded into the extensor tendons (intratendinous invasion). Histopathologic examination of the specimen demonstrated findings consistent with gouty tophi. We conclude that tophaceous gout needs to be included in the differential diagnosis of chronic extensor tenosynovitis of the hand.

Achilles allograft interposition is one treatment alternative for failed Darrach distal ulna resections, good results have been reported with its use but concern remains with the stability of the distal ulnar stump. The use of brachioradialis tendon passed through the radius and then divided in to 2 branches to pass over the volar and dorsal aspect of the ulnar stump with the Achilles tendon interposition, seems to represent a more reliable, secure, and biological way of restoring palmar and dorsal stability to the distal ulnar stump; while it allows a better control of the tension given to th

Spastic equinus deformity is common in patients with cerebral palsy. Since 1981, 612 anterior transpositions of the Achilles tendon as described by Murphy have been performed. One hundred patients with at least 30 months' follow-up were selected and retrospectively reviewed. Good correction of the equinus deformity was obtained without the need for prolonged bracing. No patient had a calcaneal gait or recurrence of equinus.

The posterior tibial tendon was rerouted by the technique described by Baker and Hill in 35 feet of children with a dynamic varus deformity due to spastic cerebral palsy. The average follow-up period was 11.4 years. In ten of the feet, rerouting of the posterior tibial tendon was the only procedure performed. Eight of the ten feet obtained a satisfactory correction. There were no overcorrection problems in these ten feet. In the remaining 25 feet, the Baker-Hill procedure was done concurrently with other procedures, such as lengthening of the triceps surae (22 feet), calcaneal osteotomy (two f

Muscle balance involves increasing or decreasing the relative strength of a muscle. It also includes redirection of muscle tendons to change their mechanical retrograde pull. Tendoachilles lengthening, gastrocnemius lengthening, anterior tendon teno-suspension, and the Kidner procedure exemplify this method. Muscle balancing is necessary whenever an imbalance exists. However, muscle balancing is usually only able to mildly effect joint position. It is important to eliminate a deforming muscle force and restore the balance of muscle power. Osteotomy to change internal bone shape is effective in

During the period 1966--75 operation to restore thumb opposition was performed in thirty four patients. Out of twenty seven patients who could be traced, twenty one had obtained a satisfactory result, five had considerably improved, while one had to be classified as a failure. Complications from the operation and the motor tendon are discussed. Among 147 cases with injury to the median nerve treated during the same period, operation to obtain opposition was indicated in 14%.

Sarcopenia is characterized by a progressive loss of muscle strength and physical performance that frequently exceeds the decline predicted by muscle mass alone. This discrepancy indicates that functional deterioration is shaped not only by the force-generating capacity of skeletal muscle, but also by the tissues responsible for transmitting and integrating that force. In this Review, we examine sarcopenia through the biology of the muscle-tendon unit, encompassing skeletal muscle, tendon, and their specialized interface. We discuss how systemic aging processes, including metabolic dysfunction

BACKGROUND: Paralysis of the trapezius muscle most commonly results from iatrogenic injury to the spinal accessory nerve.
METHODS: The clin … l surgical procedure. Rarely, scapulothoracic arthrodesis is considered for individuals with failed tendon transfers or multiple nerve involvement.
CONCLUSION: Trapezius palsy is oftentimes missed. An accurate diagnosis allows consideration of various treatment modalities that have been reported to provide good outcomes for properly selected patients.

A new technique of restored flexion in the elbow joint in an inveterate injury of the brachial plexus is described. The insertion of the long head of the triceps brachii muscle was transferred with an intact nervous and vascular supply to the anterior brachial region and sutured above the radial tuberosity with the insertion tendon of the biceps brachii muscle. The muscle strength three months after surgery according to the muscle test was 4-. Flexion in the elbow joint was possible up to 85 degrees. Extension in the elbow joint was preserved, the muscle strength was 3. Anatomical investigatio

AIMS: The traditional transosseus flexor hallucis longus (FHL) tendon transfer for patients with Achilles tendinopathy requires two incisions to harvest a long tendon graft. The use of a bio-tenodesis screw enables a short graft to be used and is less invasive, but lacks supporting evidence about its biomechanical behaviour. We aimed, in this study, to compare the strength of the traditional transosseus tendon-to-tendon fixation with tendon-to-bone fixation using a tenodesis screw, in cyclical loading and ultimate load testing.
MATERIALS AND METHODS: Tendon grafts were undertaken in 24 paired…

Background Vaughan-Jackson syndrome (VJS) is the attritional rupture of the extensor tendons secondary to arthritis or inflammation affecting the distal radioulnar joint. The surgical outcomes following Darrach's procedure and tendon transfers are not well described. The primary aim of this study was to report patient-reported functional outcome measures after surgery for VJS. Secondary aims were to report health-related quality-of-life scores, patient satisfaction, and complications Methods A single-center retrospective study was performed to identify patients who underwent surgical interve…

Quadriceps muscle and tendon injuries are a significant cause of impairment of the knee extensor mechanism, ranging from minor muscle strains to complete tendon ruptures and extensive defects following oncologic resections. This narrative review provides a comprehensive analysis of contemporary concepts in anatomy, biomechanics, diagnosis, surgical management, and rehabilitation, with a particular focus on reconstructive techniques and functional outcomes. While most muscle injuries respond well to conservative management, complete quadriceps tendon ruptures typically require surgical repair t

Rotator cuff tears are common. When indicated, surgical repair is a highly successful procedure. There are circumstances when there is not enough tendon to perform an anatomic repair because of tear size, retraction, and/or atrophy. This clinical scenario, massive irreparable rotator cuff tear, has no perfect solution. Many options exist in treating massive irreparable rotator cuff tears: partial tendon repairs, debridement, tuberoplasty, intercalary allograft repairs, tendon transfers, superior capsular reconstruction, hemiarthroplasty, arthrodesis, and reverse total shoulder arthroplasty. No…

Tendon transfers are frequently needed to improve hand function in obstetric brachial plexus injuries. The reconstruction cannot always be achieved using local donor transfers in the forearm as these are not always available. In such cases, we propose the use of the brachialis muscle as a useful donor for transfer. Five adolescents with obstetric brachial plexus palsy were operated on to reconstruct wrist extension and/or pronation using the brachialis muscle transfer to the pronator teres (n=1), extensor carpi radialis brevis (n=1) and extensor carpi radialis longus (n=3). Twelve months after…

Nineteen patients with longstanding and permanent brachial plexus injuries underwent a three-staged surgical reconstruction. First, a sural nerve was grafted from the contralateral intact medial pectoral nerve to the paralyzed arm. One year later, a free gracilis muscle was transferred and neurotized by the grafted sural nerve. This procedure results in reanimation of elbow flexion. Finally, the biceps tendon was transferred to the finger flexors using a fascia bundle of tensor fascia lata muscle. Two of the gracilis muscle free transfers failed. In the remaining 17 patients, the overall resul
