Changes in Transcript, Metabolite, and Antibody Reactivity During the Early Protective Immune Response in Humans to Mycobacterium tuberculosis Infection.

BACKGROUND: Strategies to prevent Mycobacterium tuberculosis (Mtb) infection are urgently required. In this study, we aimed to identify correlates of protection against Mtb infection. METHODS: Two groups of Mtb-exposed contacts of tuberculosis (TB) patients were recruited and classified according to their Mtb infection status using the tuberculin skin test (TST; cohort 1) or QuantiFERON (QFT; cohort 2). A negative reading at baseline with a positive reading at follow-up classified TST or QFT converters and a negative reading at both time points classified TST or QFT nonconverters. Ribonucleic

The diagnosis of latent tuberculosis infection (LTBI): currently available tests, future developments, and perspectives to eliminate tuberculosis (TB).

INTRODUCTION: Despite great efforts, tuberculosis (TB) is still a major public health threat worldwide. For decades, TB control programs have focused almost exclusively on infectious TB active cases.  However, it is evident that this strategy alone cannot achieve TB elimination. To achieve this objective a comprehensive strategy directed toward integrated latent tuberculosis infection (LTBI) management is needed. Recently it has been recognized that LTBI is not a stable condition but rather a spectrum of infections (e.g., intermittent, transient or progressive) which may lead to incipient, the

Identification of Mycobacterium tuberculosis Peptides in Serum Extracellular Vesicles from Persons with Latent Tuberculosis Infection.

Identification of biomarkers for latent Mycobacterium tuberculosis infection and risk of progression to tuberculosis (TB) disease are needed to better identify individuals to target for preventive therapy, predict disease risk, and potentially predict preventive therapy efficacy. Our group developed multiple reaction monitoring mass spectrometry (MRM-MS) assays that detected M. tuberculosis peptides in serum extracellular vesicles from TB patients. We subsequently optimized this MRM-MS assay to selectively identify 40 M. tuberculosis peptides from 19 proteins that most commonly copurify with s

Diagnosis and treatment for ocular tuberculosis among uveitis specialists: the international perspective.

PURPOSE: To assess the approach of international specialists, who primarily practice in tuberculosis-endemic areas, to ocular tuberculosis (TB). METHODS: International experts from India, Brazil, Taiwan, and more than 10 other countries were surveyed using two clinical cases and general questions. RESULTS: A total of 244 experts were sent a survey about the treatment and diagnosis of ocular tuberculosis; 65 responded (27%), of whom 34 were affiliated with practices in India, while 31 primarily practice at international sites outside of India and North America. The data from this survey were

Clinics of ocular tuberculosis.

PURPOSE: Ocular tuberculosis is an extrapulmonary tuberculous condition and has variable manifestations. The purpose of this review is to describe the clinical manifestations of ocular tuberculosis affecting the anterior and posterior segments of the eye in both immunocompetent and immunocompromised patients. METHODS: Review of literature using Pubmed database. RESULTS: Mycobacterium tuberculosis may lead to formation of conjunctival granuloma, nodular scleritis, and interstitial keratitis. Lacrimal gland and orbital caseating granulomas are rare but may occur. The intraocular structures are

Community-wide INH treatment of latent TB infection in a BCG-vaccinated population: experience in rural Taiwan.

SETTING: A contiguous four-village bacille Calmette-Guérin (BCG) vaccinated population (n = 1785) in an aboriginal township in Taiwan with a very high tuberculosis (TB) burden. OBJECTIVE: To report our experience in this first community-wide effort in Taiwan to treat latent tuberculosis infection (LTBI) in household contacts of recently diagnosed TB cases. RESULTS: Overall, 71 LTBI cases (88.8%) completed varying case-specific isoniazid treatment courses, accounting for 4.0% of the affected community (village range 2.4-7.5%, P < 0.001). CONCLUSION: LTBI in household contacts was successfull

[The evaluation of pulmonary tuberculosis patients enrolled to Eskisehir Deliklitas Tuberculosis Control Dispensary].

Tuberculosis is an important public health problem in our country. Tuberculosis Control Dispensary has been important role in tuberculosis control programme. In this study we retrospectively evaluate 891 pulmonary tuberculosis patients who registered in Eskişehir Deliklitaş Tuberculosis Control Dispensary between January 1990 and June 2000. The mean age was 38.9 +/- 15.7 years, 159 (18.3%) patients were female, 710 (81.7%) patients were male. The most common symptom was cough (88.7%). 81 percent of patients have been found by examination of symptomatic people while 6.4 percent of them by exami

[Intracranial tuberculous arachnoiditis: report of a case].

The case of a 28-year-old male affected by intracranial tuberculous arachnoiditis with unusual aspects is reported. The patient presented bilateral amaurosis and complete ophthalmoplegia with intrinsic muscle sparing in both sides. The routine laboratory findings showed only persistent leucocytosis and a Mantoux test of 10 mm. Several radiologic studies of the lungs were always normal. The spinal fluid, tomography of the cranial base and carotid angiography were normal. The diagnosis was made by histologic study after craniotomy.

Isoniazid pharmacokinetics in kwashiorkor.

Isoniazid plasma half-life and clearance rates were studied in children with kwashiorkor before and after nutritional rehabilitation. Isoniazid half-life rates fell and clearance increased with nutritional rehabilitation. The changes documented in this report are not an indication that isoniazid therapy should be altered in malnourished children, but may indicate that this should be done in malnourished adults.

Lymphocyte functions in Wegener's granulomatosis.

Lymphocyte function was examined in Wegener's granulomatosis patients and the following results were obtained. 1) The T and B lymphocyte subpopulation was within the normal range. 2) Lymphocyte blast formation was examined with the use of PHA-P, Con A or TAP (tuberculin active peptide). A decrease in responsiveness against TAP was found in four out of ten patients, but no decrease was observed in responsiveness agains Con A or PHA-P. 3) Skin tests were carried out on seven patients with six different antigens. In two out of seven patients, the skin reactions were decreased against all the anti
