Rehab and Death: Improving End-Of-Life Care for Medicare Skilled Nursing Facility Beneficiaries.

Hospitalized older adults with serious illness may be discharged to a skilled nursing facility (SNF) with the hope of improving strength to live independently and receive further disease-directed treatment, or because it is one of the few insurance-covered options for care that provides room and board coverage. For those that do not improve, discharge to a SNF can begin a cycle of costly care transitions between hospital, home with home health care, SNFs, and long-term care facilities, leading to fragmented care and missed opportunities for discussions about preferences for care, illness under

(Dis)-Trust in transitioning ventilator-dependent children from hospital to homecare.

BACKGROUND: Scholarly work is needed to develop the conceptual and theoretical understanding of trust to nursing practice. The transition from hospital care to complex pediatric homecare involves nurses in myriad roles, including management and care provision. Complex pediatric homecare transforms children, families, professionals, and communities, but its exact implications are unclear. RESEARCH OBJECTIVES: To conduct an ethical inquiry into the role and responsibilities of nurses in the qualitative experience of adults involved in the hospital-to-home transition of young, ventilator-depende

Efficacy of a Transition Program in Adolescents and Young Adults With Congenital Heart Disease: The TRANSITION-CHD Randomized Controlled Trial.

PURPOSE: Currently, nearly 90% of patients with congenital heart disease (CHD) reach adulthood in relatively good health. Structured transition programs have emerged to support adolescents and young adults in transitioning to adult care structures, improve their autonomy, and limit healthcare ruptures. The TRANSITION-CHD randomized controlled trial aimed to assess the impact of a transition program on health-related quality of life (HRQoL) in adolescents and young adults with CHD. METHODS: From January 2017 to February 2020, 200 subjects with a CHD, aged 13-25 years, were enrolled in a prospe

Continuous primary care is central to comprehensive cancer care: are we ready to meet growing needs?

Primary care engagement is essential to meet the need for high-quality, comprehensive, long-term cancer care. Primary care currently serves an integral role as the point of care for preventive and surveillance cancer screenings. As cancer prevalence rises, primary care will increasingly serve a growing need for care continuity as patients transition between screening, active treatment, and continued follow-up care.

Transitioning into the Community: Perceptions of Barriers and Facilitators Experienced By Formerly Incarcerated, Homeless Women During Reentry-A Qualitative Study.

Formerly incarcerated, homeless women on parole or probation experience individual-and structural-level barriers and facilitators as they prepare to transition into the community during reentry. A qualitative study was undertaken using focus group methods with formerly incarcerated, currently homeless women (N = 18, Mage = 37.67, SD 10.68, 23-53 years of age) exiting jail or prison. Major themes which emerged included the following: (1) access to resources-barriers and facilitators during community transition, (2) familial reconciliation and parenting during community transition, and (3) traum

Development of health support services for adults with spina bifida.

PURPOSE: To describe, within a state-wide transition framework, the pathway to improved health support services for adults with spina bifida. Our aim was to achieve adult care that addressed adult health issues, despite significant budgetary restraints and competition for the public health dollar. METHOD: A pilot clinical service project in one of the Area Health Services and a transition workforce study were carried out in the state of NSW in Australia. The advocacy skills of clinicians were harnessed. RESULTS: A regional adult spina bifida clinic was successfully created and patient resour

Asthma in middle schools: what students have to say about their asthma.

Preadolescence involves cognitive, social, and physiological changes along with changes in the child's environment. During this developmental stage, young adolescents are transitioning into middle school, forming a larger social network, and managing parental expectations for assuming more responsibility for self-care. The impact of these developmental changes on asthma management is not well understood. The purpose of this study was to better understand asthma and asthma management from the perspective of middle school students. A partnership was formed between the university researcher, seve

Navigating Adulthood with Maple Syrup Urine Disease: Patient and Caregiver Perspectives on Healthcare Transition and Independent Living.

BACKGROUND: Medical advances in diagnosis and management have increased the life expectancy of many pediatric-onset rare diseases, including inherited metabolic disorders (IMDs). Historically fatal childhood conditions are now manageable chronic illnesses, with over 90% of individuals with IMD reaching adulthood. As more individuals with IMDs reach adulthood (age 18 years), there is a new imperative for health care transition planning that promotes developmentally-appropriate skills for medical self-management and independent living. Knowledge gaps on the long-term disease-related outcomes lim

When military-to-civilian transition support falls short: A qualitative assessment of transition needs among U.S. Veterans.

Many federal agencies and nonprofit organizations support military service members transitioning back to civilian life. Yet, veterans still report that many of their transitioning needs are not being met. Studies aimed at assessing the needs of veterans during the transition process largely include homogenous samples that lack the diverse gender, racial, and ethnic identities found among current and future populations of veterans. The current study interviewed 50 post-9/11 veterans and oversampled for women and racial and ethnic minority veterans to examine potential gaps in transition support

Advance Care Planning for Patients Approaching End-Stage Kidney Disease.

Patients with chronic kidney disease typically suffer a cascade of comorbid conditions, the magnitude of which have formidable impact on advance care planning (ACP). Complex health care decisions are complicated further by contextual issues that may change over time. A dynamic and evolving process, ACP ideally begins early in the continuum of chronic kidney disease, long before end-stage kidney disease is reached. Planning ahead for care is preparatory to making decisions about kidney replacement therapy and can make for a smooth transition in addition to preventing the start of dialysis by de

Youth in transition: an exploratory comparative analysis of outcomes for youth placed in kinship and non-kinship foster homes.

Despite our improved understanding of kinship care, little is known about its benefits and challenges for transition-age youth. This study compared the experiences of youth in kinship and non-kinship foster homes, using data from National Youth in Transition Database (NYTD) and Adoption and Foster Care Analysis and Reporting System (AFCARS). Logistic regression analyses showed that youth in kinship and non-kinship foster homes were similar in educational attainment and risks of homelessness. However, youth in kinship foster homes had higher risks of incarceration. The study observed the signif

[Digital applications in the treatment of adolescent patients with borderline personality disorder].

BACKGROUND: Borderline personality disorder (BPD) is often manifested during adolescence and is associated with substantial morbidity and mortality. Early disorder-specific treatment is considered state of the art but is hampered by discontinuities at the transition to adult services and limited resources. Digital technologies play an increasing key role in adolescents' daily lives. OBJECTIVE: Presentation of key characteristics of adolescent BPD and psychiatric transition-focused care concepts, an overview of digital applications in BPD treatment and the possibilities and challenges in the c

Tracheostomy Outcomes in Muslim Intensive Care Unit Patients with Acute Respiratory Failure: Highlighting a Critical Palliative Care Gap.

BACKGROUND: Tracheostomy often marks the onset of prolonged critical illness with high mortality, yet outcome data from Muslim-majority settings are unavailable. METHODS: We conducted a retrospective cohort study (2019-2023) of Muslim intensive care unit (ICU) patients undergoing tracheostomy for acute respiratory failure. The primary outcome was in-hospital mortality; secondary outcomes included ICU mortality, code-status transitions, and one-year survival. RESULTS: Among 411 patients (median age 68 years; 55% male), 68% were hospice-eligible at admission, while 38% were bedridden. In-hospi

Changes in the quality of care during progress from stage 1 to stage 2 of Meaningful Use.

BACKGROUND: The Centers for Medicare and Medicaid Services (CMS) canceled Meaningful Use (MU), replacing it with Advancing Care Information, which preserves many MU elements. Therefore, transitioning from MU stage 1 to MU stage 2 has important implications for the new policy, yet the quality of care provided by physicians transitioning from MU1 to MU2 is unknown. METHODS: Retrospective longitudinal evaluation of the quality of care delivered by outpatient physicians at an academic medical center in the transition between MU1 and MU2. RESULTS: Between MU1 and MU2, 4 measures improved: hyperte

Bridging the gap: an integrated paediatric to adult clinical service for young adults with kidney failure.

PROBLEM: Transition from paediatric to adult care of young adults with chronic diseases is poorly coordinated, often delayed, and usually managed through a single referral letter. About 35% of young adults lose a successfully functioning kidney transplant within 36 months of transfer from paediatric to adult services. DESIGN: Before and after study of the impact of a new integrated paediatric-adult clinical service for patients with kidney failure. SETTING: Adult renal centre in Oxford and two paediatric renal centres in London. STRATEGIES FOR CHANGE: An integrated paediatric-young adult jo

Perceived Readiness and Barriers to Self-Management in Pediatric Atopic Dermatitis Patients and Parents.

BACKGROUND: Adolescents and young adults (AYA) with atopic dermatitis (AD) must transition from caregiver-led to self-managed care and eventually to adult healthcare providers. While transition readiness is well studied in other chronic conditions, it remains underexplored in AD. This study assesses transition readiness and perceived barriers to self-management among AYA with AD and parents of AYA. METHODS: A cross-sectional, online survey was conducted from June to July 2024 by the National Eczema Association among US-based AYA (ages 12-22) with AD and parents of AYA with AD. Transition read

Nursing dying people in an acute hospital: A focused ethnography.

BACKGROUND: Hospitals are the most common place of death worldwide yet dying patients' needs are frequently unmet. Acute wards are oriented toward cure, efficiency and discharge, creating conditions in which recognition of dying and transitions to comfort-focused care are often delayed. Nurses are central to end-of-life care but face moral, cultural and institutional constraints that shape how they can respond. RESEARCH QUESTION: How does the culture of the acute hospital setting shape how nurses manage the transition to comfort-focused care for dying people? DESIGN: Focused ethnography, und
